Summary The angiotensin-infusion-test according to the method ofKaplan andSilah was performed in 172 persons. A pressor dose (PD) ofm=7.85 mµg per kg body weight was found in normotensive persons, while in patients with essential and chronic renal hypertension the PD was 4.19 and in patients with renovascular hypertension 11.71. Significant differences were seen not only between patients with essential and chronic renal hypertension on the one hand and renovascular and malignant hypertension on the other hand, but also in comparison to the normotensive persons as long as patients with labile hypertension were excluded. In the patients with labile hypertension the PD for the several groups was between 7.35 and 7.98 with a larger standard deviation than in normotensive persons. Based on these results the angiotensin-infusion-test is regarded to be a valuable screening test for the separation of renovascular and malignant from essential and chronic renal hypertension. Concerning the prognostic value of the test it only can be stated that a preoperative low PD makes unlikely a postoperative normalization of blood pressure. The results are briefly discussed with respect to the pathophysiology of the different forms of hypertension.

Zusammenfassung Mit dem Angiotensin-Infusionstest nachKaplan undSilah [16–19] wurden 172 Personen untersucht. Es ergab sich für Normotoniker eine mittlere Pressordosis von 7,85 mµg/kg Körpergewicht und Minute, für Kranke mit fixierter essentieller und chronisch-renaler Hypertonie von 4,19, für Kranke mit fixiertem renovasculärem Hochdruck von 11,77. Für Kranke mit fixierter essentieller und fixierter chronisch-renaler Hypertonie einerseits und mit renovasculärem und malignem Hochdruck andererseits ergaben sich signifikante Unterschiede, nicht nur untereinander, sondern auch zu den Normotonikern, sofern die Patienten mit labilem Hypertonus nicht mitberücksichtigt wurden. Für Kranke mit labilem Hypertonus fanden sich mittlere Pressordosen zwischen 7,35 und 7,98 bei erheblich größerer Streubreite als bei den Normotonikern. Aufgrund der Befunde erscheint der Angiotensin-Infusionstest als Suchtest geeignet, einen Hochdruck renovasculärer Genese oder maligner Phase von fixiertem-essentiellen und fixiertem chronisch-renalen Hochdruck zu unterscheiden. Hinsichtlich einer Prognose zum postoperativen Blutdruckverhalten ist nur die Aussage möglich, daß eine verminderte Pressordosis eine völlige Blutdrucknormalisierung unwahrscheinlich erscheinen läßt. Abschließend wird die unterschiedliche Angiotensin-Empfindlichkeit im Hinblick auf die Pathophysiologie verschiedener Hochdruckformen diskutiert.
