    Notes: There has been an increase in the use of primary reference material as a standard for identifying the cellular response to biomaterials. One such material is NHLBI-DTB polydimethylsiloxane (PDMS). The PDMS was developed for blood contacting studies and is composed of PDMS backed on one side with mylar. The results of implantation studies of two different publications are discussed in light of the different materials and different surface topographies of each of the materials. The appropriateness of in vivo studies using this reference material is questioned. © 1993 John Wiley & Sons, Inc.

    Notes: The research field of biomaterials and surgical implants has matured to a point suggesting that a formal and comprehensive education is now required to handle all professional issues related to biomaterials and implant development. A professional curriculum is proposed for a discipline of biomaterials science and engineering on a graduate level. The curriculum includes the definition of an essential knowledge base and describes two track options for a study period of 3 years. Lists of prerequisites as well as required and suggested courses are presented and discussed. Continuing education courses are presented as examples. A quick vision of the immediate future of the field enforces the need for biomaterials professionals to take the lead in bringing the field into the next century. © 1995 John Wiley & Sons, Inc.

    Notes: Surgical implants need to be free from contaminants before implantation. The effectiveness of a presently used Clemson bioengineering cleaning (CBC) protocol was evaluated for cleaning three different biomaterials (titanium, aluminum oxide, and polyethylene terephthalate, PET) contaminated with three different contaminants (calcium chloride, zinc chloride, and hexadecane). Radiolabeled tracer analysis (RTA), with the use of liquid scintillation, was used as the surface analytical technique to quantitatively determine the removed from the biomaterial surface. On average, the ultrasonic cleaning step removed 99.96% of all three contaminants from both titanium and aluminum oxide. The CBC protocol did not sufficiently clean PET fabric contaminated with hexadecane leaving 11.76% of the contaminant after the ultrasonic step. With the use of isopropyl alcohol in series with 1% Liquinox, the ultrasonic step cleaned the fabric soiled with hexadecane within 30 min, removing 99.85% of the hexadecane initially on the surface. RTA proved to be an excellent method of quantifying surface contamination on implant materials, and for assessing the effectiveness of cleaning protocols in question. © 1995 John Wiley & Sons, Inc.

    Notes: Polyethylene terephthalate in textile form (Dacron®) has been used extensively as a surgical implant material for applications such as vascular grafts and percutaneous access devices. It is moderately histocompatible eliciting a chronic inflammatory reaction predominnately in tissue which has grown into the pores of the fabric. Titanium implants on the other hand, induce only the slightest inflammatory response and connective tissue adhesion to the titanium oxide surface is excellent. It was, therefore, hypothesized that a titanium coating on Dacron® fabric might improve its histocompatibility while leaving its desirable mechanical properties unaffected. To test this idea, Dacron® velour specimens were coated with titanium by vacuum deposition and were implanted together with uncoated controls in rabbits subcutis. After various implantation times, the specimens were recovered and the associated tissue was examined histologically. Qualitative and semiquantitative analysis revealed that tissue ingrowth quantity and quality was highly variable, not only between test and control specimens, but also between animals and even between specimens within the same animal. This indicated that there may be a number of factors influencing tissue ingrowth that were not adequately controlled in this study. The titanium coating which was undoubtedly highly oxidized had a profound qualitative and quantitative effect on fibroblast activity (ground substance formation) and fibroblast adhesion to the Dacron® fibers. The results obtained after these short-term implantation periods indicate that titanium coating affects the quality of the interfacing tissue and may actually improve long-term histocompatibility. Long-term studies will have to confirm these preliminary data.

    Notes: A permanently implantable in-series left ventricular assist device, the dynamic aortic patch (DAP), has been tested in chronic animal experiments. The DAP replaces a section of the intrathoracic aortic wall. Hemothorax and hematocele at the implantation site have been complications in recent experiments. Primary postoperative hemorrhage was ruled out, and the biocompatibility of all components was therefore examined. Dacron velour, Teflon felt, conductive polyurethane, segmented polyether polyurethane, and Teflon-coated polyester fiber sutures were implanted in the pleural cavities of dogs and tested in vitro by culturing canine saphenous vein explants on them. In vivo experiments demonstrated that all components elicited mild to moderate inflammatory reactions, but hematocele occurred only when the components were implanted in the aorta with direct blood contact and exposed to arterial blood pressures. In vitro, cells were cultured on all components with no signs of toxic reactions. These results indicated that the host tolerated all implant components without major inflammatory responses. However, histological data indicated that chronic slow bleeding into or through the Dacron velour in contact with the arterial blood serum could account for hemothorax or hematocele formation. Therefore, a configuration of the assist device using materials impermeable to blood may obviate these difficulties.

