    Notes: Background The prevalence of lower urinary tract symptoms was determined by survey as an initial step in estimating the significance of benign prostatic hyperplasia (BPH) in Asia and Australia. Methods The symptom index (0 to 35) and quality-of-life (QOL) index (0 to 6) of the international prostate symptom score were measured in 7588 men in 9 Asian countries and 146 men in Australia. Results The percentages of Asian men considered to be symptomatic (symptom index ≧ 8) were 18%, 29%, 40%, and 56% in the age groups of 40 to 49, 50 to 59, 60 to 69, and 70 to 79 years, respectively. For Australian men, these figures were 36%, 33%, and 37% in the 50 to 59, 60 to 69, and 70 to 79 year age groups, respectively. Conclusions Our estimates indicate that the prevalences of symptomatic men in Asia and Australia are similar to or greater than those in Europe and America, and suggest BPH is similarly common in these areas.

    Notes: Background: We performed intraoperative cavernous nerve stimulation with an intracavernous pressure (ICP) monitoring system to confirm nerve sparing during radical pelvic surgery and assessed the results.Methods: Nineteen cases of radical prostatectomy and three of radical cystoprostatectomy were examined. Electrical stimulation of the site where the neurovascular bundle (NVB) was determined to run was performed and changes in ICP were measured before and after prostate removal.Results: Of the 22 patients, bilateral NVBs were preserved in six patients while unilateral NVB was preserved in 16. Before dissection, all NVBs examined exhibited positive responses (ICP changes of〉5 mmHg) to nerve stimulation. After removal of the prostate, positive responses were observed in 22 (79%) of 28 macroanatomically preserved NVBs. Of 16 sides on which the NVB was not preserved, there were positive responses in five (31%). In these patients, some nerve fibers were macroscopically observed lateral to the original site of NVB. Finally, bilateral or unilateral nerve sparing was confirmed electrophysiologically in 20 (91%) of the 22 patients.Conclusion: Intraoperative stimulation of the NVB while monitoring ICP changes is a simple and reliable method of accurately evaluating the preservation of cavernous nerves. This system may provide further insight into the mechanism of postoperative erectile dysfunction.

    Notes: Aim: Vitamin D acts as an antiproliferative agent against prostate cells. Epidemiological study has shown that a low level of serum vitamin D concentration is a risk factor for prostate cancer. Vitamin D acts via vitamin D receptor (VDR), and an association of genetic polymorphisms of the VDR gene has been reported. In the current study, we examined the association of VDR gene polymorphisms with familial prostate cancer in a Japanese population.Methods: We performed a case–control study consisting of 81 familial prostate cancer cases and 105 normal control subjects. Three genetic polymorphisms (BsmI, ApaI and TaqI) in the VDR gene were examined by the restriction fragment restriction length polymorphism method.Results: Overall, there was no significant association of the VDR gene polymorphisms with familial prostate cancer risk in the cases and control subjects. However, a weak association between BsmI or TaqI genotypes and cancer risk was observed in subjects under 70 years of age. Stratification of cases by clinical stage or pathological grade did not show significant association between the VDR gene polymorphisms and prostate cancer risk.Conclusion: In the present study, we could not confirm any significant association between VDR gene polymorphisms with familial prostate cancer risk in a Japanese population. Further large-scale case–control studies are warranted to confirm the importance of VDR gene polymorphisms in familial prostate cancer.

    Notes: Background The effect of an artificial testicular prosthesis on sexual disturbance in hypogonadal men was investigated. Methods Twelve hypogonadal men were treated with testosterone replacement therapy delivered via radiation-polymerized testicular prostheses surgically implanted in their scrotums. Results An average serum testosterone level of over 200ng/dl was maintained for 48 months. Most patients treated with this prosthesis were satisfied with the improvement of their sexual activity. Moreover, the patients were psychologically more satisfied with the enlarged scrotum achieved with the implanted prosthesis. Conclusion We recommend this prosthesis as the first choice among testosterone-replacement therapies for hypogonadal men.

    Notes: Background : Few articles describe stage and pathology distribution of prostate cancer by age, yet understanding these topics may provide clues to the etiology of this disease. The present study describes the distribution of stage and pathology of prostate cancer in terms of age and considers effective detection of early stage cancer by mass screening (MS). Methods : Data presented in this study were based on patient’s files from MS and the Gunma Urological Oncology Study Group (GUOSG) registry from 1981 to 1996 in Gunma Prefecture, Japan. Results : Age did not significantly correlate with stage distribution among the GUOSG patients, whereas stage tended to advance with age among the MS patients. Stage B was detected more frequently than any other stage in all age groups of the MS patients, whereas stage C and stage D were detected at similar rates in all age groups. After prostate specific antigen (PSA) had been introduced in MS, the percentage of early stage cancer increased at an earlier age. Pathology distribution did not significantly differ in any group. Conclusions : The data obtained from MS cancer patients showed that tumor stage tends to advance with age. We believe that the younger age group should be examined by MS using PSA to effectively detect early prostate cancer.

