    Notes: Summary After the experimental demonstration of split-products of intact immun-globulins of the middle ear secretion the level of the intact immunglobulins IgG, IgA and IgM has been measured pre- and postoperatively. By these studies it could be demonstrated that particulary in cases of chronic otitis media associated with cholesteatoma the level of the intact immunglobulins is much more higher postoperatively than preoperatively. The liberation of serum proteinase inhibitors by injuring the bloodvessels during a tympanoplasty combined with appropriate antibiotic therapy causes a stabilisation of the inhibitory and immunological system. It is the purpose of further studies to inform about the capability of the split-products of the immunglobulins to attach and to absorb antigens and toxins during a chronic inflammation in the middle ear.

    Notes: Summary In purulent secretion of mucous membranes complexes between proteinases and α 1-Antitrypsin, respectively the low molecular weight inhibitor, which is specific for mucous membranes are present. The complexes were enriched and separated by chromatographical methods. The proteinases bound to the inhibitors we could identify as leucocytic proteinase by use of a monospecific Antiserum against leucocytic elastase, produced in our laboratory.

    Notes: Die Hörmeßverfahren bei Kindern können nach verschiedenen Kriterien systematisiert werden: nach dem Lebensalter, nach der Art der Reizantworten, nach der Art der Prüfsignale und nach der klinischen Fragestellung. Die gängige Einteilung nach dem Lebensalter, die auch hier verwendet werden soll, unterscheidet Testverfahren bei Neugeborenen, Säuglingen, Kleinstkindern, Kleinkindern und Schulkindern. Die Kinderaudiometrie hat zunächst zwei Zielsetzungen: 1. Aussonderungsuntersuchungen (Screeninguntersuchung): Aus der großen Anzahl aller Kinder sollen diejenigen “ausgesiebt” werden, die nicht normal reagieren und bei denen somit der Verdacht auf einen Hörschaden besteht. 2. Bestimmungsuntersuchungen: Besteht bei dem Screeningtest der Verdacht auf eine Hörschädigung, dann werden anhand spezieller subjektiver und/oder objektiver Testverfahren die Art und der Grad der Schwerhörigkeit ermittelt.

    Notes: Ohrmuschelfehlbildungen sind ein großes psychisches Problem, zunächst für die Eltern und mit zunehmenden Alter auch für die betroffenen Kinder. Die Eltern werden geplagt von Schuldgefühlen über ein mögliches Fehlverhalten während der Schwangerschaft und sind verunsichert über die weitere Entwicklung ihres Kindes. Ohrdeformitäten sind deutlich sichtbar und führen zu einer frühzeitigen Stigmatisierung der Patienten, die zunehmenden Hänseleien ihrer Umgebung ausgesetzt sind [5, 25]. Hilfesuchend wenden sich die Eltern in vielen Fällen an den Pädiater, von dessen Beratung der weitere Abklärungsprozess und die Behandlung der Fehlbildung stark abhängig ist.

