    Notes: Recombinant factor VIIa (rfVIIa) has been widely used for the treatment and prevention of bleeding episodes in haemophiliacs with high-titre inhibitors. High single doses are the treatment of choice for joint and muscle bleeds in those patients. There are only a few reports on the value of rfVIIa in cirrhotic patients with haemostatic impairment but this drug can consistently correct the prothrombin time in these individuals. We report a case of a good response to a single high dose of rfVIIa in a patient with advanced liver disease who suffered from severe refractory postoperative haematuria.

    Notes: The combination of interferon (IFN) and ribavirin is the current gold standard for treatment of chronic hepatitis C virus (HCV) infection with sustained remission rates of 35–40% being achieved in haemophilic patients. A similar beneficial effect of this combined therapy has been suggested even for patients with compensated liver cirrhosis and some authors have reported a possible role for IFN and ribavirin in the prevention or delay in the development of hepatocellular carcinoma (HCC), a well known complication of HCV infection in haemophiliacs. The absence, due to design difficulties, of definite randomized controlled clinical trials remains a handicap for the routine use of specific therapy of HCV infected patients with the aim of preventing HCC. A discussion of these important issues has been performed in this paper.

    Notes: Background It has been suggested that intestinal microbiota of allergic and non-allergic children differs in composition, and that microbiota–immune system interactions may predispose children to develop sensitization. Previous studies have examined fecal microbiota of allergic children with atopic dermatitis, but little is known about that of atopic wheezy children.Objective To investigate the composition of the fecal microbiota of young sensitized wheezy and non-sensitized non-wheezy children, using molecular methods.Methods Within the context of a prospective birth cohort, we carried out a nested case–control study of sensitized wheezy children (cases) and non-sensitized non-wheezy controls. Cases and controls were matched for age, sex, parental atopy, allergen exposure, and pet ownership. We evaluated the composition of fecal microbiota by nucleic acid-based methods (PCR combined with denaturing gradient gel electrophoresis and quantification of bifidobacteria by fluorescent in situ hybridization).Results Thirty-three case–control pairs (mean age 4.4 years) provided stool samples. Comparison of total bacterial community profiles showed that each child had a unique fecal microbiota (mean Dice's similarity coefficient 22%, range 3.3–60.8%). There was no difference between the groups in prevalence of Lactic Acid bacteria (12/33 vs. 11/33, P=0.8) or bifidobacteria (30/33 vs. 31/33, P=1.00, cases vs. controls). The bifidobacterial species detected were similar in both groups. The percentage of bifidobacteria in total fecal microflora was no different between cases (median 1.7%, range 0–20.8%) and controls (1.9%, 0–18.2%, P=0.7). However, cases with eczema had significantly fewer bifidobacteria (median 1.6%, range 0–4.8%) than their controls (4.0%, 1.9–18.2%, P=0.05).Conclusion We found no differences in fecal microbiota composition between sensitized wheezy and non-sensitized, non-wheezy children aged 3–5 years using nucleic acid-based methods. Differences appear to be isolated to those allergic children with eczema.

    Notes: Abstract Endoscopic retrograde cholangiopan-creatography (ERCP) was carried out on 19 patients with hydatid cyst of the liver, either before or after operation. Cholangiographic findings included biliary tract obstruction in 8 cases, intrinsic compression in 4 cases, laminated membranes or daughter cysts in the biliary tree in 3 cases, perforation into the biliary tract in 2 cases, stricture in the biliary ductal system in 1 case, and biliary fistula in the remaining case. Biliary tract obstruction was secondary to chemical cholangitis by formalin in 2 patients. ERCP is a useful procedure in patients with suspected intrabiliary rupture or when symptoms occur after surgical removal of the cyst.

    Notes: Abstract Insulin binding in adipocytes from patients with a phaeochromocytoma (PH) approached that of the controls (C) at low and higher concentrations of unlabeled insulin. The apparent receptor affinity was unchanged (ED50: PH 0.50×10−9M and C0.60×10−9M). Scatchard analysis of the binding data using the negative cooperative model revealed a 46% decrease in the total number of receptors together with no changes in both K−e (PH 0.55×109M−1 and C 0.36×109M−1) and K−f (PH 0.13×109 M−1 and C 0.07×109 M−1). According to the two site model, an altered proportion in the two classes of insulin binding sites was detected. This was accompanied by a catecholamine-desensitization of the adipocytes to the antilipolytic action of insulin. These events could represent a final situation of a chronic and endogeneous regulation by high levels of catecholamines of insulin receptors in human adipose tissue.

    Notes: Abstract Human adipocytes from patients with chronic endogenous hypercortisolism (Cushing's syndrome) showed a statistically significant decrease in insulin binding at low unlabelled-insulin concentrations but no change in receptor numbers (Cushing's 180,000±48,000 (3) receptors/cell and controls 189,000±30,000 (7)) together with a fourfold decrease in apparent receptor affinity (ED50: Cushing's 2.25×10−9 M and controls 0.57×10−9 M) and a decreased sensitivity to the antilipolytic effect of insulin. These events could represent the final situation of a chronic and endogenous regulation by high levels of cortisol of insulin receptors in human adipose tissue.

    Notes: Highly plasticized poly(vinyl chloride) (PVC) membranes (200 per hundred resin [phr]) form the basis of one class of ion-selective electrodes (ISEs). In previous work on the mechanical properties of membranes, the optimal ratio of plasticizer level employed to minimal level required for complete plasticization (phrexp/phrmin) was found to be 2.0. The current study was designed to determine whether this ratio is necessary or sufficient for proper ISE function. Dynamic mechanical analysis (DMA) was used to examine the effects of five plasticizers on the dynamic mechanical properties of membranes at three frequencies (110, 11.0, and 1.1 Hz) as a function of temperature (-100°C to +100°C): dicotyl sebacate (DOS), epoxidized propylene glycol dioleate (PGDO), ortho-nitrophenyl octyl ether (o-NPOE), epoxidized soybean oil (ESO), and expoxidized linseed oil (ELO). The glass transition temperature of PVC, which was found to be +77.1°C at 11.0 Hz, was depressed by the addition of 200 phr of each plasticizer from a high of -1.4°C (PGDO at 110 Hz) to a low of -70.2°C (DOS at 1.1 Hz). DMA and electromotive-force (EMF) measurements on membranes plasticized with o-NPOE through a range of phrexp/phrmin from 0.5 to 9.3 indicated that a “transition window” occurs between phrexp/phrmin of 2.0 and 3.3 in which the membranes change from minimally plasticized polymer films to predictable ion-selective membranes, coinciding with the optimal mechanical properties observed previously. Based on dynamic mechanical properties and EMF response data, the optimal phrmin/phrmin ratios for membranes as a function of plasticizer were proposed: 0.8 for ESO and ELO, 1.3 for PGDO, 1.7 for DOS, and 3.0 for o-NPOE.