    Notes: Background: Although the incidence of urogenital cancers in Japan is lower than that of other cancers, it is increasing steadily. Thus, an epidemiologic study was necessary to determine the measures that would decrease the mortality rate associated with these cancers. Methods: The subjects were 4759 patients with urogenital cancer who were living in Gunma Prefecture and who were newly diagnosed between 1985 and 1994. The data were analyzed by year and by patients' ages. The incidence rates of each disease were expressed as the number of cases per 100,000/year, and age-adjusted rates were adjusted to the world population. Results: The number of males and females afflicted by urogenital cancers increased over the 10-year period. The increase in age-adjusted incidence rates was sharpest for prostate, renal cell, and testicular cancers among males, and for renal cell, renal pelvic and ureter cancers among females. When age-specific rates were plotted against age on double logarithmic scales, the cancers were classified as type 1 (linear), type 2 (linear until a certain age, then flattening out or decreasing), or type 3 (irregular) based on the slope of the line. The magnitude of increase in the age-specific incidence rates of type 1 cancers with age was on the order of the 12th power for prostate cancer and the 5th power for bladder cancer. When the 10 years were divided into 2 periods (earlier and later), the age-specific incidence rates of prostate and renal cell cancers increased in all age groups, whereas the age-specific incidence rates of cancers that increased less sharply remained stable or even declined in some age groups. Conclusion: These epidemiologic data should be useful in reducing the mortality rates associated with these cancers.

    Notes: We identified a family of 4 brothers with prostate cancer. The proband was the second son who was seen at the Department of Urology, Gunma University, Gunma, Japan, in May 1995, with a chief complaint of dysuria. The serum level of PSA was high, and a prostate needle biopsy revealed a moderately-differentiated adenocarcinoma. Subsequently, a total prostatectomy was performed. The family history revealed that the fourth son had been under treatment at the Saitama Cancer Center Hospital for prostate cancer since 1994. Therefore, the other 2 brothers became healthy male family members of the familial prostate cancer line, and their serum PSA levels were examined after obtaining their informed consent. Both had a high serum PSA level and biopsies revealed moderately-differentiated adenocarcinomas in both. Human leukocyte antigen class II gene typing was conducted on 3 of 4 brothers and 4 healthy family members who gave informed consent, and revealed the DRB1 *0901 -DQB1 *0303 haplotype in the first, second and third sons. Familial prostate cancer is well documented in Europe and the US where the incidence of prostate cancer is high, but this is the first report of prostate cancer in 4 brothers in Japan.

    Notes: We report a case of transitional cell carcinoma arising in the fossa navicularis of the male urethra. The patient was 87-years-old and noticed intermittent urethral bleeding. We could see a tumor protruding from the urethral meatus, removed it successfully, and irradiated the base of the tumor with Nd:YAG laser. The histologic finding was grade 2 transitional cell carcinoma, pTa. There was no other tumor in the urinary bladder or upper urinary tract. The postoperative course was uneventful and the patient showed no evidence of disease for 4 months after treatment but died from other causes.

    Notes: Background:Familial prostate cancer has been studied in Europe and the United States. This study was conducted toclarify the clinical features and incidenceof the human leukocyte antigen (HLA) in familial prostate cancer in patients seen at hospitals in Japan.Methods:The age at diagnosis, clinical stage, histologic differentiation, prognosis, cause of death and family history were compiled from 43 patients from 20 families with familial prostate cancer (group F) and compared in patients with sporadic prostate cancer (group S). In addition, HLA class II gene typing was performed in 17 patients from 11 families with familial prostate cancer (group F') and their healthy family members (15 males and 25 females).Results:The age at diagnosis was significantly lower in group F patients (P 〈 0.001), but neither clinical stage nor prognosis differed between group F and group S, yet the incidence of moderately differentiated adenocarcinoma was significantly higher in group F (P& 0.01). There were no historical factors that were specific for group F patients. The frequency of HLA-DRB1 *0901 and *1201 alleles was significantly higher in group F' patients than in group J (the frequency of alleles in 1216 Japanese people attending the 11th Japan HLA Workshop), but not between group F' and their healthy male family members.Conclusion:There were no significant differences in clinical parameters between group F and group S, except that the age at diagnosis was lower in group F patients, and that the frequency of several HLA class II alleles was significantly higher in patients with prostate cancer. In men with a family history of prostate cancer, examinations for early detection of prostate cancer should begin at a young age.

    Notes: Background:Expression of transforming growth factor-β (TGF-β) in the prostate has been reported. The purpose of this study was to evaluate the effect of TGF-β on collagen synthesis by stromal cells isolated from a patient with benign prostatic hyperplasia (BPH). Methods:Human prostatic stromal cells (HPSC) derived from BPH tissue were cultured in serum-free medium. After incubation with TGF-β1, the concentration of procollagen type I C-peptide (PIP) in the HPSC-conditioned medium was measured by enzyme immunoassay while the concentration of procollagen type III N-peptide (PIIIP) was measured by radioimmunoassay. Per-cell production of each type of collagen was calculated by multiplying the measured concentration by the volume of medium and dividing by the number of harvested cells. Results:One ng/mL of TGF-β1 significantly (P〈 0.05) increased collagen production by HPSC, to 220% and 120% of control for types I and III, respectively. Increasing the amount of TGF-β1 to 10 ng/mL had no further effect. TGF-β1 did not significantly affect HPSC number at concentrations of either 1 or 10 ng/mL. There was a strong correlation between PIP and PIIIP production (r= 0.929, P〈 0.001). Conclusion:These findings suggest that TGF-β stimulates accumulation of extracellular matrix in stromal BPH tissue without affecting proliferation of stromal cells.
