    Notes: Abstract  • Background: Severe decentration and subluxation of intraocular lenses (IOLs) may lead to double vision, glare and deterioration of vision to the point of functional aphakia. The purpose of the present study was to analyse causes for severe IOL dislocation. • Material and methods: Between January 1989 and January 1996, 37 patients required IOL exchange because of decentrated or subluxated posterior chamber lenses. Twenty-five of the exchanged lenses were implanted in our hospital, 12 lenses elsewhere. After explantation the lenses were examined by light and electron microscopy. • Results: In 10 eyes, asymmetric implantation of the posterior chamber lens was responsible for decentration. Three of the lenser concerned were multifocal IOLs. Asymmetric implantation led to a significantly higher rate of explantations in eyes with multifocal lenses (P〈0.005). In five eyes decentration developed due to asymmetric capsular shrinkage, in four eyes due to posterior synechiae. A lens subluxation developed in three eyes as a result of rupture of the posterior capsule and in nine eyes because of zonular defects. In three cases decentrations were induced by an extensive secondary cataract. Macroscopically visible changed geometry of the haptics was found in nine lenses; eight of these had polypropylene haptics. Seven lenses showed severely altered haptics on electron-microscopic examination. In four eyes subluxated lenses had to be explanted together with the capsular bag because of severe defects of the zonula, which caused decentration in nine eyes. • Conclusions: Asymmetric implantation of posterior chamber lenses should be strictly avoided. Multifocal lenses require special attention concerning symmetric capsulorhexis and positioning of their haptics.

    Description / Table of Contents: Hintergrund: Dakryozystographie und Dakryoszintigraphie sind etablierte bildgebende Verfahren in der Diagnostik von Tränenwegstenosen. Beide erlauben nur indirekte Aussagen zur Morphologie des tränenableitenden Systems. Es wird untersucht, inwiefern mit Hilfe der Magnetresonanztomographie (MRT) neben der direkten detaillierten Darstellung der Tränenwege auch funktionelle Aussagen getroffen werden können, um die Rolle der Methode im Spektrum der bildgebenden Verfahren bei Tränenwegerkrankungen zu definieren. Patienten und Methoden: Es wurden 27 Tränenwegsysteme von 23 Patienten mit Epiphora in einem Ganzkörpermagnetresonanztomographen mit einer Oberflächenspule vor und nach konjunktivaler und intravenöser Gabe von Gd-DTPA untersucht. Ergebnisse: Passagestörungen wurden in 23 von 27 Tränenwegsystemen gefunden, konnten den Canaliculi lacrimales (n = 3), dem Saccus lacrimalis (n = 8) und Ductus nasolacrimalis (n = 12) zugeordnet, in Stenosen und Verschlüsse unterteilt werden und teilweise ätiologisch charakterisiert werden. Schlußfolgerungen: Die hochauflösende MRT mit konjunktivaler Kontrastmittelapplikation erlaubt neben der direkten und detaillierten Darstellung der ableitenden Tränenwege und periduktaler Strukturen zugleich eine funktionelle Beurteilung des Tränenwegsystems. Limitierungen einer verbreiteten Anwendung resultieren u. a. aus Anforderungen an gerätetechnische und patientengebundene Voraussetzungen sowie aus Kostengründen.
    Notes: Summary Both dacryocystography and dacryoscintigraphy are well established in the evaluation of stenoses of the lacrimal drainage system. They provide limited information about the ductal anatomy itself and about periductal structures. MR imaging was evaluated for its capability to directly visualize the lacrimal drainage system in detail and simultaneously provide functional characterization of dacryostenosis. Subjects and methods: Twenty-seven lacrimal drainage systems of 23 patients suffering from epiphora were examined in an MR unit before and after conjunctival and intravenous application of Gd-DTPA using a surface coil. Results: Dacryostenosis was found in 23 of 27 lacrimal systems. Stenoses were localized to the canalicular (n = 3), saccular (n = 8), and ductal (n = 12) level, and were classified as stenosis or occlusion. Conclusion: MR imaging with conjunctival contrast application allows within one examination both detailed morphological and functional assessment of the lacrimal drainage system with depiction of surrounding structures. Limitations arise mainly from demands on technical and patient-related preconditions.

