    Notes: Abstract In the gastric mucosa of human subjects and of various mammals methylation was accepted as the main pathway of histamine catabolism. However, augmentation of gastric acid secretion by aminoguanidine, the strong inhibitor of diamine oxidase, indicated an influence of diamine oxidase activity on this secretory process. Therefore a careful reinvestigation of the occurrence of diamine oxidase activity was started from the distal duodenum in the direction of the cardia. In all species studied, diamine oxidase activity decreased from distal duodenum towards the pylorus. In dogs, landrace pigs and in human subjects the diamine oxidase activity clearly exceeded the pyloric borderline gradually becoming zero in corpus or fundus. In rabbits, however, and especially in mini-pigs no diamine oxidase activity was found beyond the pylorus. Among individuals gastric diamine oxidase activity showed a variable prevalence and could not be found regularly in all the subjects. In one patient with prepyloric ulcer a strong influence of pathophysiological processes on gastric diamine oxidase activity could be suspected. Thus, in every alteration of the gastric mucosa under experimental or clinical conditions also an alteration of gastric diamine oxidase activity should be taken into account.

    Notes: Abstract To understand the role of histamine in the aetiology and pathogenesis of human diseases reliable data are urgently needed for the histamine content and for the activities of histamine-forming and-inactivating enzymes in human tissues. In order to make a substantial progress toward this aim a tissue-sampling programme during surgical interventions was carefully conceived and conducted. From March 1982 until January 1983 106 tissue specimens were taken from 56 patients who underwent surgery. Only healthy tissues, not injured or oedematous, and without adherent structures were taken by only one surgeon who was interested in this research and experienced in tissue preparation procedures in biochemistry. The times of ‘warm’ ischaemia during the operative procedures were visually estimated, the times between resection of the organs or specimens and deep-freezing of the tissues were precisely recorded. Compared to previous work in the literature and especially to our own work using the same assays for determination higher histamine contents were found in this study in most of the tissues, in particular in the gastrointestinal tract. Also the diamine oxidase activities were considerably higher in many organs, e.g. 3–4 times higher in the gastrointestinal tract when compared with those in publications of our group who used always the same analytical test. However, the histamine methyltransferase activities in this study were not at variance to those determined in previous investigations. Many of them were reported in this communication for the first time. Since the methods for histamine determination and those for measuring enzymic activities were not different in this study and in previous communications of our group we are convinced that the optimized tissue-sampling and-preparation techniques were responsible for the higher values in this communication. But the problem of the ‘warm’ ischaemia period could not be solved by sample-taking procedures of this type during operations. There are good reasons to prefer biopsy specimens for the analysis of histamine storage and metabolism in human tissues in health and disease, but — unfortunately — they are not always available.

    Notes: Abstract Following superior mesenteric artery occlusion and revasclarization in dogs all animals died in a circulatory collapse state. However, pretreatment by aminoguanidine, the strong and specific inhibitor of diamine oxidase, accelerated the circulatory break-down significantly and increased the venous plasma histamine concentrations up to levels which also in normal dogs are effective in the circulatory system. Furthermore, the haematocrit increased significantly more in the aminoguanidine-treated animals than in the dogs treated by saline. No changes in plasma diamine oxidase activity were observed in saline-treated animals during intestinal ischemia and following revascularization. In aminoguanidine-treated animals no enzymic activity could be measured. The results were interpreted by a protective role of intestinal diamine oxidase in intestinal ischemia. Enhancement of the enzymic activity in patients, for instance by heparin, may be helpful in mesenteric infarction disease.