    Notes: Abstract 2H-NMR spectra of perdeuterated octadecanoic acid (C18AC) and dotriacontane (C32AN) added to isolated and subsequently recrystallized cuticular wax from barley (Hordeum vulgare L.) leaves were recorded between 298 and 328 K. They were compared to calorimetric excess heat capacity profiles. The NMR-data revealed the presence of both an isotropic and a rigid wax component at temperatures below 313 K. At temperatures above 318 K all labels are in the fast motion regime indicating a transition in the host matrix of the labels. The presence of the surfactant C6E3 reduced the order of the C18AC-label but did not influence the order of the long chain alkane label (C32AN). Most surprisingly, calorimetry revealed that most thermotropic events take place above the apparent melting observed in NMR. Furthermore, the macroscopic softening and melting of the wax took place in the same temperature regime as in the calorimetric experiments. The excess heat capacity traces were complex and indicated a heterogeneous structural composition of the barley wax. We interpreted the apparent conflict between the NMR and the calorimetric results by assuming a crystalline host matrix, formed by C26-alcohol, the major molecular component of the wax. Within the crystal compartments there may exist an amorphous matrix with some crystalline microdomains of other wax components, including the NMR-labels. The melting of the amorphous environment leads to fast motional narrowing of the NMR spectral line of the microdomains without melting of the macroscopic structure. The measurements of diffusion coefficients (D) of radiolabelled C18AC and C32AN gave additional insight into the microstructure of the way architecture. Identical results in terms of D were obtained when radiolabelled C18AC was added to the wax from either the exterior after recrystallization or when it was recrystallized together with the wax. It is concluded that in both cases the radiolabelled molecule is located in an amorphous wax phase, which forms a percolating path through the wax and is thus also accessible to the surfactant C6E3. In contrast, D of C32AN in barley wax was about 2400 times higher when C32AN was added to recrystallized wax from the exterior compared to wax samples recrystallized together with C32AN. This indicates that in the case of C32AN the alkane is trapped within separate microdomains of the wax during recrystallization and thus it remains essentially immobile, whereas it possesses a high degree of mobility when it is added from the exterior where it has only access to the percolating amorphous wax phase.

    Description / Table of Contents: Abstract By gelfiltration and ionexchange chromatography, proteinase bound proteinase inhibitor, specific for mucous membranes was isolated from purulent sputum of patients with obstructive airway diseases. The isolated material contains 25% complexed inhibitor. The complex interacts with specific anti-inhibitor-serum and anti-inter-alpha-trypsininhibitor-serum. With anti-leucocytic-proteinase-serum an interaction is observed, too. The proteinasepart of the complexed inhibitor therefore is a leucocytic proteinase.
    Notes: Zusammenfassung Durch Gelfiltration und Ionenaustauscher-Chromatographie wird aus purulentem Sputum von Patienten mit chronisch obstruktiven Atemwegserkrankungen an Proteinasen gebundener schleimhautspezifischer Proteaseninhibitor zu 25% Reinheit angereichert. Der Komplex reagiert immunologisch mit spezifischem Anti-Inhibitor-Serum und Anti-inter-alpha-Trypsininhibitorserum. Mit Anti-Leukozytenproteinaseserum erfolgt ebenfalls eine immunologische Reaktion. Die in Frage kommenden Bindungspartner des Inhibitors in vivo sind somit im wesentlichen leukozytäre Proteinasen.

    Notes: Summary It is possible, to determine exactly the frequency resolution by simplified psychoacustical tuning-curves. Although the frequency-selectivity is very neccessary for the speech discrimination it is neglected in the audiology. In patients with different diseases with inner ear deafness tuning-curves are measured and compared with the speech discrimination-loss in Freiburger speechtest.

    Notes: Summary The application of hearing-aids in cases of atresia of the external ear is problematical. In the past usually the children were fitted by body-aids or by ear level bone-conduction hearing-aids. Now we prefer a fitting more aesthetical and effective. Several examples will illustrate the possibilities with in-the-ear aids and behind-the-ear aids. Usually there is no one best fit that is demonstrably best under all circumstances. The fixation has to be performed individually according to the anatomical local situation.

    Notes: Abstract The pathogenesis of sudden hearing loss (SHL) is still not well understood. Possible causes include increased blood viscosity, microthrombosis or altered blood flow. Hypercholesterolemia, hyperfibrinogenemia and increased platelet aggregation are frequently observed in patients with SHL. The aim of this study was to investigate whether drastic lowering of plasma cholesterol and fibrinogen by selective extracorporal apheresis could have a beneficial effect on hearing recovery in these patients. Seven patients with SHL were treated with an extracorporal procedure removing fibrinogen and cholesterol from plasma. Six of the seven patients showed an immediate improvement in auditory thresholds following a single treatment of heparin-induced low-density lipoprotein precipitation. These findings indicate for the first time that acute and drastic removal of plasma fibrinogen and low-density lipoproteins may be an effective clinical method for the treatment of patients with SHL.
