    Notes: The testicular gland (t.g.) is a glandular tissue situated adjacent to the testis of blenniid and several gobiid species. In the present study the t.g. of Blennius pavo Risso and Gobius niger L. were compared by histological and histochemical methods. In B. pavo the spermatozoa have to cross the t.g. to reach the vas deferens and thus they come into contact with the gland cells, whereas in G. niger the vas deferens is situated between the testis and the t.g. The fine structure and histo-chemistry of the t.g. cells reveal that in B.pavo the cells of the t.g. have exocrine as well as endocrine functions. The t.g. cells of B. pavo contain large amounts of lipids, form a secretion containing acid mucopolysaccharides, show positive reaction for acid phosphatase, and some cells stain for 3β-HSD and G6PD. The function of the t.g. of G. niger is exclusively endocrine. Characteristics of the gland cells of this species are well developed smooth ER and tubulovesicular or paracristal-line mitochondria. The stainings for 3β-HSD, G6PD and UDPGD give strong positive results in the whole t.g., indicating the presence of steroids and steroid glucuronides.

    Notes: Summary Purpose Comparison of the effect of different therapeutic modalities on survival time of patients with glioblastoma multiforme operated on during the last decade (1980–1990). Patients and methods The records of 157 consecutive patients with the histological diagnosis of glioblastoma multiforme were analysed for survival with respect to age of patients, extent of surgery, influence of re-operation and adjuvant postoperative treatment. The latter included fractionated radiotherapy, chemotherapy (BCNU, CCNU with Vincristine) and photodynamic therapy (PDT). Results Analysis of variance showed a significant effect for survival after macroscopically radical surgery (p=0.005), postoperative radiotherapy (p〈 0.001), chemotherapy (p〈 0.01). Low age (p〈 0.05) and a postoperative Karnofsky performance score (KPS) ⩾60 (p〈 0.001) had a positive influence; the site of tumour and pre-operative presence of seizures had no significant influence (p 〉 0.1) on survival time. Conclusion We conclude that the current adequate management of glioblastoma multiforme should include surgical resection followed by adjuvant treatment such as radiotherapy and chemotherapy.

    Notes: Summary The testicular gland of Blennius pavo Risso, 1810 is an accessory sex organ situated ventrally, adjacent to the testis. Sperm have to cross it to reach the vas deferens. Changes in the fine structure of the gland were monitored monthly over a period of 1 year. The annual cycle is closely correlated to the reproductive cycle. Maximum synthetic activity and cell size are reached during late prespawning and early spawning. At that time the number of lipid vacuoles increases markedly. Vesicles of smooth endoplasmic reticulum ER, numerous Golgi cisternae and tubulovesicular mitochondria which are characteristics of steroid producing cells can be observed. Secretory vesicles are synthesized in the apical region of the cells and then released into the ducts of the gland. The content of lipids is maintained until the resting period. The supranuclear region of the cells contains accumulations of lamellar bodies that probably derive from lysosomal vacuoles. Fine structural characteristics suggesting the function of the testicular gland are discussed.

    Notes: Summary The fine structure of the seminal vesicles ofZosterisessor ophiocephalus, the annual changes during the reproductive cycle, and the composition of the secretion are investigated by electron microscopy and by enzyme, polysaccharide, and protein histochemistry. The seminal vesicles are accessory glands of the male reproductive system which produce sialoglycoproteins during the spawning period. They show a significant annual cycle. During the postspawning period they have a regressive phase, followed by a regeneration phase during interspawning and a proliferation phase during prespawning. Possible functions of the seminal vesicle secretion are discussed.

    Notes: Summary. Deep brain stimulation (DBS) of the subthalamic nucleus (STN) with a quadripolar electrode was carried out in 9 patients with advanced idiopathic Parkinson's disease (PD) affected with severe diurnal motor fluctuations. The effect of bilateral STN stimulation was evaluated by clinical methods in all patients after 3 and 12 months. Assessment was based on the Unified Parkinson's Disease Rating Scale (UPDRS), timed motor tests, the Schwab and England Activities of Daily Living and a diary chart to document motor fluctuations. Alterations in parkinsonian signs, motor performance and functional outcome were recorded postoperatively (1) under temporary complete withdrawal of both STN stimulation and medication; (2) in the presence of STN stimulation only; and (3) in the presence of both STN stimulation and medication. The results were compared with the preoperative data assessed in defined on-phase and defined off-phase. STN stimulation on (compared to STN stimulation off) results in a significant improvement in UPDRS motor scores: after 3 months from 50.5 ± 14.3 to 27.8 ± 5.8, and after 12 months from 49.4 ± 14.1 to 27.1 ± 7.1 (p 〈 0.01). There was a significant decrease in the average duration of off-periods from 8.82 ± 2.47 hours to 1.00 ± 1.06 hours (p 〈 0.001), a marked increase in on-periods without dyskinesia from 4.62 ± 2.72 to 14.62 ± 1.51 hours (p 〈 0.01), and a sharp drop in on-periods with dyskinesia from 2.87 (± 4.18) to 0.25 (± 0.97) hours (p 〈 0.05), which remained stable up to 12 months (off-periods: 1.25 ± 1.58 hours, p 〈 0.001; on-periods without: 13.87 ± 1.95 hours, p 〈 0.001; and on-periods wth dyskinesia: 0.37 ± 1.06 hours, p 〈 0.05). However, our first PD patient with an implanted DBS electrode within the STN died from cardiac infarction two days after surgery. This sudden death was not linked either to surgery nor to stimulation – and happened by chance. Our findings confirm that STN stimulation is a suitable functional neurosurgical procedure for the modulation and control of PD signs associated with severe motor fluctuations, in that they demonstrate a beneficial effect which was fully sustained over a one year follow-up period.
