    Notes: Epidermodysplasia verruciformis-associated human papillomaviruses (EV-HPVs) are possibly involved in the development of actinic keratoses and may play a part in the pathogenesis of squamous cell carcinoma (SCC) of the skin, as the DNA of these viruses is frequently detected in biopsies of such lesions. Properly designed epidemiological studies, using serological tests to investigate the role of infection with EV-HPVs in cutaneous oncogenesis, are still rare. An IgG-specific enzyme-linked immunosorbent assay using virus-like particles composed of the major capsid protein L1 of the EV-specific HPV 8 (HPV 8 VLPs) was developed and used to test the seroprevalence of HPV 8 in 114 inhabitants of a tropical island, of whom 13 had developed SCC, and 19 had developed basal cell carcinoma. Gender, age, eye and hair colour, sun exposure and number of actinic keratoses were recorded for all individuals. The presence of antibodies against HPV 8 VLPs was associated with the development of large numbers of actinic keratoses. After adjusting for gender, age, eye and hair colour, and sun exposure, the odds ratio to develop 37 (the median in this dataset) or more actinic keratoses in the presence of antibodies against HPV 8 VLPs was 2·3 (95% confidence interval: 1·0; 5·3). Similarly, after adjustment for the same factors, the presence of these antibodies was associated with SCC with an odds ratio of 3·1 (0·74; 13·3), but the small number of individuals with SCC does not permit any definite conclusions. The presence of these antibodies did not appear to be associated with basal cell carcinoma as, after adjustment for the same factors, the odds ratio was 0·73 (0·23; 2·4). This study provides serological evidence that infection with EV-HPVs may play a part in the pathogenesis of actinic keratoses. The role of EV-HPVs in the development of SCC, however, remains to be elucidated.

    Notes: Objective The aim of this study was to assess the clinical efficacy of long-term (〉 6 months) oral fumarate maintenance treatment regimen and to determine its adverse effect profile.Background We previously showed the efficacy of oral fumarate treatment in psoriasis. However, little is known about long-term maintenance therapy with oral fumarates. Because of concern about renal toxicity and/or other potentially hazardous adverse effects, their role in the long-term management of psoriasis is still controversial and limited.Methods Clinical data and laboratory parameters of 83 psoriasis patients who were treated with oral fumarates during an uninterrupted period of 6 or more months, were collected and subsequently analysed.Results In 31 of the 83 patients the oral fumarate therapy was discontinued preliminarily (〈6 months) because of intolerable adverse effects (9 patients), lack of clinical efficacy (16 patients), insurance refunding problems (4 patients) and lost to follow-up (2 patients), A major improvement of the psoriasis was present in 41 of the 83 patients. Long-term oral fumarate maintenance therapy was accompanied by subjective adverse effects, mainly flushing (28 patients) and gastrointestinal upset (15 patients) and by a relative lymphocytopenia (35 patients).Conclusion Oral fumarates are effective well tolerated drugs suitable for long-term management of psoriasis. However, this systemic therapy still has to be considered experimental and should only be performed under strictly controlled conditions. The cause and the clinical relevance of the relative lymphocytopenia, which occurs in a high percentage of patients during long-term oral fumarate maintenance treatment regimen are unknown and still have to be determined.

    Notes: In a longitudinal study (two measurements with a 1-year interval), 69 patients with psoriasis completed the Illness Perception Questionnaire, the Medical Outcomes Study SF-20 Health Survey, and the Hospital Anxiety and Depression Scale. Data on coping (Utrecht Coping List) and severity of illness (body surface scores) were also collected. The results of regression analyses indicated that a strong illness identity was associated with more visits to the outpatient clinic, and worse outcome on physical health, social functioning, mental health, health perceptions and depression. Strong beliefs that the disease is controllable/curable and that the disease has disabling consequences were also related to more clinic visits and more negative perceived health, respectively. Patients who initially engaged in coping characterized by more expression of emotions, seeking more social support, seeking more distraction, and less passive coping were prescribed a lower number of different therapies, were less anxious, less depressed, and had a better physical health 1 year later. These results have implications for the management of patients with psoriasis, which reinforces current views on integrating psychosocial aspects into clinical care.