    Description / Table of Contents: Hintergrund: Die Dakryozystorhinostomie stellt noch immer das Standardverfahren bei absoluter Tränenwegsstenose dar. In letzter Zeit wurden Methoden entwickelt, die es ermöglichen, unter Beibehaltung der natürlichen Abflußwege eine Durchgängigkeit zu erzielen. Die auf der Seldingertechnik beruhende Methode der retrograden Implantation eines durchgängigen Polyurethan-Röhrchens in den Ductus nasolacrimalis ist eine vielversprechende Alternative. Patienten und Methoden: Es wurden 30 Patienten mit einer radiologisch gesicherten Tränenwegsstenose therapiert, darunter 15 Patienten mit absoluter und 2 mit relativer postsakkaler Tränenwegsstenose. Das Alter betrug 22–87 Jahre (Mittelwert 58,9 ± 16 Jahre). Der Polyurethan-Stent wurde unter Durchleuchtungskontrolle implantiert. Seine Durchgängigkeit wurde direkt postoperativ, nach 4 Wochen und nach 8 Monaten mittels Dakryozystographie kontrolliert. Ergebnisse: Insgesamt wurden 25 kurze (35 mm) und 5 lange (45 mm) Stents implantiert. Letztere reichten bis in den Canaliculus superior. Bei 24 von 30 Patienten konnte auch nach 4 Wochen und bei 9 von 10 Patienten nach 8 Monaten Durchgängigkeit des Stents klinisch nachgewiesen werden. Bei einer Patientin mußte der Stent unter Durchleuchtungskontrolle wieder rekanalisiert werden. Bei einem weiteren Patienten kam es zu einer Verlagerung eines kurzen Stents in den oberen Kanalikulus, so daß er nach 2 Monaten gezogen werden mußte. Postoperative Dakryozystitiden traten bei keinem der Patienten auf. Schlußfolgerung: Erfolgs- und Komplikationsraten lassen den Polyurethan-Stent als eine echte Alternative zur Toti-Operation erscheinen, wenngleich die Nachkontrollzeiten noch zu kurz für eine abschließende Beurteilung sind. Die Vorteile liegen im geringeren Zeitaufwand, der fehlenden Hautinzision und der problemloseren Anästhesie, die Nachteile in der bisherigen Notwendigkeit einer Durchleuchtungseinheit.
    Notes: Summary Dacryocystorhinostomy is still the standard procedure complete stenosis of the nasolacrimal duct. New methods try to preserve the natural lacrimal pathway. Song implanted in 1995 a nasolacrimal polyurethane stent through the nasolacrimal duct. The results and complications of this new method are described in this prospective study. Methods: Thirty consecutive patients with complete obstruction of the nasolacrimal duct or lacrimal sac were included in the study. The stenosis was localized by dacryocystography. The ages ranged from 22 to 87 years (mean, 58.9 ± 16 years). Dacryocystography was performed immediately, 4 weeks and 8 months after the procedure to verify the position and patency of the stent. Results: Twenty-five short (35 mm) and 5 long (45 mm) stents were implanted. Twenty-four of 30 patients after 4 weeks and 9 of 10 patients after 8 months had reduced or no complaints. In 1 patient the stent was obstructed. Forceful irrigation with saline solution permitted recanalization. In 1 patient the stent had moved into the upper canaliculus. Because of irritation of the canaliculus it had to be pulled out after 2 months. Conclusion: The follow-up is still too short to recommend stent implantation as a real alternative to dacryocystorhinostomy. The main advantages are that the procedure is faster, no incision is necessary, and the local anesthesia is easier. The disadvantage is the need for X-ray examination.

