    Notes: Isometric tension was recorded in ring preparations of human superficial temporal arteries contracted by noradrenaline (NA), 5-hydroxytryptamine (5-HT), and ergotamine. In contrast to NA and 5-HT, ergotamine induced long-lasting contractions refractive to additional stimulations and resistant to repeated wash-out. When tested against 5-HT, ergotamine acted as a non-competitive antagonist. When repeating the 5-HT stimulations (4.7 × 10-5 M) the contractile response decreased indicating tachyphylaxis to this agent. As ergotamine revealed both a vasoconstrictive and a 5- HT-blocking activity, the beneficial effect in migraine may be by an interference during both the vasoconstrictory and vasodilatory phases.

    Notes: Clinical findings of four female members from one family with familial hemiplegic migraine are briefly summarized. Cerebral blood flow (CBF) studies using the xenon 133 inhalation method were carried out during and between hemiplegic attacks in two of the family members. CBF was significantly lower over the affected hemisphere during attacks, while equal flow on both sides was seen in headache free periods. The findings indicate that cerebral perfusion is altered, but not necessarily decreased during attacks of familial hemiplegic migraine.

    Notes: Summary Two cases are reported of malignant, cerebral oligodendroglioma with diffuse dissemination to the ventricular system and subarachnoid space. All metastases were gelatinous and mucin-positive. In one case multiple, small tumours of macroscopic size in the roots of the cauda equina were an exceptional finding. Differential diagnostic problems are briefly discussed.

    Notes: Summary The series studied comprises all 851 patients with symptom-producing intracranial saccular aneurysms admitted to the departments of neurosurgery in Denmark in the five-year period of 1970–1974. The series was divided into seven clinical stages according to Hunt's classification. More than half of the patients were grouped in stages IV and V (Hunt grade 2–3). Of the patients, 76% were found primarily suitable for operation. The mortality within the individual stages was to a great extent independent of the time of operation. A total of 94% of the aneurysms were localized within the region of the carotid artery, with a roughly equal distribution among the internal carotid, anterior communicating, and middle cerebral arteries. Multiple aneurysms were found in 19% of the patients subjected to panangiography. There was a distinct correlation between the severity of the spasms and the clinical condition. The course of operation was complicated in 49% of the patients, the complications being equally distributed within the various clinical stages. The mortality for the patients who underwent operation (total 567) was 32%, and 80% of the deaths were due to direct or indirect consequences of aneurysmal bleeding. At the follow-up performed two to seven years after operation, 52% of the survivors were fully capacitated, 20% were partly capacitated, and 28% were incapacitated. Based on a retrospective analysis, we have started a comprehensive prospective study with registration of available parameters in patients with saccular aneurysms admitted to all departments of neurosurgery in Denmark.

    Notes: Summary Specific antagonists to the influx of calcium, necessary for the excitation-contraction coupling process in arterial smooth muscle, are potentially useful in the treatment of cerebral vasospasm but systemic hypotension might limit their clinical applicability. We studied the effect of the calcium antagonist nimodipine (BAY e 9736) on cerebral arterial spasm, intraventricular pressure and blood pressure (BP), when administered into the cerebral ventricles of the dog. Cerebral vasospasm was produced by the injection of autologous blood into the cisterna magna. In a group of 8 dogs, 100 μg of nimodipine was injected into the lateral ventricle. The effect of the drug on the basilar artery was monitored angiographically. Nimodipine always relieved spasm, and often the relaxation surpassed the resting vessel diameter. In a control group, the injection of placebo did not relax the spastic arteries. Determinations using gas chromatography of nimodipine in CSF and blood demonstrated that a concentration of 1 μg/ml in cisternal CSF was sufficient to reduce spasm while concomitant plasma concentrations of 0.004 μg/ml did not result in significant BP reduction.
