    Notes: We report the case of a 72-year-old male patient who developed a florid photosensitive eruption while on ranitidine therapy. Ultraviolet A sensitivity was detected by irradiation monochromator testing, suggesting drug-induced photosensitivity. Ranitidine was concluded to be the cause of his photosensitivity since the eruption resolved and the phototest abnormalities returned to normal following cessation of therapy. Similar cases have been reported to the Committee on Safety of Medicines but not published.

    Notes: S-100, an acidic calcium-binding protein, is present within cells of neuroendocrine origin. Its value in the immunohistochemical diagnosis of tumours of melanocytic origin is well established. More recently, a potential role has been proposed for the serum concentration of this protein as a marker of metastatic melanoma disease activity. In the present study, the concentration of serum S-100 protein was measured in 97 patients with histologically proven malignant melanoma who were attending a dermatology and/or oncology department for the follow-up of their disease. Serum S-100 was also measured in 48 control subjects without malignant melanoma. The clinical stage of the patients was classified according to the criteria of the American Joint Committee on Cancer into stages I-IV. The median (range) serum S-100 protein concentration was significantly higher in stage I (0·11 (0·1–0·21) μg/L, P〈0·001), stage II (0·11 (0·05–0·22) μg/L, P〈0·001), stage III (0·24 (0·07–0·41)μg/L, P〈0·0001) and stage IV (0·39 (0·06–15·0)μg/L, P〈0·0001) compared with the control group (0·1 (0·05–0·15)μg/L). At a threshold value of 0·2μg/L, the sensitivity and specificity for detection of advanced disease were 82% and 91%. respectively. Thus serum S-100 protein may be a valuable prognostic marker for malignant melanoma and for monitoring therapy. Serum S-100 protein concentration was also compared with the Breslow thickness of the tumours. There was a significant correlation between these variables (n=72. rs=0·32, P〈0·01). Combining a serum S-100 threshold value of 〉0·22μg/L and a Breslow thickness of 〉4mm improved the sensitivity and specificity for the presence of secondary spread to 91% and 95%, respectively. Therefore, a combination of both baseline serum S-100 protein and Breslow thickness may provide a better indication of the prognosis at diagnosis.

    Notes: The case is described of a female patient with necrobiosis lipoidica of the legs, and a previous history of acute sarcoidosis. There has been only one previous report of an association between necrobiosis lipoidica and sarcoidosis.1 The relationship between these disorders may be suggested by the known association of sarcoidosis and other granulomatous and necrobiotic disorders.

    Notes: One hundred consecutive histologically benign lesions seen at a pigmented lesion clinic were studied prospectively to assess the diagnostic accuracy of the seven-point check-list. Lesions were scored by both patient and physician. Thirty per cent of lesions attained a suspicious score. While there was a high level of agreement between patient and clinician regarding the individual characteristics of a lesion as defined in the check-list, final assessments differed. Most patients considered their lesions suspicious, whereas the clinicians correctly recognized that in 94% cases they were benign. The seven-point check-list often fails to exclude common benign pigmented lesions. The revised (1989) check-list is even less specific (70% of lesions achieving a suspicious score). Such check-lists are difficult for the inexperienced to interpret. Clinical familiarity is required in order to provide a confident diagnosis. Future emphasis should be directed towards improving screening in primary care.

    Notes: Subcutaneous sarcoid appears to be rare, particularly in Caucasians. When it does occur, it usually heralds systemic involvement. A case of subcutaneous sarcoid is reported, which is unusual both in its extent, and by its lack of demonstrable extra-cutaneous disease.Cutaneous sarcoid is well recognized, but subcutaneous sarcoid has rarely been described. We report only the fourth case of subcutaneous sarcoid from the UK.

    Notes: A pigmented-lesion clinic was established in the skin department at King's College Hospital to coincide with the British public-education campaign on malignant melanoma. One-hundred cases of malignant melanoma have subsequently been seen and evaluated prospectively with regard to the usefulness of the Glasgow seven-point check-list as a guide to diagnosis. The most-frequently reported patient observations were growth of the lesion (74%) and change in its colour (53%) (especially darkening). However, 50% of the lesions were smaller in size than the 1 cm referred to in the check list and 15% were less than 0.5 cm. Itching was noted in 24% and bleeding and crusting were remarked upon in 18% of cases. Abnormal shape was noted in 20% of the patients. Inflammation was visible in only 11%. Seventy-three per cent of our patients had early lesions (less than 1.5 mm Breslow thickness) and nearly half presented at the insistence of their relatives or friends or because of the family doctor's incidental diagnosis. They stated they would not otherwise have attended the hospital and many had noticed nothing untoward even on direct questioning. Half of the patients scored less than three on the check list and 10 out of the 22 cases of nodular melanoma might not therefore have been referred to our clinic. The check list has subsequently been revised and now provides a more-sensitive guide.

    Notes: This is a retrospective review of our experience in the treatment of the cutaneous eruption of mycosis fungoides (MF) with a combination of topical nitrogen mustard (HN2) and photochemotherapy (PUVA). Our aims were to evaluate the efficacy of the combination in clearing the cutaneous eruption of MF, to establish whether the remissions thus achieved could be maintained by the use of HN2 alone, and to determine whether the combination with PUVA would inhibit the development of sensitization to HN2.The regimes employed were found to be an effective means of clearing the plaque lesions of MF. The use of PUVA, either started simultaneously or five to seven treatments prior to HN2, was not found to significantly inhibit the development of sensitization to topical nitrogen mustard. Nevertheless, topical desensitization in those who sensitized allowed the use of long-term HN2 in the great majority of patients. Once the cutaneous eruption had cleared, long-term therapy with HN2 alone was found to be a simple, convenient, and effective means of maintaining the patient in remission. It should not however be regarded as a curative regime, since relapse was common when maintenance therapy was discontinued.

    Notes: This prospective study was undertaken to determine whether the 2-mm punch biopsy technique yields specimens of sufficient size and quality to allow a reliable histologicai diagnosis to be made. A histopathological comparison was made between tissue obtained from a 2-mm punch biopsy and a standard ellipse biopsy taken from a wide range of dermatoses and benign and malignant skin rumours. In 7J of the 84 cases studied, the same histopathological diagnosis was reached with the 2-mm punch biopsy and the standard ellipse. Use of the 2-mm punch biopsy technique produces specimens which allow an accurate histological diagnosis to be made in a wide range of dermatological conditions.