    Notes: This study examined the hypothesis that healing within porous implants differs from that in normal connective tissue. Special attention was given to extracellular components including collagen, reticular fibers, and ground substance, and to enzymes associated with activated macrophages. Using Dacron velour and the rabbit as host, the healing of normal connective tissue and that of the tissue/implant interface were histologically compared 10 and 28 days postimplantation. The results exhibited significant differences between connective tissue healing, implant capsule formation, and granulation tissue generation. The healing of connective tissue and implant capsule formation were essentially complete at 28 days. However, tissue inside the implant was qualitatively different and did not significantly change between 10 and 28 days. It was characterized by macrophages and giant cells, a predominantly acid mucopolysaccharide ground substance, and qualitatively fewer and less well defined collagen and reticular fibers were observed than in normal wound healing. Thus we conclude that the connective tissue inside Dacron velour does not resemble normal connective tissue after 10 or 28 days of healing. Furthermore, the collagen never fully matures into orderly bundles, a phenomenon which may be related to an altered mucopolysaccharide composition and a diminished reticular network. The lysosomal enzymatic activity of the macrophages and perhaps the giant cells at the tissue/implant interface may be linked to these differences.

    Notes: To determine whether progressive infection along percutaneous conduits (PC) usually leads to infection of cardiac assist devices, two PCs each were implanted into the subcutis of 11 dogs. One month later, one end of one conduit was inoculated with either E. coli or S. aureus. The resulting infections in all dogs progressed little during the first two weeks and in 10 of the 11 animals involved less than 40% of the total length of the PC. Bacteriological tests one month after the infecting organisms were introduced showed that they were no longer present at the site of inoculation in 4 of the 11 dogs and no bacteria were found at the opposite end of the inoculated PCs or in the noninoculated PCs. Histological observations confirmed these results and indicated that in all animals encapsulation of the infected area prevented its further enlargement.

    Notes: A method is described which is proposed to prolong the functional life of a Silastic finger joint endoprosthesis which has been shown to fail because of abrasion and cutting by the recipient bones. Swanson design Silastic finger joints prostheses were implanted into the knee joints of rabbits. The technique of simultaneously transplanting ear cartilage to the prosthetic implant bed was tested for the purpose of protecting the prostheses from wear and eventual failure. The clinical, radiographic, gross, and histologic results of the animals implanted for 1, 3, and 6 months show that the cartilage graft remains viable and it proliferates. The cartilage provides a smooth, gliding surface with normal appearing joint fluid surrounding the Silastic implant. Control limbs, with no cartilage grafts, had thinner, fibrous tissue encapsulating the implants. The presence of the cartilage reduced the amount of prosthetic wear particles observed in the tissue sections, thus indicating a protective function. The rabbit model developed an osteoarthritis that was caused by the prostheses in its knee joints and was unaffected by our cartilage grafting procedure. The success of our cartilage grafts, in the presence of such an arthritis, provides the impetus for its clinical use in arthritic human finger joints. Further studies are necessary to determine the clinical feasibility of a cartilage grafting technique and its efficacy in reducing implant failure in human finger joints.

    Notes: Percutaneous healing is plagued by materials compatibility problems, epidermal reactions, and mechanical factors. To our knowledge, no studies have evaluated the effects of one of these factors while controlling the others. The tympanic membrane is an implant site where mechanical factors are minimized. Titanium has been shown to be a very histocompatible material, although no reports of the histology of percutaneous titanium implants exist. This study reports on aspects of the epidermal and connective tissue healing around titanium tympanic membrane implants that were explanted from human subjects. The lack of inflammation and the direct adherence of the tissues substantiate the materials compatibility and the absence of mechanical factors. Tissue healing patterns appear to reach an equilibrium which is suitable for the long-term exixtence of the percutaneous device. This implant location provides a suitable model for studying the tissue reactions to various percutaneous materials.