    Notes: Abstract Three hundred and forty-one drugs, commonly used in intensive care units (ICU), were chosen for an investigation of possible activation or inhibition of the histamine metabolizing enzyme diamine oxidase (DAO). After examination of 164 substances, using both canine and human DAO in anin vitro screening test, 61 agents inhibited DAO activity to various degrees. Of these, 44 inhibited the enzyme from both species, 4 inhibited the canine enzyme only and 13 the human DAO only. No. compound tested was able to enhance the enzyme activity. The inhibiting agents included representatives of all major therapeutic groups. A particlarly strong inhibition was observed with the neuromuscular blocking drugsd-tubocurarine, pancuronium and alcuronium, however, the other commonly used neuromuscular blocking drug, suxamethonium, was without effect. Similarly with the cephalosporines, cefotiame and cefuroxime caused a marked inhibition of the human DAO activity, whereas another regularly-used substance of this class, cefotaxime, inhibited neither the human nor the canine enzyme in concentrations up to 10−3 M. The observation that within a given therapeutic group some members inhibit and others do not, could be useful in choosing a therapy concept which minimizes the risk of a more severe ‘histamine’ reaction in seriously ill patients.

    Notes: Abstract In the operative treatment of appendicitis the so called negative appendectomy is an important issue because of its increased morbidity. From the hypothesis that the intestinal diamine oxidase activity is a suitable marker of mucosal integrity, the distribution pattern of the enzyme in appendices histologically classified as inflammed or not inflammed was studied. Histologically apparent inflammation of the appendix was connected with a significant reduction of diamine oxidase activity. The determination of this enzymic activity may be a simple and sensitive test for mucosal inflammation of the appendix even at a very early state. This could reduce the rate of negative appendectomies and influence thereby risk-cost-benefit calculations.

    Notes: Abstract In human subjects, apart from in the kidney, diamine oxidase occurs mainly in the gut. Therefore this enzyme can be used as an indicator of intestinal integrity. In biopsies of rectal mucosa the diamine oxidase activity was assayed in 55 patients, 41 having a histologically normal mucosa and 14 being diseased. The determinations of the enzymic activity were supervised by statistical quality control. In the unchanged rectal mucosa the diamine oxidase activity was 40 nmol/min×g on average. In 7 patients with rectal polyps the enzymic activity was significantly diminished in these benign tumours (x=7.7 nmol/min×g) apart from one, where it was elevated. A decrease in diamine oxidase activity was further observed in rectal carcinoma and ulcerative colitis. Whether the reduction of intestinal diamine oxidase activity accompanies premalignant or malignant states or whether it is a general sign of a disturbance of intestinal integrity remains questionable.

    Notes: Abstract The distribution of diamine oxidase (DAO) activity was studied in patients having no carcinoma disease. Besides in gut DAO occurred in high activity only in kidney and mesenteric lymph nodes. In patients with adenocarcinoma of the large bowel or of the stomach the enzymic activity was reduced in the tumour tissue itself as compared with the adjacent mucosa in which part the highest activities were observed. In stomach it seemed most important that the enzymic activity was enhanced in the whole mucosa, also in a 10 cm distance from tumour where no histological alterations were found. This fact should be checked for significance in early tumour diagnosis.

Wir haben die Ergebnisse einer prospektiven Studie von 100 konsekutiven Operationen ohne Deviationscolostomie mit 148 historischen Kontrollen unter Anlage eines Entlastungsanus verglichen. In der Kontrollgruppe gab es 25% Wund- und 35% Anus praeter-Komplikationen, Gesamt-Anastomoseninsufizienzen von 35% und eine Letalität von 9,6%. Im Studienkollektiv betrug die Nahtdehiscenzrate 3% und die Letalität usw. 1%. Wir folgern daraus, daß eineroutinemäßige Entlastungscolostomie verzichtbar ist.
    Notes: Summary Whether a protective colostomy should be used routinely or not in left-sided colorectal resection is still controversial. We compared the results of a prospective trial including 100 consecutive operationswithout protective colostomies to 148 historical controlswith protective colostomies. In the study group only the protocol concerning perioperative treatment and surgical technique was strictly standardized. In the controls there were: 25% complications in the laparotomy wound, 35% complications in the diverting anus, 36% anastomotic leaks, and a 9.6% mortality. In the study group without colostomies the anastomotic insufficiency rate was 3% and mortality only 1%. We conclude that protective colostomies need not to be applied routinely in left-sided colon surgery.
