    Notes: Previous studies have shown that pretreatment of skin with beta2-adrenoceptor stimulants antagonizes anti-human IgE elicited wheal and flare reactions in atopic and non-atopic subjects. This experimental system was employed to further study the hypothesis that atopic disease and bronchial asthma unrelated to atopy might be associated with a beta-adrenoceptor defect. Eight patients with extrinsic and 10 patients with intrinsic asthma deprived of oral bronchodilator treatment and corticosteroids for at least a week, and in a clinically stable condition, 10 patients with extrinsic rhinitis and on no treatment, as well as age and sex matched healthy control subjects showed almost identical dose-response relations for the inhibitory effect of intradermally injected terbutaline (0.25–100 ng), on anti-IgE elicited skin reactions. In four asthmatic patients the inhibitory effect of terbutaline did not change following a treatment period with oral terbutaline and theophylline. The data do not support the presence of a cutaneous mast cell beta2-adrenoceptor defect in patients with atopy or bronchial asthma unrelated to atopy.

    Notes: Bronchial hyperreactivity (BHR) is found in Sjögren's syndrome, as in a number of other conditions such as asthma. BHR associated with asthma can be effectively treated with corticosteroids or sodium cromoglycate. We treated 19 Sjögren's syndrome patients with BHR with inhaled budesonide and inhaled cromoglycate for 6 weeks each. None of the treatments had any significant effect on symptoms of hyperreactivity or lung function. There was no effect on BHR measured as methacholine reactivity. Primary Sjögren's syndrome is a disease with inflammation not only in the salivary and lacrimal glands but also in the pulmonary alveoli and the bronchi. The main inflammatory cell is the lymphocyte, whereas, in the bronchi in asthma, the eosinophil granulocyte is the characteristic inflammatory cell. The cause of the discrepancy with regard to treatability of BHR in asthma and in Sjögren's syndrome is not known. Possibly not all BHR is caused by inflammation. There is not a perfect correlation between inflammation and hyperreactivity even in asthma. Even if the bronchial inflammation and the asthma symptoms are easy to treat with anti-inflammatory medicines, a considerable component of BHR usually still remains, as measured with methacholine or histamine.

    Notes: Interaction between human Clq and IgG coaled latex particles has been studied by means of a standard aggregometer equipment. A dose-dependent agglutination was observed and 100 ng of Clq were readily detected. The kinetics of the agglutination was also monitored. Serum, partially puritied Cl, and high molecular weight fractions from Sephadex G-200 fractionated serum produced agglutination only in the presence of EDTA. In the absence of this chclator these products disintegrated prerormed Clq-IgG-latex particle agglutinates. This disagglutin-ating principle is heat-sensitive and tentatively macromolecular Cl dependent. The most probable basis of the activity is the competition between Cl, with a high affinity for IgG particles, and Clq. The inability of Cl to induce particle agglutination might be causetl by the Cl subunits Clr and Cls sterically inhibiting the subunit Clq to bridge between the panicles.

    Notes: Background We have found a polymorphism in the ECP (eosinophil cationic protein)-gene at position 434 according to GenBank accession number NM 002935. This polymorphism would cause the change of the amino acid arginine (base at position 434 is G) at position 97 to threonine (base at position 434 is C).Objective To investigate the prevalence of the ECP-polymorphism and to screen for disease associations.Methods DNA of 209 medical students and 76 asthmatic subjects was analysed. The 434 genotype in the ECP-gene was detected by cleavage of the amplified DNA sequence with the restriction enzyme PstI and analysis of the cleaved product by agarose gel electrophoresis.Results The prevalences of the polymorphism in the student population were 53%, 39% and 8% for the 434GG, the 434GC and the 434CC genotype, respectively, with allele frequencies of 72% (434G) and 28% (434C). Subjects reporting allergy had a higher prevalence of the 434G allele than non-allergic subjects (P = 0.0056). Of the students who were Phadiatop-positive and had allergic symptoms, 79% had the 434GG genotype, whereas the 434GC and 434CC genotypes were present in 82% of those who did not express allergic symptoms (P 〈 0.001). Among the 76 patients with asthma, patients with allergic asthma had a significantly higher proportion of 434GG compared with patients with non-allergic asthma (P = 0.04). None of the 18 subjects of the two groups with the 434CC genotype had allergy.Conclusion The 434(G 〉 C) polymorphism in the ECP-gene is related to the development of allergic symptoms, suggesting a central role for the ECP molecule in the process.

