Zusammenfassung 15 Stämme vonEnterococcus faecalis, alles klinische Isolate aus Blutkulturen von Patienten mit Endokarditis, wurden in Experimenten zur Abtötungskinetik mit Penicillin G, Ampicillin and Amoxicillin allein und in Kombination mit Tobramycin untersucht. Die minimalen Hemmstoffkonzentrationen waren für Penicillin 4 mg/l, für Ampicillin 2 mg/l, für Amoxicillin 2 mg/l und für Tobramycin 32 mg/l. In den Experimenten zur Abtötungskinetik wurden äquipotente Dosen der Antibiotika 1/2 × MHK, 1 × MHK, 4 × MHK) verwendet. Synergismusstudien wurden mit einer Kombination des β-Laktamantibiotikums bei 1/2 × MHK und Tobramycin in einer Konzentration von 8 mg/l durchgeführt. Die bakterizide Aktivität eines jeden Kombinationspartners war nicht höher als 10+3 KBE/ml zur Stunde 5. Nach fünf Stunden fand sich bei allen drei β-Laktamantibiotika eine synergistische Wirkung mit Tobramycin, d. h. eine mehr als 100fache Verminderung der Koloniebildnerzahlen im Vergleich zu der wirksamsten Substanz allein. Amoxicillin zeigte eine signifikant höhere bakterizide Wirkung als Ampicillin oder Penicillin und zwar sowohl allein als auch in Kombination mit Tobramycin. Die klinische Bedeutung dieser Befunde muß inIn-vivo-Studien weiter untersucht werden.

Summary Fifteen strains ofEnterococcus faecalis, all clinical blood culture isolates from patients with endocarditis, were studied by kill-kinetic experiments using penicillin G, ampicillin and amoxicillin alone and in combination with tobramycin. The median minimal inhibitory concentrations (MIC), were penicillin 4 mg/l, ampicillin 2 mg/l, amoxicillin 2 mg/l and tobramycin 32 mg/l. Equipotent doses of the antibiotics (1/2 × MIC, 1 × MIC and 4 × MIC) were used in the kill-kinetic studies. Synergism was studied using a combination of 1/2 × MIC of the beta-lactam antibiotic and 8 mg/l of tobramycin. The bactericidal activity did not exceed 10+3 cfu/ml at 5 hours for any single compound. After 5 h all three beta-lactam antibiotics in combination with tobramycin resulted in synergism, i.e. more than one hundredfold reduction of colony forming units (cfu) as compared to the most active single agent. Amoxicillin had a significantly higher bactericidal potential than ampicillin or penicillin both alone and in combination with tobramycin. The clinical significance of these findings warrants further studiesin vivo.
