Zusammenfassung Es wird über Langzeitergebnisse der Resektionsbehandlung von 164 Kranken mit idiopathischer Bronchiektasie an Hand von 20jähriger Erfahrung bei 175 Resektionen berichtet. Heilung konnte nur in 50% erzielt werden, 45% wiesen ein befriedigendes Ergebnis auf und in 5% blieb die Symptomatik unverändert. Ursächlich für das Weiterbestehen von Symptomen wird das unvermeidliche Zurückbleiben von Lungenabschnitten mit deformierender Bronchitis angesehen. Diese sind auch verantwortlich für die postoperativen Komplikationen. Aus der Verlaufsform der idiopathischen Bronchiektasien ist zu schließen, daß zu ihrer Entstehung konstitutionelle und exogene Faktoren in gleicher Weise beitragen.

Summary This is a report about a follow-up study of 164 surgical treated patients suffering from idiopathic bronchiectasis. Excellent results could be obtained only in 50% of the cases. 45% showed good results and 5% had the same symptoms as they had before surgical treatment. The cause for the bad results is seen in the deforming bronchitis in the remaining parts of the lung, which is often found in neighbouring segmental bronchi in bronchiectasis. The deforming bronchitis also is the reason for the development of postoperative complications. The course of idiopathic bronchiectasis allows the conclusion., that both, constitutional and exogenous factors, are important in the development of this disease.