    Description / Table of Contents: Hintergrund: Während geringe bis mäßige Astigmatismuskorrekturen heute bereits photoablativ mit gutem Erfolg durchgeführt werden können, erfordert die Korrektur mittlerer und höherer Astigmatismen nach wie vor den Einsatz refraktiver Keratotomien. Bei Verwendung heute üblicher Tunneltechniken mit selbstschließenden Inzisionen kann durch die lamellierende Präparation sowohl eine frühere postoperative Stabilisierung der Refraktion, als auch eine hohe mechanische Stabilität erreicht werden. Wir versuchten, die Vorteile dieser lamellierenden Technik mit der refraktiven Wirkung einer paarigen T-Inzision in der bogenförmigen lamellierenden Keratotomie (BLK) zu kombinieren. Patienten und Methode: Wir untersuchten prospektiv die klinischen Langzeitergebnisse (prä- und postoperativ: mit Zeiss-Ophthalmometer gemessener Astigmatismus, sc-Visus sowie cc-Blendungsvisus ‚Humphrey’-Autorefraktor) nach BLK von 41 Patienten bei durchschnittlich 3-jähriger Nachbeobachtungszeit. Alle Patienten wiesen vor BLK einen Astigmatismus von mind. 2,0 bis max. 7,0 dpt auf. Wir verwendeten bei der Korrektur 7- (n = 26) und 8- (n = 15) mm-Zonen, sowie eine einheitliche Bogenlänge von 3 mm. Alle Inzisionen wurden paarweise durchgeführt. Ergebnisse: Der durchschnittliche, präoperative mit dem Zeiss-Ophthalmometer gemessene Astigmatismus betrug 4,01 ± 1,90 (Median 3,50) dpt. Nach einer Nachbeobachtungszeit von durchschnittlich 3 Jahren betrug der postoperative Astigmatismus im Mittel 1,59 ± 1,29 (Median 1,38) dpt. Die induzierte vektoranalytische Astigmatismusänderung betrug nach 3 Jahren 3,23 ± 2,23 (Median 3,16) dpt. Der ohne Korrektur erzielte Visus (log MAR) betrug präoperativ 0,20 ± 0,12 (Median 0,22) und nach durchschnittlich 3 Jahren 0,41 ± 0,14 (Median 0,39) dpt. Der Blendvisus betrug präoperativ mit Korrektur 0,23 ± 0,19 (Median 0,10) dpt und postoperativ 0,25 ± 0,22 (Median 0,14) dpt. Schlußfolgerungen: Die bogenförmige lamellierende Keratotomie (BLK) hat sich bei mäßiggradigen Astigmatismen auch in den Langzeitergebnissen als refraktives Verfahren mit stabilen postoperativen Refraktionsergebnissen bewährt. Eine Reduktion des Blendvisus konnte nicht festgestellt werden.
    Notes: Introduction: The correction of low to moderate astigmatism is possible today by means of photoablation (PRK), while the treatment of moderate and higher astigmatism still involves refractive keratotomy. Experience has shown that cataract surgery, using modern tunnel techniques with self-healing incisions, results in earlier stability in both the refractive outcome and wound healing. In this study, we attempted to combine the advantages of lamellar keratotomy with those of a pair-wise T-incision as arcuate lamellar keratotomy (ALK). Patients and methods: The clinical outcome of 41 patients who underwent ALK was investigate in a prospective study over a period of 3 years. The pre- and postoperative investigations undertaken included the measurement of astigmatism using a Zeiss keratometer, uncorrected visual acuity, and corrected glare vision using a Humphrey autorefractor. All patients had astigmatism between 2.0 and 7.0 D. Together with a uniform arcuate incision, we used 7 mm (n = 26) and 8 mm (n = 15) mm zones for correction. Results: The average preoperative astigmatism was 4.01 ± 1.90 (median, 3.50) D. After a 3-year follow-up the average postoperative astigmatism was 1.59 ± 1.29 (median, 1.38) D. The astigmatic change induced (Jaffé) after 3 years was about 3.23 ± 2.23 (median, 3.16) D. The average uncorrected visual acuity (log MAR) before ALK was 0.20 ± 0.12 (median, 0.22) and after follow-up, 0.41 ± 0.14 (median, 0.39). Corrected glare vision before surgery was 0.23 ± 0.19 (median, 0.10) and afterwards, 0.25 ± 0.22 (median, 0.14). Conclusions: Arcuate lamellar keratomy (ALK) stood the test as a routine clinical procedure for correction of moderate astigmatism with stable postoperative functional outcomes. We did not find impairment of glare vision following this procedure.

