    Notes: Abstract Twelve infants operated upon for choledochal cyst (CC) are reviewed with emphasis on the operative technique of biliary tract reconstruction, incidence of cholangitis, postoperative hypergastrinemia, biliary excretion, and upper gastrointestinal (GI) motility in a follow-up of 24 to 35 months. In 7 patients biliary reconstruction was performed with a Roux-en-Y hepaticojejunostomy (HJ), and in 5 with a hepaticoantrostomy (HAST). In the HJ group 4 patients had recurrent episodes of cholangitis and intermittent diarrhea and serum gastrin levels were significantly elevated in 5. Hepatobiliary scintigraphy showed unobstructed excretion of labelled bile through bile ducts into the Roux-en-Y loop, but with significantly delayed emptying of bile into the distal jejunum in all patients. Gastric emptying and upper intestinal passage were normal. In the HAST group no episode of cholangitis occurred and serum gastrin levels were within the normal range. Scintigraphically, hepatobiliary excretion, and duodenojejunal passage of labelled bile was normal, except in 1 patient who developed a postoperative stenosis of the left hepatic duct. Upper GI contrast studies demonstrated normal gastric emptying without reflux into the biliary system. These results suggest that biliary reconstruction with HAST can be performed safely with a low incidence of complications HAST offers a more physiologic method of biliary reconstruction after resection of a CC that allows bile to drain directly into the duodenum.

    Description / Table of Contents: Zusammenfassung 90 Rattenmägen von 3 Gruppen (Kontrolle, SPV, SPV + Pypl.) wurden nach 30 Wochen Carcinogenexposition röntgenologisch, makroskopisch und histologisch untersucht. Die Kontrollgruppe und die Gruppe SPV + Pypl. waren ohne pathologischen Befund. Die Gruppe SPV wies in 89% Stasemdgen auf, in 39% Staseulcera, bei 28% wurden im Bereich dieser Staseulcera adenomatöse Hyperplasien mit Zelldysplasien nachgewiesen. Eine SPV ohne Pyloroplastik führt bei der Ratte zu Retention. Staseulcera und Dysplasien lassen sich durch die Kombination der SPV mit einer Pyloroplastik vermeiden.
    Notes: Summary The stomachs of 90 rats in three groups (control, SPV, SPV + Pypl.) were x-rayed and submitted to macroscopical and histological examination 30 weeks after administration of the carcinogen was started. No macroscopical and histological alteratons were observed in the control group and in the group SPV + Pypl. Of the SPV group without pyloroplasty, 89 % showed stasis stomachs, 39% had stasis ulcers, and 28% had stasis ulcers with adenomatous hyperplasia and cell dysplasia; SPV without pyloroplasty on rat leads to retention. Stasis ulcers and dysplasia can be avoided by combining SPV with pyloroplasty.

    Notes: Summary The 30 and 60 day course of primary and secondary bone healing following partial and total osteotomy has been studied in the rat. Radiographs, scintigrams, callus thickness and scintillation counting were analyzed. Primary healing is completed by 30 days and secondary healing by 60 days. By use of this model, the effect of various influences on fracture healing can be quantitatively and qualitatively determined. Der physiologische Ablauf der primären und der sekundären Knochenheilung nach Teilbzw. Totalosteotomie wurde über 30 bzw. 60 Tage an der Ratte untersucht. Als Befunde wurden synchron Röntgenaufnahmen, Szintigramme, die Callusdicke und die Bohrlochkristallmessung dokumentiert. Die primäre Heilung ist nach 30 Tagen, die sekundäre Heilung nach 60 Tagen abgeschlossen. Mit diesem Modell können Einflüsse auf die Knochenbruchheilung quantitativ erfaßt und qualitativ dokumentiert werden.

    Description / Table of Contents: Zusammenfassung Die Serumaktivität der alkalischen Phosphatase und die Konzentration von Calcium und Phosphor im Frakturheilungsverlauf und unter Calcitoninbehandlung wurden bestimmt. Bei 160 Albinoratten wurde die rechte Tibia osteotomiert, 80 davon wurden mit Calcitonin behandelt. Der Calciumspiegel unterschied sich während des 50tägigen Beobachtungszeitraums zwischen calcitoninbehandelten und Kontrolltieren nie signifikant. Dagegen unterscheidet sich an 4 Meßtagen die Aktivität der alkalischen Phosphatase und der Serumphosphatspiegel signifikant (P 〈 0,05). Eine höhere Aktivität der alkalischen Phosphatase als Ausdruck der Osteoblastentätigkeit weist auf die unterstützende Wirkung von Calcitonin bei der Knochenneubildung und der Frakturheilung hin.
    Notes: Summary Serum alkaline phosphatase activity, serum calcium and phosphorus concentrations were determined during bone healing under treatment of thyrocalcitonin. The right tibiae of 160 albino rats were osteotomized. Of these, 80 were treated with thyrocalcitonin. During the 50-day observation period, the calcium level in the thyrocalcitonin-treated sample never differed significantly from that of the control group. However, the alkaline phosphatase activity and the serum phosphorus level significantly differed from the control levels (P 〈 0.05) during 4 days of observation. The increased serum alkaline phosphatase activity — a sign of osteoblastic activity — suggests that thyrocalcitonin may aid in osteogenesis and bone healing.

Dazu wurde die rechte Tibia osteotomiert, die Hälfte der Tiere wurde mit Calcitonin behandelt. Bis zum 60. Tag postoperativ wurde an den isolierten Tibiae die Aktivitätsanreicherung von99mTC-Methylendiphosphonat gemessen und das Impulsratenverhältnis auf signifikante Unterschiede untersucht. An den Tagen 10 bis 25 postoperativ unterschieden sich die Impulsratenquotienten der mit Calcitonin behandelten von denen der Kontrolltiere signifikant (P 〈 0.05). Die Ergebnisse legen die Vermutung nahe, daß Calcitonin die sekundäre Frakturheilung zu unterstützen vermag.
    Notes: Summary The effect of thyrocalcitonin on secondary bone healing was studied in 180 Sprague-Dawley-rats. The right tibia of all the rats was osteotomized. Half of the animals was treated with thyrocalcitonin. Until the 60th postoperative day, the increase in99mTc-Methylendiphosphonate activity was measured and significant differences in ratios were noted. Between the postoperative days 10 to 25, there was a significant difference in the impulse quotients between the thyrocalcitonin and untreated rats (P 〈 0.05). These results indicate that thyrocalcitonin may aid in secondary bone healing.

    Notes: Abstract  Acute myelofibrosis is a rare, malignant hematological disorder of unknown etiology with an inevitably fatal outcome. Here we present the study of a 63-year-old Caucasian man with acute onset of pancytopenia. Repeated bone marrow biopsies showed dense fibrosis and hypoplastic hematopoiesis raising various differential diagnoses of malignant and nonmalignant conditions. Bone marrow scintigraphy and magnetic resonance imaging (MRI) showed areas suggesting neoplastic infiltration, mainly in both femurs and tibias. Histological examination of a surgical biopsy of the left tibia revealed acute megakaryoblastic leukemia. As the patient refused polychemotherapy, therapy with interferon gamma was initiated but discontinued prematurely because of intolerable side effects. The presented case therefore suggests that the combination of bone marrow scintigraphy and MRI is a valuable diagnostic tool in patients presenting with myelofibrosis of unknown origin.
