    Notes: Objective To describe the sonographic appearance of normal anal sphincter anatomy and sphincter defects evaluated with a conventional 5 MHz convex transducer placed on the perineum.Design Prospective, single-blind study.Setting Department of Obstetrics and Gynecology, University of Michigan Medical Center, USA.Population Twenty-five women with symptoms of faecal incontinence, 11 asymptomatic nulliparous women, and 32 asymptomatic parous women.Methods A convex scanner was placed on the perineum with the woman in lithotomy position. Images were taken at three levels of the sphincter canal. Pictures were evaluated by two examiners who were blinded to the case history of the women and to the results of each other for the presence or absence of sphincter defects.Main outcome measures Description of anal sphincter appearance on endoanal ultrasound. Reproducibilty of the evaluation of sphincter defects.Results The internal anal sphincter is visible as a hypoechoic circle; the external anal sphincter shows a hyperechoic pattern. Proximally the sling of the puborectalis muscle is visible. Sphincter defects were detected in 20 women. In all five women who subsequently underwent surgery, the presence and location of the defect was confirmed at the time of surgery. Examiners were in agreement 100% of the time on the presence or absence of internal defects. They disagreed in one patient on the presence of an external defect.Conclusion Exoanal ultrasound provides information on normal anatomy and on defects of the anal sphincter.

    Notes: We observed a dramatic improvement in the performance of polymer light-emitting diodes (LEDs) upon light doping of the organic layer. The LEDs betrayed symmetrical electrical and light-emission characteristics. Their turn-on voltage is lower and their external quantum and power conversion efficiencies are higher by nearly an order of magnitude when compared with devices that utilized a nominally undoped organic layer. We attributed these results to the modification of the tunneling barrier in metal–polymer–metal junctions due to the presence of an induced polarization electric field associated with the ionized dopant counterions and charged polymer chains. © 1995 American Institute of Physics.

    Notes: Potasium (Kx x〈40) and sodium (Nax, x〈66) clusters were generated in molecular beams and probed by photoionization mass spectroscopy. Results obtained include measurements of neutral cluster abundances and determinations of ionization potentials. The I. P. values can be rationalized in terms of a global electrostatic model which extrapolates to the bulk work function. This model also applies to transition metals as can be shown by comparng it to the limited experimental data sets available. The I. P.'s of small clusters exhibit "quantum size'' effects which can be understood by individual quantum chemical calculations. As previously found for sodium clusters, photoionization mass spectra obtained for potassium, mixed potassium/sodium and potassium/lithium exhibit abundance maxima at M+8 and M+20, where M is an alkali metal. This has been interpreted in terms of increased thermodynamic stability of the corresponding neutrals relative to neighboring clusters. We present data which show that a spherical jellium model, while providing a set of numbers correlating well with those of preferred stability in alkali clusters, is less successful in explaining other properties.

    Notes: The device stability of blue light emitting diodes based on N,N′-diethyl-3,3′-bicarbazyle, (EtCz)2, are substantially improved by coevaporation with N,N′-diethyl-3,3′-bicarbazyle-6,6′ dicarbaboxylic acid, (OcCzCOOH)2. We attribute this effect to the formation of a network of hydrogen bonds in the alloy related to the presence of carboxyl groups. A three layer device with transport layers for both electrons and holes improves the quantum efficiency: With an Al cathode we obtain a luminance of 100 cd/m2 at current density levels of 50 mA/cm2. We find that the current voltage characteristics of (EtCz)2 is well described by our recent microscopic model for space charge limited currents in an assembly of conjugated polymer segments. The incorporation of an electron transport layer splits and shifts the electroluminescent band to lower energies and we observe a significant additional downshift after a period of three weeks which we attributed to molecular diffusion at the interface between the emitting and electron transport layers. © 1998 American Institute of Physics.

    Notes: Fluid and electrolyte imbalances are frequently encountered in a variety of medical and surgical disorders therefore requiring the clinician's astute evaluation of his/her patient's fluid and electrolyte status. Disorders involving the gastrointestinal tract, kidneys, and endocrine organs will frequently cause abnormalities in sodium, potassium, magnesium, and chloride balance. An accurate initial assessment of serum electrolyte concentrations and monitoring during the treatment is essential in order to maintain electrolyte homeostasis. Failure to do so can lead to severe clinical consequences for the patient and change a diagnostic triumph into a therapeutic failure.     Notes: Objective: This case report describes antivenin-associated acute and delayed hypersensitivity reactions in a dog envenomated by an Eastern diamondback rattlesnake (Crotalus adamanteus), specifically reviewing the syndrome of antivenin-associated serum sickness. Clinician awareness of this syndrome is important in order to allow for its recognition and appropriate treatment.Case summary: A Boxer dog was envenomated by an Eastern diamondback rattlesnake. Shock, echinocytosis, and coagulopathy were manifested, and the dog was given antivenin crotalidae polyvalent therapy and supportive care. The early onset of an anaphylactoid reaction was attributed to antivenin therapy and was managed with diphenhydramine and subcutaneous epinephrine therapy. Fever, chemosis, and limb edema occurred during the 3rd through 6th hospital days following antivenin therapy and were consistent with serum sickness syndrome as described in humans. Further immunoassay support reflecting complement activation and response to treatment were characteristic.New information provided: To our knowledge, this describes the first reported case of antivenin-associated serum sickness in a dog.

    Notes: Acute hypoadrenocorticism in the dog is a true medical emergency that requires a prompt and accurate diagnosis and appropriate treatment in order to provide for the most optimal outcome. Because the signs of adrenal insufficiency mimic those caused by other more common clinical conditions such as renal failure, intoxications, and various gastrointestinal disorders, it behooves the emergency clinician to always include this condition in the list of differential diagnoses.