    Notes: To evaluate the putative role of human papillomaviruses (HPV) in the development of skin cancer in renal transplant recipients, a series of skin biopsies from premalignant and malignant skin lesions was analysed using the polymerase chain reaction. Four different consensus primer pairs were used. HPV DNA was detected in five of 24 cases of squamous cell carcinoma, in one of three cases of Bowen's disease, in none of four basal cell carcinomas, in two of seven cases of actinic keratosis and in one of five cases of keratoacanthoma. Typing by direct sequencing of the amplified HPV DNA was possible in seven of nine cases, and revealed epidermodysplasia verruciformis (EV)-associated HPV types, or HPV types related to EV-associated types. Hence, HPV DNA could be detected in a significant proportion of (pre)malignant skin tumours in renal transplant recipients. The finding that some of the detected HPV types were as yet uncharacterized EV-related types, suggests that HPV DNA could be present in a higher percentage of lesions, and might be detected with refinement of the techniques.

    Notes: Summary The expression of intercellular adhesion molecule-1 (ICAM-1) on keratinocytes (KC) was previously demonstrated in biopsies from various inflammatory skin lesions. KC were, however, found virtually ICAM-1 negative, in normal skin, when the same immunocytochemical techniques were employed. By contrast, epithelial cells resident in different organs constitutively express ICAM-1, albeit weakly. The aim of the present study was to use an immunostaining system more sensitive than the conventional immunocytochemistry, namely the in situ immunogold labelling of ultracryosections, to investigate the constitutive ICAM-1 expression by resting KC in normal skin, in vivo. The semiquantitative analysis performed on 500 resident KC, visualized within tissue ultracryosections of normal human skin, revealed that gold granules were present along the cell membrane in a small percentage (14·6%) of resident KC. The density of gold particles (10 nm sized) observed on the cell surface per KC section was as scarce as 13·72 ± 4·6 (mean ± standard deviation), although highly significant when compared with controls (P 〈 0·005). This indicates the presumably low expression of ICAM-1 moieties on the plasma membrane of this KC subset. This ICAM-1 expression could be important in modulating the trafficking to and from normal epidermis of migrating Langerhans cells and occasional leucocytes. The fact that the ICAM-1 expression on KC in normal skin is limited can be considered favourable, because it can account for the prevention of inappropriate KC/leucocyte interactions in the resting cutaneous environment.

    Notes: A 30-year-old man had suffered from persistent ulceration within an area of necrobiosis lipoidica diabeticorum for 13 months. The ulcerating necrobiosis lipoidica was resistant to topical therapy and oral therapy with acetylsa-licylic acid. However, the ulcers healed completely within 8 weeks of administration of 400 mg pentoxifylline twice daily.

    Notes: At present, initial high-dose prednisone is the treatment of choice for patients with pemphigus and bullous pemphigoid. To reduce the risks associated with long-term corticosteroid treatment, other immunosuppressants are often given as steroid-sparing agents. Occasionally, the dose of steroids cannot be reduced. In this study, we report six patients with pemphigus vulgaris, pemphigus foliaceus and bullous pemphigoid, in whom the daily corticosteroid dose could only be tapered to acceptable, effective, maintenance levels following treatment with high-dose intravenous gammaglobulin.

    Notes: In this study, we characterized the stratum corneum barrier function in 39 patients with various keratinization disorders (autosomal dominant ichthyosis vulgaris [ADI] [n=7]. X-linked recessive ichthyosis [XRI] [n=6], autosomal recessive congenital ichthyosis [C1] [N=10]. dyskeratosis follicularis [Darier's disease: DD] [n=8], erythrokeratoderma variabilis [EKV] [n=8]). and 21 healthy volunteers, using two non-invasive methods: transepidermal water loss (TEWL) measuring outward transport of water through the skin by evaporimetry. and the vascular response to hexyl nicotinate (HN) penetration into the skin as determined by laser-Doppler flowmetry.Significantly increased TEWL values were found on the volar forearm in all three forms of ichthyosis, compared with the healthy control group, with the highest TEWL values in the CI group. The penetration of HN on the volar forearm was accelerated in patients with ADI. XRI and CI, as indicated by a shorter lag time (to) between HN application and initial vascular response. However, differentiation between CI and the other ichthyoses was not possible by this method. When using both methods in DD and EKV. no differences compared with the healthy controls could be detected on the volar forearm, where the skin was principally unaffected: only the measurements from the affected skin on alternative sites demonstrated significantly increased TEWL values. In ADI and CI. however, normal-appearing skin also showed impaired values.We conclude that both TEWL and the vascular response to penetration of HN are suitable methods to monitor the skin barrier function in keratinization disorders, and are helpful in discriminating between these disorders.

    Notes: Both fresh and cryopreserved cultured epithelial allografts were used to treat 20 patients with a total of 30 chronic ulcers. A profound stimulation of host epithelialization from the wound edges and epidermal appendages was observed. Our data suggest that allografts, prepared using a simple cryopreservation technique, retain their morphological and functional characteristics, and are as effective as fresh allografts in healing chronic ulceration.
