    Notes: Elective orthopaedic surgery is regularly withheld from patients with haemophilia and high inhibitor titre despite the presence of severe arthropathy and urgent medical need. A knee joint arthroplasty was performed in a patient with severe haemophilia A and a high inhibitor titre using recombinant factor VIIa (rFVIIa) as the sole coagulation factor. There was no abnormal bleeding during surgery although an increased blood loss through surgical drains did occur during the first 6 h postoperatively. Rehabilitation was started on day 1 and continued for 3 months. Walking commenced on day 4. After 1 year of follow-up, the clinical outcome of surgery was considered excellent with no pain, knee mobility at 0–5-90°, and an International Knee Society score of 95/100. No rFVIIa-associated side-effects or thrombotic complications were reported. In conclusion, knee joint arthroplasty is now an option for haemophilia patients with a high inhibitor titre. An international review of all available data on elective orthopaedic surgery in inhibitor patients is required so that the optimal treatment regime can be defined and the short- and long-term risk–benefit ratio of surgery compared to that of noninhibitor patients.

    Notes: Many nail abnormalities have traditionally been described in association with rheumatoid arthritis (RA), but their specificity has never been assessed in a controlled study. Our purpose was to evaluate the frequency and the specificity of nail changes associated with RA in a case-controlled study including 50 patients suffering from RA and 50 controls. For each patient, a general skin examination was performed and the 20 nails were examined. The nail features were noted and classified. Ax2 test or a Fisher test was used to compare the two groups. The only nail abnormalities significantly associated with RA were longitudinal ridging on nine or 10 finger nails (29 patients in the RA group vs. three in the controls. X2: p〈0.001) and clubbing on at least one nail (24 patients vs. 10.X2:p〈0.01). Other nail changes were noticed but were not frequent enough to be significant. The presence of longitudinal ridging on the finger nails was significantly associated with RA.

    Notes: Abstract  Percutaneous injection of methylmethacrylate is now increasingly employed to treat bone lesions, both malignant and benign. However, the risks of this procedure are still to be fully established. In this report, we describe a case of rapid chondrolysis appearing after the intra-articular leakage of cement during injection of an acetabular subchondral cyst, resulting in hip replacement. Although the mechanism of such chondrolysis is unknown, this event suggests a chondrolytic effect of the acrylic cement. Thus, it is essential to systematically search for the presence of an intra-articular passage before injecting bone cement into a peri-articular cyst. This unusual complication highlights the need for rigorous evaluation of the benefits and risks of percutaneous injection of acrylic cement in the treatment of benign lesions, especially close to an articulation.

    Notes: Poly(vinyl chloride) and chlorinated poly(vinyl chloride) films were saturated at 100°C with fluorotrichloromethane (Genetron-11, a foaming agent) and then allowed to desorb at RT and 65°C. The desorption data for a three month period was fitted into a mathematical relationship which could be used for long term predictions of the G-11 retention in the films. Using thermoanalytical techniques, plots of glass transition temperature vs G-11 concentration were made for the polymers under study. These plots were used to demonstrate the effect of stabilizer, polymer type and temperature on the desorption of the G-11.

    Notes: Extrusion experiments on thirteen polymers in our laboratory and extrusion and forging studies on nine polymers in other laboratories reveal that solid-state extrusion and malleability occur in semi-crystalline polymers that have a crystalline relaxation, αc at the temperature Tαc, and do not occur in semicrystalline polymers devoid of αc. The solid-state extrusion takes place in the temperature range of Tαc ≤ T 〈 Tm with Tm being the melting point. In this temperature, range, the polymeric crystals show a measure of disorder, observed by several techniques and by small heats and entropies of fusion. A survey of the literature indicates that, in general, polymers which have a αc and are solid-state extrudable have shallow and broad bottoms to the energy-wells describing their most stable crystalline conformations, and polymers showing no αc have steep-walled energy-wells for the same conformations.

    Notes: A number of techniques have been used in the past for obtaining the infrared spectra of fine fibers. A new technique has been developed which overcomes many of the problems of previous methods. The present technique involves winding a single filament between two salt windows which are separated by a spacer which is the same or slightly greater in thickness than the fibers. This results in a layer of adjacent parallel fibers. Good-quality polarized infrared spectra were obtained on oriented poly(ethylene terephthalate) and oriented nylon 6 fibers with the new technique. The data demonstrate the high level of “A” conformation orientation present in highly drawn nylon 6 fibers and the existance of chain folds in drawn poly(ethylene terephthalate) fibers. The structural changes which occur during the shrinkage of nylon 6 fibers are also demonstrated.

    Notes: The product of the thermal polymerization of ß-carboxymethyl caprolactam has been identified as a poly[(2,6-dioxo-1,4-piperidinediyl)trimethylene]. An indication of various chain conformations has been explained by the possible existence of two different conformations of the dioxopiperidine moiety. One conformation pertains to an equatorial position of the trimethylene moiety with respect to the plane of the imide group. The other is derived from a structural unit in which the trimethylene group is positioned axially to the plane of the ring. While the former conformation has been ascribed to the crystalline modification the latter appears to be the predominant structure of the polymer in formic acid solution as indicated by NMR analysis. In the glassy state the macromolecule is assumed to be composed of segments of either conformation. The unoriented linear polymer crystallizes upon annealing in positively birefringent spherulites. A triclinic unit cell with the following parameters: a = 9.64 A., b = 11.32 A., c = 15.80 A., α = 98°, β = 96°, γ = 114° has been found to contain eight structural units. The calculated density, 1.30 g./cm.3, agrees with experimental data. A crystal structure is proposed on the basis of the x-ray data.
