    Notes: Abstract. This paper investigates the parallel time and processor complexities of several searching problems involving Monge, staircase-Monge, and Monge-composite arrays. We present array-searching algorithms for concurrent-read-exclusive-write (CREW) PRAMs, hypercubes, and several hypercubic networks. All these algorithms run in near-optimal time, and their processor-time products are all within an $O (\lg n)$ factor of the worst-case sequential bounds. Several applications of these algorithms are also given. Two applications improve previous results substantially, and the others provide novel parallel algorithms for problems not previously considered.

    Notes: Two cases of formalin toxicity developed within 5 to 10 minutes of the placement of packs soaked in formalin within hepatic hydatid cysts. Hypotension, severe tachycardia, postoperative anuria, tachypnoea, restlessness, unconsciousness and coma developed. There was no response to the treatment given. Treatment was continued for about 12 hours. The problems associated with the use of formalin are discussed.

    Notes: Summary.  We previously reported durable complete responses following brief courses of rituximab and prednisone with or without cyclophosphamide in four patients with autoimmune haemophilia and inhibitor titres of 5–60 BU. We report here responses to this monoclonal anti-CD20 antibody in four additional patients, including two patients with inhibitor titres 〉200 BU. Factor VIII levels became normal 2–35weeks after 4 or 8 weekly doses of rituximab, brief courses of prednisone and in one patient immunoglobulin. Complete responses are ongoing at 10 months in two patients. Two patients relapsed: a patient whose initial inhibitor titre was 525 BU relapsed at 3.5 months and a long-term prednisone-dependent patient at 8.5 months. Both responded to second courses of rituximab and prednisone and are in remission. Our experience suggests that rituximab is a safe and effective addition to immunosuppression with prednisone and cyclophosphamide to treat autoimmune haemophilia, and may permit early discontinuation or even avoidance of these potentially toxic agents. High-titre inhibitor patients, however, may require multiple courses of rituximab or the addition of cyclophosphamide. Pending randomized studies, we propose an algorithm based on our experience and other reports for incorporating rituximab in the treatment of this rare disorder.

    Notes: This paper presents network models to determine the optimal replacement policy for a fleet of vehicles over a finite planning horizon. First, a minimum cost-flow model is developed to determine the optimal replacement schedule for a fleet of fixed size consisting of a single type of vehicle of various ages. The model is then extended to allow for restrictions on capital expenditures that limit the purchase of new vehicles in any time period and to allow for fluctuations in the fleet size due to planned expansion or retrenchment. Finally, a multi-commodity network model is developed for a fleet consisting of multiple vehicle types and ages.

    Notes: The aim of this study was to assess a simple method of measuring relaxation rate in the jaw-closing system for the purpose of quantifying jaw muscle fatigue. A summary of the various different methods of measuring relaxation rate is also provided. The rates of twitch contraction and relaxation were measured in 30 symptom-free subjects following bilateral direct electrical stimulation of the masseter muscles. The resulting twitch force was recorded via a force transducer placed between the anterior teeth. The transducer was held between the teeth with as little force as possible while four single stimuli were delivered at 5-s intervals. The stimulating electrodes were then removed and replaced and the experiment was repeated. The force records of the resulting twitches were averaged and the half-contraction time, twitch amplitude and half-relaxation time were measured. There was a significant difference in half-relaxation time between males and females, being faster in females (P=0·0045, independent t-test). No significant difference was found in twitch amplitude and half-contraction time between males and females. Half-relaxation time and half-contraction time were independent of twitch amplitude. This method of measuring the relaxation rate of the masseter muscles was found to be practical and the results were reproducible between sessions.

    Notes: The current–voltage characteristics of logarithmic amplifiers depend considerably on the ambient temperature. A theoretical correction has been estimated considering the temperature dependence of various parameters in the diode equation. A log electrometer using light-emitting diodes has been fabricated and its I–V characteristics are obtained experimentally for calibration and also to determine the device constant. If a measurement is obtained at a temperature other than the one for which calibration has been done, the theoretical correction can be applied to get the correct value. It has been shown that the theoretically corrected readings lie within ±4% of the experimentally obtained data in the temperature range of 5–60 °C for the operating current range of 10−12–10−4 A. The alternative can be to introduce temperature compensation circuitry into the logarithmic amplifier. However, this makes the electrometer circuit more complex. The proposed theoretical correction method may find applications where space and power are at a premium and the accuracy requirement is not very stringent. © 1996 American Institute of Physics.

    Notes: An unusual giant combined dermatofibroma is reported in a 34-year-old man who presented with skin-coloured swellings on the medial aspect of the left scapula. The plaque was well defined, reddish-brown and 25–30 cm in diameter. It was also tender and indurated. Several similar smaller lesions (satellites) were present around the plaque. Light microscopy of an incisional biopsy from the main lesion showed architectural features of a deep penetrating type of dermatofibroma, with xanthomatous aggregates, myxoid changes and probable myofibroblastic differentiation. Our case represents an extraordinary example of giant combined dermatofibroma with satellitosis. Despite its benign nature, a wide excision is contemplated because of its unsightly appearance and physical discomfort.
