    Notes: SUMMARY.— The problems of treatment of histiocytosis X with x-ray, cortisone, methotrexate, and vinblastine are briefly reviewed. In all 16 patients with focal histiocytosis X of bone, there was a good response to surgery or roentgen therapy. In 2, the lesions healed after surgery for biopsy alone. In histiocytosis X of the skin, the response to roentgen therapy was good when ulcerative lesions of the mucous membranes occurred. Corticosteroid therapy resulted in the improvement of 6 of 10 patients who bad pulmonary histiocytosis X, but in only 3 did the roentgenogram of the lung show complete clearing. In some children with systemic histiocytosis X, there was improvement with corticosteroids or methotrexate or both. Six children treated with intravenously administered vinblastine have achieved remission or remarkable improvement.

    Notes: SUMMARY.— The epidermal barrier unit is composed of cells of the keratinocytic, transitional, and horny layers. The epidermis often contains other dendritic, leukocytic, and neural cells in the intercellular spaces. The epidermal intercellular canals and epidermal phagocytosis are 2 routes of transepidermal and transcorneal passage of materials. Individual, regional, age, and species differences in epidermis may account for variations in cutaneous permeability. Mitotic rate may be the most significant factor in barrier production.

    Notes: SUMMARY. Three cases are reported of necrotising vasculitis occurring with or preceding the onset of a malignant reticular disease. The vasculitis occurred at about the time lymphosarcoma was diagnosed in one patient who had received penicillin. In a second patient the appearance of vasculitis preceded the diagnosis of Hodgkin's granuloma by 2 months. And in a third patient, vasculitis was present about 6 years prior to the development of possible multiple myeloma. These cases emphasize that vasculitis and lymphoma may coexist. The clinician must be alert to the possibility of the development of lymphoma in patients with necrotising vasculitis of chronic course. Treatment with immunosuppressive drugs in two patients resulted in only temporary improvement, and the skin lesions recurred in spite of continued therapy. This raises some doubt concerning the immunological etiology of necrotising vasculitis.

    Notes: Acropustulosis, or chronic palmar plantar pustulosis (PPP), is a phenomenon of recurrent sterile pustules, erythema, and scaling affecting the palms and soles. Its pathogenesis is unclear, and it is difficult to treat. The purpose of this study was to elucidate further the factors involved in causing PPP, thereby enhancing the ability to manage this disease. All cases of PPP seen at Mayo Clinic Scottsdale from 1987 to 1993 were reviewed. 21 patients with PPP were identified, 15 of whom had been patch tested. 9 of the 15 patients (60%) showed positive patch test results. Fragrance was the most common sensitivity, but nickel, formaldehyde, para-phenylenediamine, thiuram, neomycin, mercury, balsam of Peru, and cinnamic aldehyde sensitivities were demonstrated. Less important factors included atopy, fungal and bacterial infections, and irritation. Although the mechanism of this sterile pustulosis response does not depend solely on delayed hyper sensitivity mechanisms, we believe that we have demonstrated such a large number of positive patch tests in this chronic pustular dermatosis that patch testing should be considered in the routine work-up of these patients.

    Notes: Clinical and histopathologic data in 8 cases of epidermoid hidradenoma are presented. Most of the tumors were in the head and neck, and most of the patients were middle-aged and older adults. Generally, the lesions were asymptomatic nodules that sometimes showed ulceration or rapid growth. Despite having some histologic atypia or focal mitotic activity, the tumors were found to be benign on long-term evaluation. This epidermoid variant should not be otherwise differentiated from the benign group of solid-cystic hidradenomas, because cytologic variability did not predict a significant change in prognosis.

    Notes: The characteristic histologic change seen at 2 h in the Rumpel-Leede (Hess) test is, in addition to focal hemorrhage in the upper dermis, a perivascular lymphocyte infiltrate with focal areas of lymphocytic epidermal invasion. The histologic findings, even with the liquefaction degeneration, are similar to those of purpura simplex and indicate that, early in many forms of vasopermeability, the lymphocyte may be a primary inflammatory cell.

    Notes: Two cases of tubular apocrine adenoma were studied by light, histochemical, and electron microscopic methods. This benign apocrine hamartoma must be considered in the classification of appendage tumors, particularly those associated with organoid epithelial nevus, and may be confused with metastatic adenocarcinoma. The tumor is characterized by dermal and subcutaneous lobular masses of tubular structures. Connection to the epidermis by one or more ductlike structures is observed. The tubules contain typical apocrine epithelial cells, some with hyaline or clear cell differentiation. The tumor cells have apocrine differentiation by histochemistry and ultrastructure. The localization to the scalp and the nodular, lobulated appearance and the apocrine structures indicate that this tumor is similar to nevus syringocystadenoma papilliferum and must be differentiated from it.
