    Notizen: This article presents an outcomes review of breast cancer patients identified from the cancer registries of four area hospitals. These patients had family histories of breast cancer, ovarian carcinoma, or both and were treated with conservative surgery and radiation to the involved breast. Patients were as follows: group 1, one first-degree relative ( n = 165, one synchronous bilateral breast cancer); group 2, ≥2 first-degree relatives (n = 21); group 3, one second-degree relative (n = 20); and group 4, ≥2 second-degree relatives (n = 18). The total of patients and breast cancer events was 224 and 225, respectively. Group 5 was a subgroup of 53 patients with a substantial risk (〉10%) of a BRCA1 or BRCA2 mutation. After a median follow-up of 3.9 years, 5 patients had local failure (2%), and 5 developed a contralateral breast cancer (2%). There were no significant differences in local failure rates between groups (p = 1.0): group 1, 5 of 166 (3%); group 2, 0 of 21 (0%); group 3, 0 of 20 (0%); and group 4, 0 of 18 (0%). Local failure for group 5 was 2% (1 of 53). Four of 143 patients (3%) with a minimum 3 years of follow-up (median, 5.6 years) had local failure, and 5 (4%) developed a contralateral breast cancer. A univariate analysis was statistically significant for differentiation only (well, 0 of 67; moderately, 1 of 57 [1.8%]; poor, 3 of 26 [11.5%], p = 0.008). Overall survival for groups 1–4 did not differ significantly. Although follow-up has been relatively short, we have not found that breast cancer patients with various degrees of family histories of breast/ovarian carcinoma have had a detrimental outcome when treated with conservative therapy.

    Notizen: Weaving the seamless web of care: an analysis of parents’ perceptions of their needs following discharge of their child from hospital Changes in children’s nursing services in the United Kingdom in recent years have focused on the need for adequate and efficient services to be provided in the interests of the child. Early discharge is now the norm and children are often sent home in an earlier recovery stage than adults with comparable conditions. Whilst the contributions of paediatric community nursing services have gone some way to providing specialist nursing care for children and their families in their own homes, the majority of children are discharged home without such support being available. This may place an overwhelming responsibility of caring for a recovering child onto parents. The purpose of the qualitative research reported in this paper was to identify any gaps in nursing services for acutely sick children and their families following discharge, and to suggest ways to improve integration and communication between hospital and primary care to facilitate a ‘seamless web of care’ for families. Families were surveyed following discharge (n=164) and interviews carried out with those experiencing problems (n=20). General practitioners were also surveyed for their opinion as key contributors of primary care. Findings revealed the isolation felt by parents following discharge, with their need for information about a child’s illness and expected recovery, and for reassurance and specific advice through some means of support, which was clearly not being met. The perceived benefit of continuity of care was a common theme, with both parents and professionals acknowledging the importance of closer liaison between hospital and primary health care services. This study is valuable in providing preliminary qualitative information regarding the gaps in children’s nursing services and how these can be overcome by using our present resources more imaginatively, in order to ensure the delivery of cost-effective and quality health care services in the best interests of local need.

    Notizen: Through simulation we investigate how characteristics of forced distribution rating systems (FDRS), which require firing a certain percentage of the workforce each year, might interact with ratings reliability, selection validity, selection ratio, and voluntary turnover to improve average workforce performance potential. Results suggest that a FDRS could lead to noticeable improvement in workforce potential, that most of the improvement should be expected to occur over the first several years, and that improvement is largely a function of the percentage of workers to be fired and the level of voluntary turnover. Greater improvement is associated with higher numbers being fired and lower levels of voluntary turnover. The effects of bundling FDRS with recruiting and retention initiatives are also investigated.

