    Notes: Abstract: σ-Receptors are nonopioid, nondopaminergic receptors that bind with high affinity several antipsychotic drugs and appear to be involved in regulation of posture and movement. The second messenger system coupled to these receptors is still unknown. Recently, an inhibitory effect of various σ-compounds on carbachol-stimulated phosphoinositide metabolism has been reported. We have investigated the effect of six σ-compounds on carbachol- and norepinephrine-stimulated 3H-inositol phosphate accumulation in rat cerebral cortex slices. All compounds tested had a dose-dependent inhibitory effect on both systems, although their order of potency differed between neurotransmitters. Pentazocine and 1,3-di-o-tolylguanidine were the most potent inhibitors of carbachol-stimulated phosphoinositide turnover (IC50= 31.5 and 45.7 μM, respectively), while haloperidol showed the greatest potency on the norepinephrine-coupled system (IC50= 3.5 μM). In the presence of IC50 concentrations of these inhibitors, the dose-response curves for the agonists were shifted to the right and the EC50 values were significantly increased. σ-Compounds also inhibited the binding of [3H]quinuclidinyl benzilate to muscarinic receptors and of [3H]prazosin to α1-adrenoceptors in cortical membranes. In the presence of IC50 concentration (11 μM) of 1,3-di-o-tolylguanidine, no differences were found in the maximal number of muscarinic binding sites, whereas the dissociation constant increased approximately fivefold, indicating a decrease of the radioligand's affinity for the receptor. These results indicate that σ-compounds, at micromolar concentrations, inhibit muscarinic and α1-adrenergic receptor-coupled phosphoinositide metabolism, probably through an interaction with the neurotransmitter recognition sites.

    Notes: Abstract: Metabotropic glutamate receptors (mGluRs) have been recently described as a family of guanine nucleotide-binding regulatory protein-coupled receptors with multiple signal transduction pathways. At least one of these receptors appears to be negatively coupled to adenylyl cyclase when stably expressed in transfected cells. We have studied how activation of native mGluRs modulates cyclic AMP (cAMP) formation in brain slices prepared from rats at different ages. 1S,3R-1-Aminocyclopentane-1,3-dicarboxylic acid (1S, 1R-ACPD), a selective agonist of mGluRs, slightly increased basal cAMP formation but reduced forskolin-stimulated cAMP formation in adult hippocampal slices, in agreement with previous results. The action of 1S,3R-ACPD on basal cAMP formation was not reproduced by the ionotropic receptor agonists N-methyl-d-aspartate, kainate, and α-amino-3-hydroxy-5-methylisoxazole-4-propionate and was antagonized by l-2-amino-3-phosphonopro-pionate (l-AP-3). l-AP-3, however, did not prevent but rather mimicked the inhibitory action of 1S,3R-ACPD on forskolin-stimulated cAMP formation. In hippocampal slices from 1-, 8-, or 15-day-old rats, 1S,3R-ACPD increased basal cAMP formation but failed to reduce the action of forskolin. A similar developmental pattern of modulation was observed in hypothalamic slices with the difference that 1S,3R-ACPD did not stimulate basal cAMP formation in the hypothalamus of adult animals. These results suggest that inhibition of forskolin-stimulated cAMP formation by 1S,3R-ACPD is mediated by a specific mGluR subtype that is preferentially expressed in the adult.

    Notes: Abstract  Background: The Ross operation has been applied to various aortic valve pathologies, particularly when somatic growth is an issue. However, associated cardiac disease and technical problems may limit its use with regard to associated procedures and issues of right ventricular outflow reconstruction. Materials and Methods: From December 1992 to March 1998,24 patients underwent aortic pulmonary autograft implantation. There were 14 males and 10 females, 15 ± 10 years of age (mean ± SD) (range 1 to 50 years), weighing 42.8 ± 20 kg (mean ± SD) (range 8 to 78 kg). Aortic insufficiency was present in 15 (62.5%) patients, stenosis in 8 (33.3%) patients, and valvar stenosis associated with left ventricular outflow tract obstruction in 1 (4.1%) patient. Etiology was rheumatic in 17 patients and congenital in 7. The Ross procedure was accompanied by a partial-Konno left ventricular outflow enlargement in one patient, and mitral valve annuloplasty, mitral commissurotomy, and tricuspid valve replacement in three other patients, respectively. The right ventricular outflow was reconstructed with a valved pulmonary homograft in 14 patients and with a Shelhigh No-React ® porcine pulmonary conduit in 10 patients. Evaluation was done by New York Heart Association (NYHA) Class and by echocardiography at a follow-up of 22.8 ± 24 months (mean ± SD) (range 3 to 63 months). Results: There were no operative mortalities and no postoperative arrhythmias. One (4.1%) patient required intra-aortic balloon pump (IABP) support for 3 days, one (4.1%) patient died 2 years later of probable arrhythmia, and one (4.1%) patient required mechanical aortic valve replacement 2 years later for severe autograft insufficiency. Left ventricular ejection fraction was unchanged (preoperative 62.4% ± 30%, postoperative 64.2% ± 30% [mean ± SD], [p = NS]) and no significant gradient was documented by echocardiographic Doppler in the right and left ventricular outflow tracts. The aortic insufficiency scale decreased from a mean of 3.9 ± 0.2 to a mean of 1 ± 0 (p 〈 0.01). NYHA Class decreased to 1 in all patients, from III (10) and II (14). Conclusions: The pulmonary autograft in the aortic position is suitable for aortic valve replacement in pediatric and adult patients with good medium-term results and in patients with rheumatic etiology, and it provides a desirable solution in the presence of associated pathologies, such as left ventricular tract obstruction or associated multivalvular disease. The development of new means of right ventricular outflow reconstruction must parallel the progress achieved for the left side.