    Description / Table of Contents: As the lateral incision in comparison to the classic incision at the 12 o'clock position induces less astigmatism and shows higher wound stability, we wanted to determine if this technique could be used as a routine procedure for most patients. Patients and methods: A total of 186 patients were prospectively included in this study. They all had a lateral incision with the no-stitch technique, either as a clear corneal incision or as a corneoscleral or scleral incision. Postoperatively, patients were followed up for up to 12 months. Results: Whereas the scleral incision showed the highest wound strength, one patient with a corneoscleral incision needed a later suture. There were two cases of endophthalmitis after a clear corneal incision; 6 – 12 months postoperatively the mean induced astigmatism amounted 0.64±0.22 D after a scleral incision, 0.71±0.47 D after a corneoscleral incision, and 0.92±0.63 D after a clear corneal incision. Conclusion: The lateral approach with a scleral is a safe procedure and induces very little astigmatism. It can be used routinely for all patients who have inverse preoperative astigmatism or none at all. The clear corneal incision shows instable wound closure and a higher infection risk. In the long term it induces an astigmatism of about 1 D and therefore is of no use for correction of higher inverse astigmatism.
    Notes: Da der laterale Zugang einen geringeren Astigmatismus induziert und im Vergleich zum klassischen Zugang bei 12 Uhr auch eine höhere Wundstabilität hat, stellt sich die Frage, ob diese Technik als Routineeingriff eingesetzt werden kann. Patienten und Methode: Prospektiv wurden 186 Patienten in eine Studie aufgenommen, bei denen der laterale Zugang entweder als rein kornealer, korneoskleraler oder skleraler Tunnelschnitt ohne Naht im Sinn der No-stitch-Technik durchgeführt wurde. Die Kontrolluntersuchungen erfolgten bis 12 Monate postoperativ. Ergebnisse: Während der sklerale Tunnelschnitt die stabilsten Wundverschlüsse zeigte, wurde in 1 Fall mit korneoskleralem Schnitt eine Fadennachlegung notwendig. In 2 Fällen mit kornealem Schnitt kam es zu einer Endophthalmitis. Der induzierte Astigmatismus lag im 6. bzw. 12. Monat postoperativ beim skleralen Schnitt durchschnittlich bei 0,64±0,22 dpt, beim korneoskleralen bei 0,71±0,47 dpt und beim kornealen bei 0,92±0,63 dpt und damit am höchsten. Schlußfolgerung: Der laterale Zugang durch einen skleralen Tunnelschnitt ist zuverlässig, induziert einen geringen Astigmatismus und kann deshalb routinemäßig bei allen Patienten angewendet werden, die präoperativ keinen oder einen geringen inversen Astigmatismus haben. Der korneale Schnitt hat einen unsicheren Wundverschluß und ein erhöhtes Infektionsrisiko. Der korneale Tunnelschnitt induziert darüber hinaus langfristig einen Astigmatismus um 1 dpt und ist infolgedessen für die Korrektur eines höheren Astigmatismus gegen die Regel nicht geeignet.