    Notizen: Abstract  To study the early stages of cell death in various types of chronic liver injury, liver biopsies from a total of 26 patients, including 7 with chronic hepatitis C(CHC), 4 with chronic hepatitis B(CHB), 7 with alcoholic liver disease (ALD), 4 with autoimmune or drug hepatitis(AI/DH), and 4 with primary biliary cirrhosis(PBC), were examined by an in situ nucleotidyl transferase assay (ISNTA), which detects DNA fragmentation. Positive nuclei in hepatocytes and sinusoidal lining cells were counted in all parenchymal areas, excluding triads and areas of fibrosis, using a computer with Sigmascan software. The number of positive hepatocytes/mm2 was similar in the biopsies of patients with CHC, CHB, ALD and AI/DH, but significantly lower in PBC. The number of positive sinusoidal lining cells/mm2 was significantly greater in biopsies with CHC compared to CHB, ALD, AI/DH and PBC. Double staining revealed that the ISNTA-positive sinusoidal lining cells were also CD68 positive, indicating that they were Kupffer cells. The frequency of ISNTA positivity did not correlate with serum AST or ALT levels, steatosis, cell swelling or cirrhosis. ISNTA-positive hepatocytes were more frequent than acidophilic bodies in every disease category. We conclude that apoptosis may be a common pathway of cell death in different liver diseases, that the high frequency of DNA fragmentation in Kupffer cells in CHC suggests that during chronic hepatitis C infection activated Kupffer cells may be subject to regulatory control by apoptosis and that ISNTA is more sensitive than acidophilic bodies in assessing the degree of cell injury in the liver.

    Notizen: Abstract In much of the San Joaquin Valley, California, USA, inadequate drainage of applied irrigation water and accumulating salts in the soil have necessitated the installation of subsurface tile drainage systems to preserve crop productivity. At present, these subsurface drainage waters are disposed of by means of evaporation ponds or discharges into the San Joaquin River. Unfortunately, most of these agricultural drainage waters contain high concentrations of salts and naturally occurring trace elements, such as selenium, and recent evidence indicates that substantial numbers of waterbirds are exposed to contamination by selenium in the evaporation ponds. In order to avoid, minimize, or mitigate the adverse impacts on wildlife using the ponds, alternative pond management methods must be identified and evaluated for implementation. A number of methods have the potential to be cost-effective in significantly reducing the contamination hazard to birds using agricultural evaporation ponds. Twenty general methods were evaluated in this study, and four methods are recommended for implementation: remove levee vegetation, remove windbreaks, deepen the ponds, and haze birds. A number of other methods are recommended for further consideration because they appear to have good prospects for reducing the contamination hazard: steepen interior levee slopes, apply herbicides and insecticides, place netting on pond shorelines, and provide freshwater habitat adjacent to evaporation ponds. It may be necessary to use a combination of methods to effectively control selenium contamination of aquatic birds because it is unlikely that a single affordable pond management method will be able to entirely eliminate the contamination hazard.

    Notizen: Abstract Relatively few patients with chronic hepatitis C treated with standard doses of interferon-α2b (3 million units per week for 24 weeks) have a sustained response. Our aim was to evaluate whether higher doses of interferon would improve this rate of response. Twenty-four patients with chronic hepatitis C who had failed to respond to (N=21) or had relapsed after (N=3) an initial course of standard interferon therapy were randomized to 15 million units (N=13) or 22.5–30 million units per week (N=11) for 24 weeks. Five of 13 subjects given 15 million units per week and 3/11 of subjects given 22.5–30 million units per week had complete normalization of serum alanine aminotransferase levels during therapy. Five patients (24%) who had not responded to standard interferon had a complete response to high-dose interferon during therapy. Only one patient had a sustained response, with normal serum alanine aminotransferase 24 weeks after stopping interferon. Six patients were withdrawn before completing treatment, five in the 22.5–30 million unit per week group. We conclude that higher doses of interferon ameliorate the severity of hepatitis in patients who failed to respond to or relapsed after standard interferon therapy, but are unlikely to produce a sustained response. High-dose therapy is associated with an increase in side effects.

    Notizen: Abstract Ecological issues have become a growing concern to businesses. This experimental study examined the effect of a pro-environmental stance on the recruiting efforts of a fictitious firm. Results supported the idea that a proactive company stance on the environment would be positively related to perceived company attractiveness, intentions to pursue employment with that company, and acceptance of a job offer (n=303). Further, personal environmental stance influenced only the intent to pursue employment. This suggests that a positive environmental stance articulated in a recruitment brochure could positively affect potential recruits, even when a recruit does not personally see herself or himself as being particularly pro-environmental.
