    Description / Table of Contents: Zusammenfassung Vor Einführung kryochirurgischer Eingriffe am Labyrinth in die Klinik muß die Möglichkeit einer Schädigung der benachbarten pars tympanica des N. facialis überprüft werden. Wir arbeiteten mit einer Kühlsonde von 2 mm Durchmesser, an deren Auflagefläche − 190°C zu messen war. Bei 3 Kaninchen nahmen wir Temperaturmessungen in der pars tympanica des N. facialis während der Unterkühlung des oberen vertikalen Bogenganges vor. Die Sonde wurde 7 Versuchstieren 10 min lang auf den oberen vertikalen Bogengang und weiteren 7 Kaninchen auf die pars tympanica des N. facialis aufgesetzt. Durch Unterkühlung des oberen vertikalen Bogenganges wurde in keinem Falle die Temperatur im N. facialis unter + 6°C gesenkt. Die Reizschwelle des Nerven veränderte sick weder unmittelbar nach dem Eingriff noch während der mehrwöchigen Überlebenszeit. Feingeweblich waren während der gesamten Beobachtungszeit außer einer geringfügigen Verbreiterung des Endo- Bowie Perineuriums keine Veränderungen nachzuweisen. Im Gegensatz dazu verursachte die direkte Unterkühlung der pars tympanica des N. facialis bereits nach 2 Tagen eine Unerregbarkeit des distalen Nervenabschnittes. Dem entsprach feingeweblich eine fortschreitende Degeneration des Nerven, der schließlich durch eine Narbe ersetzt wurde. Wir erwarten demnach bei der Kryochirurgie des Labyrinths unter der Voraussetzung einer exakten Applikation der Kühlsonde am benachbarten Anteil des N. facialis keinen Schaden.
    Notes: Summary Before using the cryosurgical treatment on the labyrinth one should be aware of the possibility of the damage to the adjacent facial nerve. The cooling probe with a diameter of 2 mm and a temperature of − 190° C was applied. Temperature at the pars tympanica of facial nerve in three rabbits was measured when cooling the upper vertical semi-circular canal. The probe was rested at the upper vertical semi-circular canal of seven experimental animals for 10 min; and other seven rabbits on the pars tympanica of the facial nerves. When cooling the upper vertical semi-circular canal, in no case the temperature within the facial nerve could be made below + 6° C. The threshold of the nerve has never been changed immediately after treatment or later. No changes of tissue-structure were detectable except small widening of the endo- and perineuriums. On the other hand, the direct cooling method at the pars tympanica of facial nerve resulted non-excitation of the distal part of the nerve two days after the treatment. This causes the degeneration of the nerves and it finally replaced by a scar. The authors anticipate that with the cryosurgical treatment at the labyrinth causes no damage on the facial nerve on the assumption that an exact application of the cooling probe was used.

    Description / Table of Contents: Zusammenfassung Zur Frage der Resistenz der Gefäße gegenüber kryochirurgischen Eingriffen in ihrer unmittelbaren Umgebung unterkühiten wir 1/2 bis 5 min lang mit einer Kryosonde von 1 bzw. 2 mm ø and einer Arbeitstemperatur von minimal −190° C die A. carotis von 20 Versuchstieren. In der dünnkalibrigen A. carotis communis des Meerschweinchens erstarrte das Blut unmittelbar nach dem Aufsetzen der Kühlsonde zum Eisthrombus. Die feingewebliche Untersuchung konnte weder sofort noch in der Verlaufsbeobachtung Veränderungen an der Gefäßwand aufdecken. In der A. carotis communis des Kaninchens war infolge ihres weiten Lumens der Blutdurehfluß auch nach länger dauernder Unterkühlung nicht zu stoppen. Die feingewebliche Untersuchung ließ bei Erhaltenbleiben des Endothels sowie der elastischen Fasern eine rich allmählich entwickelnde, scharf begrenzte Narbe erkennen. Diese unterschiedlichen Ergebnisse können durch das verschiedene thermophysikalische Verhalten des gefrorenen und flüssigen Blutes erklärt werden. Aus den Ergebnissen ziehen wir den Schluß, daß bei kryochirurgischen Eingriffen in der Nähe großer Gefäße Komplikationen ausbleiben werden.
    Notes: Summary Twenty guinea-pigs were used for the study of the low temperature resistance of blood vessels against cryosurgical treatment. We cooled the A. carotis for a period of time between 1/2 to 5 min by means of a cryosonde which has a diameter of 1 or 2 mm and at a minimum temperature − 190° C. After applying the cooling probe to the narrow A. carotis communis, the blood within was coagulated immediately. No immediate changes nor any alteration on the wall of the blood vessels. As the result of the wide lumen of the A. carotis in rabbit, the blood-flow was not stopped after a long period of cooling. The tissues showed clearly limited scars which developed gradually; the endothelials as well as the elastic fibers were kept in good form. These results could be interpreted by different thermophysical behaviour of cooling and the blood.

    Notes: Summary Experiments on rats were conducted in three series: 1. Preparatory experiments on 13 animals to develop operative techniques. 2. In four animals the left carotid artery was ligated and cut through. In the 3rd week after the operation 0–40 Gy by 8 MeVē was irradiated on the right side of the neck. The animals were observed live for up to 122 days; the angiogram of the right carotid artery was normal; the histological examination also showed normal conditions. 3. After end-to-end-anastomosis of the right carotid artery, the left one was ligated and cut through. In the 3rd week followed irradiation of the right side of the neck with 30 Gy. The animals survived for 136 to 223 days and were then killed. The angiogram of the carotid artery was regular. The histology of the arterial wall was largely in order; in the adventitia small granulomes from the thread were found. The survival-time, angiography and histological results demonstrate that the radiation had no negative influence on the microanastomosis of the rats carotid artery. The biological effects of 30 Gy in one application corresponds, according to Ellis, to 101, 70 Gy NSD (Nominal Standard Dose), almost twice as much a tumour dose.

    Description / Table of Contents: Zusammenfassung Im Meerschweinchonversuch wurde α-Chymotrypsinlösung unterschiedlicher Konzentration (1, 3 bzw. 6 Einheiten in 0,1 ml) in die Bulla instilliert und nach 24 Std das Mikrophonpotential (MP) gemessen (40 Versuchs tiere). Gegenüber einer Vergleichsgruppe, bei der physiologische NaCl-Lösung verwendet wurde, trat eine deutliche Reduktion der Intensität des NIP auf, die um so starker war, je höher die Konzentration des α-Chymotrypsins gewahlt wurde. Diese Beeinträchtigung der Innenohrfunktion durch lokale Anwendung proteolytischer Fermente am Ohr macht in der klinischen intratympanalen Anwendung dieser Mittel beim Adhäsivprozeß eine gewisse Vorsicht erforderlich.
    Notes: Summary In a test on guinea pigs various concentrations of α-chymotrypsin (1, 3, and 6 units in 0.1 ml) were instilled into the bulla tympanica. After 24 hours microphonics was measured of the 40 test animals. A control group of animals was used, which received physiological NaCl solution. In the comparison, the reduction in microphonics intensity of the test animals was marked, and became more noticeable with increasing α-chymotrypsin concentration. In clinics treating otitis media adhaesiva, this injury to the function of the ear from local application of proteolytic ferments points to a rather cautious intratympanal use of these preparations.
