Of six patients treated on the humerus by implantation of internal titanium fixators, two had fractures of the surgical neck, three had pseudoarthrosis of the diaphysis, and one a fracture of the diaphysis. The internal titanium fixators used were identical to those used for the tibia. A minimally invasive approach to the surgical neck was performed by splitting the deltoid muscle, and, in patients with fracture and nonreduction of the humeral diaphysis, a partially minimally invasive approach was used, placing two small incisions and creating a subcutaneous tunnel for plate insertion. Bone healing was uneventful in all patients. Hardware complications occurred in one patient with a multiple fracture of the surgical neck, necessitating early removal of the implants, as the screws had begun to loosen. Bone healing had already commenced. These results confirm similar experiences with other body regions: the angular stable interlocking plate system has proven its value as a method of treatment, especially in patients with osteopenia and reduced bone quality due to osteoporosis.

Es wird über 6 Anwendungen eines Titanfixateurs interne am Humerusschaft und proximalen Humerus berichtet. In 2 Fällen erfolgte die Versorgung von subkapitalen Humeruskopffrakturen Typ Neer IV mit einem Titanfixateur interne. Weiterhin wurden 3 Humerusschaftpseudarthrosen bzw. 1 Humerusschaftfraktur durch einen Titanfixateur interne versorgt, welcher mit dem Unterschenkeltitanfixateur interne identisch ist. Der Zugang im Bereich des Humeruskopfs erfolgte in 1 Fall minimalinvasiv durch ein Deltasplitting. Im Oberarmschaftbereich erfolgte die Implantation teilweise minimalinvasiv durch 2 kleine Inzisionen. Der Heilungsverlauf war bei allen Patienten ungestört. Es kam in allen Fällen bisher zur knöchernen Konsolidierung. Bei einer subkapitalen Humeruskopfmehrfragmentfraktur war es bei unveränderter Frakturstellung zu einem Heraustreten der Schrauben aus dem Knochen gekommen. Hier erfolgte bei knöcherner Konsolidierung eine vorzeitige Materialentfernung. Die Erfahrungen im Bereich des Humerusschafts und des proximalen Humerus bestätigen die Erfahrungen aus anderen Körperregionen. Winkelstabilität bewährte sich hier besonders im spongiösen Knochen und bei verringerter Knochenqualität durch Inaktivitäts- oder Altersosteoporose.
