    Notes: Abstract We propose a set of standards to aid the physician in the care of older patients. These standards are based on the practical experiences of our own group and of others with years of clinical practice in geriatric medicine. The standards also reflect the guidelines, position papers, and deliberations of various organizations concerned with the care of older people. This article does not discuss specific illnesses or the common geriatric syndromes. The proposed standards cover comprehensive care and assessment, especially of vulnerable elders and prevention of disease and disability. We also propose standards for facilitation of care across the health service continuum, care of the nursing home resident, and palliative and hospice care.

    Notes: A low oxygen atmosphere, which shows promise of controlling insect infestations during storage of raisins was studied in relation to its effect on flavor changes during storage of raisins at 18.5 and 27°C for 1 yr. Raisins stored in the low oxygen atmosphere for 1 yr at 18.5 or 27°C were equal or superior in flavor quality and acceptability to similar samples stored in normal atmosphere for the same length of time.

    Notes: A low oxygen atmosphere, known to control insect infestations in stored almonds, was studied to determine its effect on flavor changes in stored inshell almonds and almond meats at 18.5 and 27°C. Sensory comparisons of the products stored in low oxygen and normal atmosphere at these temperatures after 1, 3, 6, 9 and 12 months of storage indicate the following: (1) In normal atmosphere at both 18.5 and 27°C, inshell almonds were much more stable than almond meats; (2) In the low oxygen atmosphere, no significant differences were observed between storage as meats or inshell; (3) Temperature had a highly significant effect on off-flavor development in meats stored in normal atmosphere but no significant effect on those stored in the low oxygen atmosphere; (4) Storage in the low oxygen atmosphere caused less off-flavor development than in normal atmosphere for both meats and inshell almonds.

    Notes: Summary Depletion of high-energy phosphates, accumulation of inorganic phosphate and intracellular acidosis have each been proposed as important events in the transition from reversible to irreversible ischemic injury. To assess whether each variable is predictive of functional recovery on reperfusion, these were measured in the isolated isovolumic rat heart using31P NMR. Perfused hearts were subjected to either 10, 12 or 40 min of normothermic ischemia followed by 40 min of reperfusion. Hearts were then freeze-clamped for further analysis of phosphate metabolites by NMR and ion chromatography. High-energy phosphates, Pi, phosphomonoesters and pH were measured by31P NMR spectroscopy at 2 minute intervals. Heart rate and developed pressure were monitored simultaneously. All hearts undergoing 10 min of ischemia and 40% of hearts subjected to 12 min of ischemia demonstrated good functional recovery. The remainder of hearts ischemic for 12 min went into contracture on reperfusion with little return of function. Hearts subject to 40 min of ischemia went into ischemic contracture and showed no recovery on reperfusion. Intracellular pH, [ATP], and [Pi] measured prior to reperfusion did not predict the extent of recovery. However, phosphomonoesters were detected prior to reperfusion in all hearts that did not recover well, but were not observed in hearts that showed good mechanical recovery. Analysis of tissue extracts by31P NMR and ion chromatography indicated that the most prominent components of the phosphomonoesters were glucose 6-phosphate, α-glycerol phosphate and AMP. In conclusion, of the various phosphorus metabolites that can be measured by31P NMR, only one group, the phosphomonoesters, was predictive of functional recovery.