    Notes: Abstract lntrapericardial teratomas are unusual tumors that often cause respiratory distress and might be lethal in the newborn. The purpose of this article is to present the clinical and pathological findings in a 12-day-old male successfully treated for a big intrapericardial teratoma. Given that the diameter of this tumor is generally related to the age at the time of diagnosis, the rarity of our case is the presence of a huge intrapericardial teratoma in a newborn. The surgical resection was lifesaving.

    Notes: Visually responsive neurons have been recorded in the lateral suprasylvian area (LSA) of cats raised with either a convergent or a divergent strabismus. In contrast to areas 17 and 18, where many studies have documented a profound loss of binocularly activated neurons following early strabismus, in the LSA the majority of cells could still be binocularly driven. Acute or chronic section of the splenium of the corpus callosum reduced but did not abolish binocularity in the LSA. We propose that the widespread callosal connections, the large size of the receptive fields and the peculiar internal circuitry of the LSA all concur in permitting the maintenance of binocular coding in spite of early misalignment of the eyes.

    Notes: 1. Food sources and trophic structure of the macroinvertebrate community along a longitudinal gradient were examined in a glacier stream of the Swiss Alps (Val Roseg). Analysis of multiple stable isotopes (δ13C and δ15N) and measurement of C : N ratios were used to differentiate between allochthonous and autochthonous organic matter.2. Although isotopic signatures of algae varied widely among sites and dates, it was possible to discriminate between allochthonous and autochthonous food sources using a site-specific approach.3. Dominant food sources of herbivorous invertebrates in all main channel sites were epilithic diatoms and the filamentous gold alga Hydrurus foetidus. Allochthonous organic matter was of some importance only in a groundwater-fed stream close to the floodplain margin.4. Seasonal changes in the δ13C signature of the macroinvertebrates corresponded with seasonal changes in δ13C of the gold alga H. foetidus. This indicated that the energy base remains autochthonous throughout the year.5. Because of limited food sources, feeding plasticity of the invertebrate community was high. Both grazers and shredders fed predominantly on algae, whereas gatherer-collectors seemed to be omnivorous.6. The overall enrichment of δ15N was 2.25‰ (r2=0.99) per trophic level. On a gradient from the glacier site to a downstream forested site trophic enrichment was constant but variation in δ15N within trophic levels decreased.

    Notes: Background : Small intestine bacterial overgrowth is associated with the presence of predisposing conditions, acting through different mechanisms. Therefore, the failure to define a standardized therapy may be due to a methodological bias: to treat a condition characterized by different pathophysiological mechanisms with the same pharmacological approach. Non-absorbable antibiotics could have a lower efficacy than absorbable drugs in patients with blind loops which exclude a portion of the intestine from the transit.Aim : To evaluate the efficacy of absorbable vs. non-absorbable antibiotics in this subgroup of patients.Methods : A group of small intestine bacterial overgrowth patients with total gastrectomy or gastrojejunostomy and blind loop underwent a therapeutic trial comparing rifaximin to metronidazole. Seven patients underwent a course of rifaximin followed by a course of metronidazole on recurrence of symptoms. To compare the effect of the drugs, another two groups of patients underwent two consecutive courses of rifaximin or metronidazole. Hydrogen breath test after glucose administration and symptom severity measurement were performed.Results : Both drugs reduced breath H2 excretion but a much better improvement was achieved after metronidazole. Symptom improvement was higher after metronidazole.Conclusion : Metronidazole is more effective than rifaximin for the treatment of small intestine bacterial overgrowth associated with the presence of a blind loop.

    Notes: Simethicone, activated charcoal and antimicrobial drugs have been used to treat gas-related symptoms with conflicting results.〈section xml:id="abs1-2"〉〈title type="main"〉Aim:To study the relationship between gaseous symptoms and colonic gas production and to test the efficacy of rifaximin, a new non-absorbable antimicrobial agent, on these symptoms.〈section xml:id="abs1-3"〉〈title type="main"〉Methods:Intestinal gas production was measured by hydrogen (H2) and methane (CH4) breath testing after lactulose in 21 healthy volunteers and 34 functional patients. Only the 34 functional patients took part in a double-blind, double-dummy controlled trial, receiving, at random, rifaximin (400 mg b.d per 7 days), or activated charcoal (400 mg b.d per 7 days). The following parameters were evaluated at the start of the study and 1 and 10 days after therapy: bloating, abdominal pain, number of flatus episodes, abdominal girth, and cumulative breath H2 excretion.〈section xml:id="abs1-4"〉〈title type="main"〉Results:Hydrogen excretion was greater in functional patients than in healthy volunteers. Rifaximin, but not activated charcoal, led to a significant reduction in H2 excretion and overall severity of symptoms. In particular, in patients treated with rifaximin, a significant reduction in the mean number of flatus episodes and of mean abdominal girth was evident.〈section xml:id="abs1-5"〉〈title type="main"〉Conclusions:In patients with gas-related symptoms the colonic production of H2 is increased. Rifaximin significantly reduces this production and the excessive number of flatus episodes.
