    Notes: Background Previous studies have suggested varying molecular weights for mast cell derived tumour necrosis factor alpha (TNFa) and little data exist upon the factors which may regulate the control of this cytokine in these cells.Objective To determine the molecular weight of canine mastocytoma-derived TNFrt. to determine whether it is pre-formed within the granule and whether its expression could be up-regulated by stem cell factor (SCF).Methods Molecular sizing was assessed by immunoblot. The cellular localization of the TNFα was determined by immunocytochemistry before and after stimulation by A23187 and passive sensitization. Subcellular localization was performed by immuno-gold immunocytochemistry. Changes in the level of mastocytoma mRNA for TNFα in response to stimulation with SCF or fibroblast conditioned media for up to 12 weeks were studied using Northern analysis and changes in the level of TNFα protein expression on immunoblot and immunocytochemistry.Results Mast cells contained authentic 17 kDa TNFα as identified by immunoblotting. Immunocytochemical studies demonstrated preformed TNFα which was released by stimulation with antigen after passive sensitization. or by the calcium ionophore A23187. Further confirmation of the preformed nature of this TNFa was provided by immunogold electron microscopy which localized this cytokine to the granule of the inactive mast cell. Northern blotting revealed a constitutive message for TNFα. which increased in response to fibroblast conditioned media (FCM) and to recombinant human SCF. Immunocytochemical studies of mast cells cultured long-term with FCM or with recombinant SCF showed increased expression of TNFa over the course of 12 weeks incubation with these stimuli.Conclusion Mastocytoma derived—TNFα is a preformed, granule—associated 17 kDa cytokine which is released on stimulation with A23187 or passive sensitization. It is up-regulated by stem cell factor and by FCM over the course of 12 weeks.

    Notes: Background: Latex sensitivity is an increasing problem among health-care workers. Allergic responses are associated with changes in nitric oxide (NO) generation and the changes secondary to latex have not been described. Methods: A total of 22 subjects comprising equal numbers of control volunteers and subjects with self-reported latex sensitivity were recruited to undergo latex skin prick testing. Symptom scores, exhaled nitric oxide (eNO), and nasal nitric oxide (nNO) were studied 1) before and after a controlled latex challenge (n=16), and 2) at the beginning and end of the working week, during exposure to latex (n=18). Results: Latex challenge caused a significant fall in nNO levels in latex-sensitive subjects, compared to normal control subjects (P=0.04). eNO levels also decreased in the latex-sensitive subjects after latex challenge, but to a lesser degree. There were no significant differences between the beginning and end of the working week in terms of eNO or nNO in either group, although symptom scores showed a nonsignificant increase in latex-sensitive subjects. Conclusions: Fall in nasal NO after latex challenge is associated with reported symptomatic latex sensitivity, and this corresponds to latex skin prick test positivity. Neither nNO nor eNO showed a clear relationship to routine workplace exposure.

Solutions of lithium chloride and tin (IV) chloride in sulfolane contain the complex ions SnCl 5 − and SnCl 6 2− at 1∶1 and 2∶1 mole ratios of constituents, respectively. The complexes are characterized by conductimetry and by Mössbauer, IR, and Raman spectroscopy.

    Notes: Abstract A radiograph of the left hand and wrist was taken in 141 children and young adults with phenylketonuria and hyperphenylalaninaemia. Ten (7.1%) had a bony abnormality and, of these, six (4.3%) showed evidence of reduced bone density. Only one patient had evidence of spiculation of the lower ulnar metaphysis previously described in phenylketonuria. Bone maturity was also assessed in 130 patients by the Tanner-Whitehouse (TW2) method. In 28 patients (21.5%) bone age was less than chronological age by 1 year or more. Bone age delay was greater in younger children on a more restricted diet with a lower intake of natural protein.

