Abstract In rats, serum secretory component (SC) iselevated in mechanical but not hepatocellularcholestasis. To determine if serum SC might distinguishcholestatic syndromes in humans, serum samples wereobtained from control subjects and patients withmechanical and hepatocellular cholestasis. Equal volumesof serum were assayed for SC by immunoblotting with anantibody specific for human SC. Quantitativedensitometry of these immunoblots showed that inmechanically obstructed patients serum SC was reversiblyelevated to a level ~10-fold higher than that ofpatients with hepatocellular cholestasis (P 〈 0.001).When comparing the two cholestatic groups, levels ofserum alkaline phosphatase, but not bilirubin andalanine aminotransferase, were significantly higher inthe group with mechanical cholestasis (P 〈 0.01). When comparing individual patients, serum SCwas more reliable than alkaline phosphatase indistinguishing the two cholestatic syndromes (P 〈0.05). Thus, serum SC may distinguish mechanical fromhepatocellular cholestasis in humans.