    Description / Table of Contents: Hintergrund: Seit Januar 1992 wird an der Augenklinik des Virchow-Klinikums die geplante extrakapsuläre Kataraktextraktion (ECCE) routinemäßig mit selbstschließender 11-mm-Tunnelinzision durchgeführt. Der Anteil der geplanten ECCE beträgt 10% der Kataraktoperationen. Im Gegensatz zur Phakoemulsifikation mit max. 7 mm Tunnelbreite ist bei der ECCE zur Kernausleitung eine Tunnelbreite von 11 mm erforderlich. Patienten und Methode: 250 Patienten wurden 4 Gruppen mit unterschiedlichen Wundverschlüssen zugeordnet und prospektiv bezüglich des induzierten Astigmatismus und der aufgetretenen Komplikationen 1–3 Jahre postoperativ nachkontrolliert. Ergebnisse: Die Komplikationsrate lag bei 4% (Sickerkissen, Irisprolaps, passagere Hypotonie). Eine Starschnittruptur wurde nicht beobachtet, eine Endophthalmitis trat 1mal ein. Der spätpostoperative mittlere Astigmatismus nach 1 – 3 Jahren betrug nach Inzision bei 12 Uhr ohne Naht 2,05±1,16 dpt (präoperativ 1,01±0,96 dpt, n = 70), nach radiärer Einzelnaht 1,63±1,08 dpt (0,86±0,95 dpt, n = 100), nach Kreuzstichnaht 1,76±0,88 dpt (0,73±0,55 dpt, n = 40) und nach nahtlosem Wundverschluß in temporaler Position 0,78±0,52 dpt (1,0±0,69 dpt, n = 40). Die Ergebnisse wurden auch vektoranalytisch berechnet. Schlußfolgerung: Der Astigmatismus ist frühpostoperativ stabil. Durch den nahtlosen Wundverschluß in 12-Uhr-Position wird unabhängig von der präoperativen Achsenlage der höchste, durch die radiäre Einzelnaht oder Kreuzstichnaht ein signifikant geringerer Astigmatismus induziert. Bei präoperativ bestehendem Astigmatismus gegen die Regel besonders über 1,5 dpt sollte von temporal operiert werden.
    Notes: Since January 1992 planned extracapsular cataract extraction (ECCE) is performed routinely with the no-stitch technique at our clinic. To minimize surgically induced astigmatism further, modified wound constructions for planned ECCE with on 11-mm tunnel width were evaluated. The follow-up time was up to 3 years postoperatively. For 250 eyes wound closure was performed prospectively either sutureless (n = 70), with a single perpendicular suture (n = 100) or cross sutures (n = 40) at the 12 o'clock position or sutureless in the temporal position (n = 40). The complication rate was 4% (filtering bleb, iris prolapse or transient hypotonia). There were no wound ruptures, but once endophthalmitis was observed. Late mean astigmatism after up to 3 years follow-up for vertical incision was 2.05±1.16 D (1.01±0.96 D preoperatively) for sutureless wound closure, 1.63±1.08 D (0.86±0.95 D) for perpendicular and 1.76±0.88 D (0.73±0.55 D) for cross-sutures. A temporal incision resulted in 0.78±0.52 D (1.0±0.69 D) of astigmatism and was only performed on eyes with against the rule astigmatism preoperatively. Surgically in-duced astigmatism was stabilized early. For with the rule astigmatism preoperatively, a 12 o'clock incision with a perpendicular single suture is recommended and for against the rule astigmatism, a temporal incision.