    Notes: Abstract Radiographic analysis of 24 cases of hypophosphatasia (H) from 9 Paediatric Centres was performed. 3 cases were of neonatal (lethal), 18 cases of infantile (severe) and 3 cases of late (benign) type. Some of the patients were in reality borderline cases between these groups. In the authors' material all the patients showed radiographic signs of the disease. These were divided into diagnostic, characteristic and suggestive features. All of the patients had in common generalised (usually irregular) osteoporosis, generalised (usually irregular) metaphyseal changes, craniostenosis (13 of 18 infantile cases) or widened cranial sutures and often bowing of the long bones. Besides the well know radiographic features of hypophosphatasia some less well known, rare or ‘new’ ones such as, 1. spurs of the long bones (Bowdler sign), 2. distal femoral central metaphyseal defects and epiphyseal defects, 3. S-like deformities of the tibiae, 4. abnormal shape of the distal phalanges of the fingers, 5. multiple rib fractures and slender bones, 6. wedging of the lower thoracic and upper lumbar vertebrae, 7. partial premature fusion of the epiphyses, 8. nephrocalcinosis, 9. loss of lamina dura around the teeth, 10. variation in radiographic appearances of a pair of siblings with lethal form, and, 11. rapid changes in roentgen appearances, are discussed. In two of our patients (siblings) phosphoethanolamine was undetectable in the urine. The authors doubt if a normal skeletal survey may be present at any stage in any of the three major types of hypophosphatasia.

    Notes: Abstract Pelvis-shoulder dysplasia is rare skeletal dysplasia characterised by symmetrical hypoplasia of the iliac wings and scapulae. It may be associated with unstable hips in infancy, a prominent lumbar lordosis and a waddling gait. Symptoms tend to lessen with increasing age. A child with this condition who has been followed up to the age of five years is described. Her pelvic dysplasia is milder than in previous cases and has caused no disability. Minor skeletal abnormalities are also present in the lumbar spine, femora and rib cage. Radiography of other members of the family was normal.

    Notes: Abstract Clinical and radiological features of seven patients with Morquio's disease Type A (MPS 1V A) are presented. The diagnosis was established by two-dimensional electrophoresis of the urinary glycosaminoglycans and enzyme assay. The clinical manifestations were mild or moderate in all patients and the radiological changes were milder than those described in classical Morquio's disease.

    Notes: Abstract The technique of contrast examination of the common types of cerebrospinal fluid (CSF) shunts is described. The findings in 250 shuntograms are analysed and the results discussed. The most common cause of shunt malfunction was shunt blockage. Errors of interpretation occurred in 16 shuntograms and were more frequent in Pudenz systems than in Spitz-Holter. The technique is simple, safe, accurate and does not require special apparatus. There was only one serious complication in 250 shuntograms. It is particularly useful when the anterior fontanelle has closed.

    Notes: Abstract A retrospective analysis (1980–1990) of normally formed low birthweight (〈2500g) infants surviving to at least 28 days following intermittent positive pressure ventilation (IPPV) for longer than 12h was performed. Bronchopulmonary dysplasia (BPD) was defined as oxygen dependency at 28 days with characteristic radiographic findings. Logistic regression analysis of risk factors, before and after the initiation of IPPV was performed on 412 infants. Decreasing birth weight (BW) and gestational age (GA) were associate with an increased risk of BPD. When controlled for these variables, predictive factors prior to IPPV were gender, age at IPPV, respiratory diagnosis, and year of birth. Following IPPV, duration of peak inspiratory pressure 〉25cm H2O, duration of fraction of inspired oxygen (FiO2)〉0.60 (DO2), maximum peak inspiratory pressure (MPIP), maximum FiO2, patent ductus arteriosus, bacteraemia and either pneumothorax or pulmonary interstitial emphysema were associated with an increased risk of BPD Adjusting for BW and GA, there was a significant reduction in BPD risk from 1980–1990 (relative odds of 0.88 for each year compared to the previous year). This trend could belargely accounted for by decreases in MPIP and DO2 during the study period. Surfactant treatment was not independently associated with a significant change in the risk of BPD. Based on this analysis, we developed a scoring system for predicting the risk of BPDL in the neonatal period which we evaluated in a random sampleof infants. This predicted infants at risk of BPD with a sensitivity of 65% and a specificity of 88%. Use of this score would allow prediction of BPD at a tim when earlier preventive treatment could be started.

    Notes: Abstract Nickel sulphide (NiS) was characterised using X-ray diffraction, thermal gravimetric analysis (TG) and differential scanning calorimetry (DSC). The 'as received' Millerite, stoichiometric NiS, observed to be slightly nickel deficient, was found to readily decompose in a nitrogen atmosphere at elevated temperatures (450°C max.) to the sulphur deficient Godlevskite, Ni7S6. DSC and X-ray measurements demonstrated that the high temperature form of the Godlevskite was readily stabilised at room temperature. The kinetics of the α-β re-transformation in Godlevskite were then investigated using DSC and were observed to be first order.
