    Description / Table of Contents: Zusammenfassung In den letzten Jahren ist durch die Quinacrine-Fluorescenzmethode und die Giemsa-Bänderverfahren eine Identifizierung sämtlicher Chromosomen des Menschen möglich geworden. Dabei hat sich auch gezeigt, daß mindestens 10 von den 23 Chromosomenpaaren zum Teil sehr häufige Variabilitäten aufweisen, die, da sie in die Erbträger integriert sind, notwendigerweise vererbt werden. Die Frequenzen dieser Polymorphismen würden, auch bei Verwendung nur der am leichtesten zu erkennenden Strukturverschiedenheiten, beim Vaterschaftsnachweis eine Ausschlußwahrscheinlichkeit von ca. 70% ergeben.
    Notes: Summary The recent quinacrine fluorescence analysis and the Giemsa banding methods allow identification of all human chromosomes. When human chromosomes were studied by using these methods, at least 10 of the 23 chromosome pairs showed polymorphism occuring at a relatively high frequency. Since these features are carried by the chromosomes they are necessarily inherited. The frequency of these variabilities is high enough to allow exclusion of paternity in affiliation cases at a likelihood of at least 70%.

    Notes: Abstract The influence of methylprednisolone on the migratory characteristics of neutrophil granulocytes was investigated in 10 patients with early rheumatoid arthritis (RA) and compared to 12 controls. The migration of neutrophils was measured with a whole-blood membrane filter assay with and without stimulation by the chemoattractant N-formyl-methionyl-leucyl-phenylalanine (fMLP). Total migration index (TMI), distribution characteristics (DC) and the product of TMI and DC (neutrophil migratory activity; NMA) served to characterize the migratory behaviour of neutrophils. The data demonstrated an increased polymorphonuclear leucocyte (PMN) migration in patients with early RA, indicating a bystander role of PMNs in inflammatory joint injury. Treatment with methylprednisolone reduced significantly the penetration depth (DC) of neutrophils, but did not influence the number of migrating cells (TMI). The unstimulated NMA was significantly reduced due to the marked DC reduction, whereas steroids did not influence the stimulated NMA of neutrophils. A significant reduction in PMN penetration depth was demonstrated only after a steroid therapy of at least 10 days, suggesting that a longer period of steroid therapy is necessary to provide effective inflammatory control.

    Notes: Summary It has been suggested that ultrasonic lithotripsy causes soft tissue damage by cavitation processes, by DNA alterations at subcavitational ultrasound levels, by thermic and mechanical trauma from the probe and from metal fragments splintered off the sonotrode. These issues were investigated in vitro and in vivo in the leporine bladder. Ultrasound intensity levels measured at the tips of sonotrodes never exceeded the cavitation threshold, so that cavitation processes are not involved in ultrasonic lithotripsy. The incidence of sister chromatid exchange in growing human lymphocytes was not altered, so that mutagenic effects appear unlikely. Thermic damage by the sonotrode is reliably prevented by irrigation cooling with 20 ml/min saline (20°C); higher flow rates are necessary for adequate vision in clinical practice. In spite of vibration amplitudes of up to 50 μ, the sonotrode causes only mucosal abrasion and submucosal hemorrhage; lesions of deeper layers of the bladder wall were never observed. Metal fragments are splintered off the sonotrode and nephroscope by contact, and this can largely be prevented by teflon coating of the wands. The fragments were washed out of leporine bladders within 24 h when instilled into the bladder cavity, and within 7 days when rubbed into the mucosa. In conclusion, significant soft tissue damage by ultrasonic lithotripsy was not observed. This corresponds to clinical experience with technique in 518 kidneys.

Glucose linkage to HbA1c reflects mean blood glucose levels during the last 3 months before examination. Various methods for HbA1c determination show abnormal values with hemoglobin variants. In some diabetic patients excessively high HbA1c values with high-performance liquid chromatography (HPLC) led to the detection of Hb Graz. In addition to Hb Graz, other silent hemoglobin variants have been found in Austria. Here we review Hb Graz, Hb Sherwood Forest, and Hb Okayama detected while using HPLC for the measurement of HbA1c in diabetic patients.
