    Notes: Abstract  A total of 101 cancer patients with 121 febrile neutropenia episodes were randomised to receive empirical treatment with i.v. meropenem (1 g/8 h) or ceftazidime (2 g/8 h). After 3 days, 89% of patients were on unmodified therapy in the meropenem group, compared with 83% in the ceftazidime group. Of the evaluable episodes (n=106), the success rate with unmodified empirical therapy until the end of the treatment course was slightly higher with meropenem than with ceftazidime (48% vs 38%, P=0.39). Furthermore, initial success with further infections was observed in 22% of episodes treated with meropenem and in 13% of episodes treated with ceftazidime. Glycopeptides were used as first modification in 28% and 39% of meropenem and ceftazidime recipients, respectively. Both treatments were well tolerated and there were no reports of drug-related nausea/vomiting or seizures. No significant differences in response rate or in tolerability were observed when analysing only the first febrile episodes. In conclusion, meropenem seems to be as efficacious and well tolerated as ceftazidime and may be associated with a lesser requirement for the addition of glycopeptides.

    Description / Table of Contents: Zusammenfassung Mycobacterium simiae ist ein Bakterium, das in der Natur häufig zu finden und in seiner Pathogenität umstritten ist. Wir berichten über einen Fall vonM. simiae-Infektion mit Erregernachweis im Blut und Krankheitsmanifestationen in der Lunge und an der Haut. Die pathogenetische Bedeutung vonM. simiae war in diesem Fall eindeutig zu beweisen, da es der einzige aus Sputum, Bronchoalveolarlavageflüssigkeit, Blut und Haut angezüchtete Keim war. Die Identifizierung vonM. simiae bereitet bei Verwendung konventioneller Methoden Schwierigkeiten. Um die Identifizierung zu bestätigen, muß, wie im vorliegenden Fall, eine Analyse der Fettsäure- und Mycolsäure-Muster durchgeführt werden.
    Notes: Summary Mycobacterium simiae is commonly found in nature and its role as a pathogen has been controversial. A case of disseminatedM. simiae infection with blood, pulmonary and cutaneous localization is reported here. The pathogenic role ofM. simiae was clearly demonstrated as it was the only organism isolated from sputum, broncho-alveolar lavage fluid, as well as blood and skin tissue. Identification ofM. simiae by conventional testing may be difficult. Analysis of fatty and mycolic acid patterns, as performed in this case, is necessary to confirm its identification.

    Notes: Abstract A 29-month prospective study was carried out in a leukemia unit with the aim of investigating the epidemiology of Clostridium difficile infections and limiting their spread. Systematic cultures of stools and assays for cytotoxin were performed on patient admission and at weekly intervals, yielding 1,355 cultures and assays. The study period was divided in period A, before total unit renovation, and period B, afterwards. During period B all patient carriers of Clostridium difficile received vancomycin. A comparison of the two periods showed that the percentage of positive cultures fell from 16.6 % to 3.6 % and the positive toxin assays from 9.9% to 1.2%. It was concluded that colonization by Clostridium difficile can be prevented in hospital wards with generally high rates of infection by a combination of decontamination of the environment, introduction of preventive measures and treatment of Clostridium difficile carriage with vancomycin.

    Notes: Abstract We retrospectively assessed the clinical coursein four patients with long-standing Crohn's disease whobecame infected with human immunodeficiency virus (HIV).The duration of active Crohn's disease was 21, 10, 4, and 4 years in our four patients.They experienced a stable remission of Crohn's diseasesymptoms after HIV infection. In three patients Crohn'sdisease was in stable remission for 5, 8, and 8 years after HIV infection and all three diedfrom acquired immunodeficiency syndrome-related disease.One patient was still alive without recurrence ofCrohn's disease symptoms 7 years following HIVdetection. Our observations of a spontaneous improvementin the clinical course of Crohn's disease after HIVinfection, suggests that the integrity of the immuneresponse, especially that of CD4 T cells, plays a major role in the tissue injury mechanism in Crohn'sdisease.
