    Notes: Summary.— Using an instrument designed to exert known pressures on the skin, an incidence survey of factitious wealing in the general population has been carried out; 2813 subjects have been tested with a pressure of 4900 g/cm2, and the overall incidence was 4·24% (females, 5·2%; males, 4·34%). The incidence did not vary significantly in the different age groups.A series of patients with symptomatic dermographism has been followed up in the outpatient Department and serial measurements of the weal size have been made. In 20 of these comparable measurements are available over prolonged periods. The total number of patients showing at least 50% reduction in weal size was: at 2 months, 1; at 4–6 months, 7; at 6–9 months, 10; at 9–12 months, 12.The incidence of factitious wealing in various patterns of urticaria is mentioned and the results of the incidence survey are discussed in relation to patients presenting with symptomatic dermographism.

    Notes: Summary.— Specimens of faeces from 217 children with napkin rashes aged less than 3 months were examined for the presence of Candida albicans The 85 infants from whose faeces C. albicans was isolated were divided into 2 groups. The children in one group were treated with oral and local nystatin, and those in the other group were given an oral placebo and local nystatin. Further specimens of faeces were examined 2 weeks and 2 months after starting treatment.The children who received oral nystatin were less likely than the others to have C. albicans in subsequent faecal specimens, and their napkin rashes were less likely to relapse; but no differences reached statistical significance. One-third of the children who received the oral placebo also lost the C. albicans from their faeces within 2 weeks, and it is suggested that host factors determine whether or not infection by C. albicans is transitory or is followed by a period of commensalism.

    Notes: SUMMARY.— When a weal evoked as a cutaneous allergic response has disappeared, that area of skin, although appearing normal, shows factitious wealing: this tendency persists for 72–96 hr. The phenomenon is present after weals caused by positive intradermal prick tests from various substances, after weals from certain insect bites, and on the site of fading lesions of acute urticaria. Weals caused by intradermal histamine are not followed by this factitious wealing, nor is it present on skin which has been the site of a weal in chronic urticaria.

    Notes: SUMMARY.— Clinical observations are made on a series of 22 patients with cholinergic urticaria, representing 0·2% of total referrals to dermatological out-patients and 7% of patients with urticarial eruptions. The average age of onset was 16 and the average duration 7·5 years.The weals are widely scattered and although some coincide with follicles, others arise between them.The precipitating factors are discussed, particularly in relation to water contact; it is concluded that this acts only when heat or exercise are coincident.The fact that the same precipitating factors can play a part in other physical urticarias often leads to confusion in diagnosis.Treatment is briefly discussed and a double blind comparison of hydroxyzine with antihistamine therapy in 14 patients is described; hydroxyzine was preferred in 8 patients.

    Notes: 56 patterns with chronic urticaria were patch tested with particular reference to the immediate weal response to a range of materials and this was followed by challenge tests to most of the same substances.Positive immediate patch tests were commonly seen with balsam of Peru and cinnamon and this appears to be a reaction in many normal subjects.Positive challenge tests in patients who also had positive immediate patch tests to the same substance occurred in two patients with cinnamon and two patients with cloves. Other substances with a similar action may come to light- in which case the use of immediate patch tests in investigating chronic urticaria is more likely to be of value.4 patients showed positive immediate patch tests to nickel, one of whom also had a positive 48 h patch test to nickel. 2 other patients had only positive 48 h patch tests lo nickel. However, none of these patients pave a positive challenge lest lo nickel.

    Notes: A classification of urticaria pigmentosa with special reference to the local appearances, general effects, distribution and age is described.The various types of factitious urticaria are mentioned. Because of the similar appearances of factitious urticaria in diffuse cutaneous mastocytosis and dermographism, it might be expected that there would be an increased number of mast cells in the skin in the latter condition. This, however, has not been demonstrated, but it is still possible that some functional abnormality of mast cells exists in dermographism.
