    Notes: Abstract Fifty-one patients with early glaucoma and 29 patients without the disease had their contrast sensitivity, colour vision, differential light threshold, neuroretinal rim area, retinal nerve fibre layer, pattern-reversal electrogram and visually evoked potential examined. A stepwise discriminant analysis found that the combination of the vertical cup disc ratio, the diffuse nerve fibre layer score and the localized nerve fibre layer score correctly identified 98% of the normals and 84% of the glaucoma patients. Single parameters and various combinations of parameters were also examined for their ability to discriminate between the two groups. Fifty-two glaucoma suspects were similarly examined and were classified into those whose discriminant functions were normal or abnormal.

We used [18F]6-fluorodopa (FD) positron emission tomography (PET) to examine the severity of nigrostriatal dopaminergic dysfunction in 67 patients with Idiopathic Parkinsonism (IP), 52 with fluctuations and 15 with a stable response to levodopa. FD uptake (Ki) was reduced by 12% in the caudate (p = 0.08) and by 28% in the putamen (p = 0.0004) of patients with fluctuations compared to those with a stable response. However, there was considerable overlap of FD Ki values between the two groups. The fluctuators had a longer symptom duration (11.6 ± 5.7 years) than the patients with a stable response to levodopa (4.3 ± 2.4 years; p 〈 0.0001) and the age of onset of symptoms was earlier in the fluctuators (43.9 ± 8.9 versus 54.1 ± 10.4; p = 0.0004). Similar reductions in FD Ki in the fluctuators persisted following adjustment for these variables (7.5% in the caudate and 26% in the putamen; p = n.s. and 0.007, respectively). When smaller groups (n = 15 each) were matched for duration of symptoms, the reduction in caudate Ki in the fluctuators was only 1.9% (p = n.s.), but there was still a 24% reduction in putamen Ki (p = 0.05). These findings suggest that fluctuators and non-fluctuators may differ in the severity of their nigrostriatal damage and provide modest support for the hypothesis that fluctuations may in part reflect altered "buffering" capacity of dopaminergic nerve terminals. However, the considerable overlap between groups suggests that other factors such as altered postsynaptic mechanisms and/or increased turnover of dopamine may make a substantial contribution to the development of motor fluctuations.

    Notes: Abstract The sensitivity of the visual field declines with age. The density of the crystalline lens increases with age and may be partly responsible for the decline in sensitivity. To determine whether lens opacity influences the mean threshold of the visual field, 82 normal subjects were enrolled. All had their lens opacity measured with the Interzeag Lens Opacity Meter 701 and the visual field assessed with the Ring Resolution Perimeter. Lens opacity was highly correlated with age, while mean visual field threshold was also correlated with age but showed much greater variation. Multiple regression failed to show any influence of lens opacity measurements on the mean field threshold after age had been accounted for.

    Notes: Summary Tendon reflexes were examined in 119 patients with idiopathic parkinsonism (IP) and 40 spouse controls to estimate the type and frequency of any alterations in the reflexes. Forty one of 119 patients and 2 of 40 controls had reflex ratings of 3+ at two or more sites (p〈0.001). There was no correlation of reflex score with the severity of disease or with the cardinal signs of IP. In 21 patients with asymmetric tendon jerks the side with the more active reflexes correlated with the side with greater parkinsonian signs. We conclude that an increase in tendon jerks is a feature of IP. The pathophysiology of this change in reflexes should be investigated further to establish if it is a heretofore overlooked manifestation of basal ganglia dysfunction or a link with other neurodegenerative diseases.

    Notes: Abstract Computerized pallor measurements of the optic nervehead were performed by global analysis with the Rodenstock Disk Analyzer. Total intervideographic and intravideographic variability was calculated. The coefficients of variation ranged from 8.3% to 20.08% for intervideographic variability of different pallor histogram values. The intravideographic variability for pallor histogram values ranged from 0.82% to 2.94%.