    Notes: Twenty-four patients with asthma of suspected allergic origin were subjected to a total of twenty-five bronchial provocations with the relevant allergen. Eighteen of them reacted with bronchial obstruction: eleven with an isolated early reaction, five with a dual reaction and two with only a late reaction. Extensive lung function tests were performed. The patterns of early and late reactions were qualitatively very similar regarding variables of pulmonary physiological function, but there were quantitative differences. Patients with dual reactions showed significantly greater changes in FEV1, RV and TGV during the early response than those with isolated early reactions. Maximum mid-expiratory flow rates with the patients breathing air and a mixture of He and O2 were measured before and 6 hr after the challenges. Four of five patients with both an immediate and a late response showed a decrease in the response to helium during the latter phase, suggesting increasing involvement of the small airways. An increase in the slope of the alveolar plateau (phase III) was observed in four of the five patients with dual responses and all five had increased CC%. It was concluded that the changes in pulmonary function occurring during the dual responses were widespread, involving the airways generally and resembling those in the clinically more severe asthma. Usually, individuals with dual reactions showed stronger reactions. In two cases, however, a late reaction was obtained without any preceding early reaction.

    Notes: We evaluated the allergy status of 134 immigrants from Asia, Africa, the Middle East and South America, who were referred to our clinic during the past 10 years. Fifty Swedish patients were used for comparison. When the atopy state was not taken into account, no significant difference was found between the two groups with respect to total IgE levels. However, IgE levels of non-atopic immigrants were significantly higher than the IgE levels of non-atopic Swedes. While there was no significant difference in IgE levels between atopic and non-atopic immigrants, this difference was significant in Swedish patients. In general, IgE levels of immigrants showed a decline with time and reached approximately the same levels as for the Swedish patients in 10.5 years. In the immigrant group atopic women had a considerably lower IgE level than the atopic men. Among the atopics there were no differences between sexes. In Swedes and immigrants pollen was the most common group of allergens. The spectrum of allergy in the immigrant group changed with time in Sweden, and gradually became more similar to the Swedish spectrum. Skin and/or RAST positivity to birch increased from 16% within 2.5 years to 53% after more than 10.5 years in Sweden. Our data indicate that environmental factors rather than hereditary differences determine the IgE state. Within a few years the immunologic status of immigrants adapts to the new environment.

    Notes: Background: Excessive production of interleukin (IL)-4, IL-5, IL-10, and IL-13 is thought to be important in the development of allergy and asthma. The objective of this investigation was to study Th1/Th2-like cytokine profiles in vitro in seven patients allergic to birch pollen and six nonallergic controls during the birch-pollen season. Methods: Peripheral blood mononuclear cells (PBMC) were isolated and cultured with birch-pollen extract (BPE) or tetanus toxoid (TT) for 7 days, harvested, and restimulated with the mitogens phytohemagglutinin (PHA) and phorbol 12-myristate 13-acetate (PMA) for 24 h. Cytokine production was determined by ELISA, and logarithmic cytokine ratios were compared between the two groups and between the antigens. Results: In the allergic group, the cultures prestimulated with BPE had a more Th2-like cytokine response than the TT-prestimulated cultures; i.e., lower IFN-γ and higher IL-10 production (P〈0.05), as well as higher IL-5/IFN-γ and IL-13/IFN-γ ratios (P〈0.05). There were also significantly higher IL-4/IFN-γ (P〈0.005) and IL-5/IFN-γ (P〈0.05) ratios in BPE-stimulated cultures in the allergic group than in the control group. The IL-4 and IL-13 production in vitro correlated with the specific serum IgE levels. Conclusions: BPE stimulation induces a Th2-like cytokine response by PBMC isolated during the pollen season from birch-pollen-allergic patients, indicating a Th2-type immune response to birch pollen in vivo.