    Description / Table of Contents: Einleitung: Bei Pseudoexfoliationssyn-drom (PEX) kann es zu morphometrischen und qualitativen Veränderungen des Hornhautendothels kommen. In dieser Untersuchung sollte überprüft werden, ob nach Kataraktoperation das Endothelzellverhalten gegenüber normal-gesunden Augen verändert ist. Patienten und Methoden: In einer kontrollierten klinischen Studie wurden prospektiv 20 Patienten mit PEX und ein gleich großes alterskorreliertes Vergleichskollektiv mit standardisiertem, selbstschließendem, 7 mm breitem korneoskleralem Tunnelschnitt, Phakoemulsifikation und HKL-Implantation unter Hyaluronsäureschutz operiert. Neben einem vollständigem ophthalmologischen Status wurden präoperativ, am 1. postoperativen Tag sowie nach 4 Wochen quantitative und qualitative Endotheluntersuchungen der zentralen und peripheren Hornhaut mit der Non-Kontaktspekularmikroskopie (Konan Noncon Robo-ca SP 8000, Konan, Japan) durchgeführt. Ergebnisse: Präoperativ zeigte sich bei PEX (2394±271 Zellen/mm2) eine um 10,5% (p〈0,05) erniedrigte Endothelzelldichte im Gegensatz zur Kontrollgruppe (2674±341 Zellen/mm2). Intraoperativ unterschieden sich Phakoemulsifikationszeit (90±51 s) und -leistung (38±17%) nicht. Die mittlere Endothelzelldichte nahm sowohl in der PEX-Gruppe als auch beim Vergleichskollektiv nach 4 Wochen um 10,4 bzw. 9,8% (p〈0,001) ab. Die mittlere Zellfläche nahm gleichzeitig um 55 bzw. 48 µm2 (p〈0,001) zu. Der Polymegathismus nahm postoperativ zu, um im späteren Verlauf nach 4 Wochen annähernd präoperative Werte zu erreichen. Der Pleomorphismus nahm nur in der PEX-Gruppe signifikant zu. Schlußfolgerung: Bei PEX scheint es trotz präoperativ reduzierter Zelldichte im Vergleich zu normal-gesunden Augen frühpostoperativ zu keinem erhöhten Endothelzellverlust nach korneoskleralem Tunnelschnitt und Phakoemulsifikation zu kommen. Aufgrund der präoperativ reduzierten Endothelzelldichte werden bei bekanntem PEX endothelschützende Maßnahmen empfohlen.
    Notes: Introduction: Corneal endothelial involvement can be found in pseudoexfoliation syndrome (PEX). Evaluation of possible differences in endothelial cell loss following cataract extraction was compared to normal eyes. Patients and methods: In a controlled clinical study we prospectively studied 20 patients with PEX and compared them with an age-matched control group with senile cataract. All patients were treated with a standardized self-sealing 7-mm corneoscleral tunnel incision, phacoemulsification and posterior intraocular lens implantation using sodium hyaluronate. In addition to a complete ophthalmological examination, quantitative and qualitative endothelial cell analysis of the central and peripheral cornea was performed preoperatively, at the first postoperative day and after 4 weeks using non-contact specular microscopy (Konan Noncon Robo-ca SP 8000, Konan, Japan). Results: In eyes with PEX (2394±271 cells/mm2) endothelial cell counts were 10.5% (P〈0.05) lower than in the control group (2674+341 cells/mm2). Intraoperatively, ultrasound time (90±51 s) and power (38±17%) did not differ between the two groups. After 4 weeks the mean endothelial cell loss in the two groups was 10.4% and 9.8%, respectively (P〈0.001). The mean cell area increased by 55 and 48 µm2 (P〈0.001), respectively. Polymegethism increased postoperatively in both groups and stabilized at 4 weeks at preoperative values. Pleomorphism increased significantly only in the PEX group. Conclusions: In eyes with PEX no increased cell loss was found in the early postoperative period compared to normal eyes following corneoscleral tunnel incision and phacoemulsification. Due to preoperative reduced endothelial cell densities, endothelium-protecting measures are recommended in eyes with PEX.

    Description / Table of Contents: Background: Several studies have confirmed, that the no-stitch technique results in early stabilization of astigmatism. In these papers, however, the follow-up was quite short. Former studies about sutured corneoscleral incisions have shown that even after some years astigmatism increased. Therefore, in this prospective study we investigated the change of induced astigmatism 5 years postoperatively. Methods: The prospective study included 66 patients with scleral tunnel incisions who were monitored up to 5 years. Mean age was 74.3±10.7 years. The incision length of the scleral tunnel was 7 mm. The postoperative astigmatism was measured with keratometry after 1 day, 3 weeks, 1 year and 5 years postopertively. Results: The average postoperative induced astigmatism (PIA) as measured with a keratometer was after 1 year 0.95±0.83 D. After 5 years PIA was 0.96±0.48 D, and therefore no significant statistical difference could be shown. At the first postoperative day (0.94±0.73 D) the induced astigmatism was virtually stable. There was also no statistically significant difference in absolute astigmatism. After 1 day the absolute astigmatism was 0.91±0.73 D and after 5 years 0.96±0.61 D. Conclusions: The contrast to former sutured corneoscleral incisions, the no-stitch technique in cataract surgery yields stable postoperative corneal astigmatism.
