Summary Three well differentiated and 3 de-differentiated papillary adenocarcinomas of human endometrium were studied electron-microscopically. In comparison to normal endometrial glandular cells, with increasing de-differentiation the tumor cells showed an irregular layering of the cells, and a distinct tendency for irregular, almost structureless processes to extend out against neighboring cells and stroma. In addition, the microvilli were partially absent, the endoplasmic reticulum was decreased, and there was a swelling of the mitochondria whose cristae were sparse and revealed only a slight contrast. With morphometric methods it could be shown, that glands of the normal endometrium, of glandular hyperplasia, and of variously differentiated papillary adenocarcinomas possess about the same mitochondrial volume (“intramitochondrial space”). On the other hand, the portion of cytoplasmic volume taken up by the osmiophilic granules in glandular hyperplasia was 2.5 times that found in normal glandular cells, and in well differentiated adenocarcinomas was increased about 6 times. In contrast, the de-differentiated tumors again showed lower values which were in the range of values for normal glandular cells. The stromal papillae at the base were demarcated from the tumor cells by a normally structured basement membrane. Towards the tip the basement membrane ceased. The boundary was formed by finely threaded structures which could represent pre-collagen filaments. The papillary tips consisted of a homogeneous basement membrane-like material with wide lymph spaces into which the tumor cells penetrated with villus-like basal projections.

Zusammenfassung Drei gut ausdifferenzierte und drei entdifferenzierte papilläre Adenocarcinome des menschlichen Endometriums wurden elektronen-mikroskopisch untersucht. Mit zunehmender Entdifferenzierung zeigten die Tumorzellen im Vergleich zu normalen endometrialen Drüsenzellen eine unregelmäßige Schichtung der Zellen, eine deutliche Neigung, unregelmäßige, strukturarme Fortsätze gegen Nachbarzellen und Stroma vorzutreiben. Außerdem bestand ein teilweiser Verlust der Mikrovilli, eine Verminderung des endoplasmatischen Reticulum und eine Schwellung der Mitochondrien, deren Cristae spärlich waren und einen geringen Kontrast aufwiesen. Mit morphometrischen Methoden konnte gezeigt werden, daß Drüsen des normalen Endometriums, bei glandulärer Hyperplasie und unterschiedlich ausdifferenzierten papillären Adenocarcinomen etwa das gleiche Mitochondrium-Volumen (“intramitochondrialer Raum”) besitzen. Der Volumen-Anteil der osmiophilen Granula am Cytoplasma-Volumen stieg, im Vergleich zu normalen Drüsenzellen, bei der glandulären Hyperplasie auf das 2,5fache und bei gut ausdifferenzierten Adenocarcinomen auf etwa das 6fache an. Die entdifferenzierten Tumoren wiesen hingegen wieder niedrige Werte auf, welche in der Größenordnung normaler Drüsenzellen lagen. Die Stroma-Papillen werden an der Basis von einer normal gebauten Basalmembran gegen die Tumorzellen abgegrenzt. Weiter zur Spitze hin bricht die Basalmembran ab, die Abgrenzung erfolgt durch feinfädige Strukturen, die präkollagenen Filamenten entsprechen könnten. Die Papillenspitze hingegen besteht aus einem homogenen basalmembranartigen Material mit weiten Lymphspalten, in das die Tumorzellen mit basalen zottenartigen Vorragungen eintauchen.