    Notes: Hintergrund: In zahlreichen Studien konnte belegt werden, daß die No-stitch-Technik zu einem frühzeitig stabilen Astigmatismus führt. Die Untersuchungszeiträume waren jedoch in allen bisherigen Studien relativ kurz. Von der früher üblichen korneoskleralen Schnittechnik mit Kreuzstichnaht ist bekannt, daß selbst nach Jahren noch eine Astigmatismusänderung möglich ist. Es wird daher erstmals eine prospektive Studie zur langfristigen kornealen Astigmatismusentwicklung bei der No-stitch-Technik vorgelegt, die einen Nachkontrollzeitraum von über 5 Jahren umfaßt. Patienten und Methoden: Es konnten 66 Patienten mit einem Durchschnittsalter von 74,3±10,7 Jahren, die Anfang 1991 (Zeitpunkt der Einführung der No-stitch-Technik als Standardtechnik im Virchow-Klinikum Berlin) operiert worden waren, über durchschnittlich 5 Jahre und 2 Monate nachverfolgt werden. Die Breite des skleralen Tunnels betrug 7 mm. Es wurde nach 1 Tag, 3 Wochen, 1 Jahr und 5 Jahren der Astigmatismus mit dem Zeiss-Ophthalmometer bestimmt und der induzierte Astigmatismus vektoranalytisch berechnet. Ergebnisse: Es zeigte sich, wie schon in früheren Arbeiten vermutet wurde, daß durch Wegfall der Naht eine frühe Stabilität der Astigmatismusänderung auf einem vergleichsweise niedrigem Niveau erreicht wurde. So konnte zwischen den Werten nach 1 Jahr mit vektoranalytisch berechneten 0,95±0,63 dpt im Vergleich zu den Werten nach 5 Jahren mit 0,96±0,48 dpt kein signifikanter Unterschied gefunden werden. Bereits ab dem 1. postoperativen Tag (0,94±0,73 dpt) war die Astigmatismusinduktion, die neben dem Astigmatismusbetrag auch dessen Achse berücksichtigt, stabil. Auch beim Astigmatismusbetrag allein war mit 0,91±0,73 dpt nach 1 Tag und 0,96±0,61 dpt nach 5 Jahren keine statistisch signifikante Änderung zu verzeichnen. Bei den Astigmatismusachsen überwog nach 1 Jahr und nach 5 Jahren ein Astigmatismus gegen die Regel. Schlußfolgerung: Im Vergleich zur früheren korneoskleralen Schnittechnik mit Kreuzstichnaht ist bei der No-stitch-Technik der postoperative korneale Astigmatismusbetrag bereits frühzeitig auf niedrigem Niveau stabil.

    Description / Table of Contents: Background: Since increased intraoperative iris irritation can lead to increased postoperative inflammation, we are interested in postoperative reactions to several varied surgical procedures. We performed pupil stretching, iridotomy with iris suture, and partial sphincterectomy. Materials and methods: From January 1995 to January 1996, 100 patients (103 eyes) with narrow pupils underwent cataract surgery. In 13 eyes a iridotomy and iris suture were performed, in 28 eyes a partial sphincterectomy. In 62 eyes there was no surgical intervention after pupil stretching. Postoperative examinations were carried out in the early postoperative phase (up to 5 days postoperative) as well as 4 weeks after surgery. Fibrin reaction was classified according to 3 grades: (I) faint fibrinous threads, (II) fibrinous net, and (III) membranous fibrin exsudation. Results: In the group without additional surgery there was a fibrinous reaction of grade I and II in 11 eyes. This complication occurred after iridotomy and iris suture in four cases and after sphincterectomy in four cases. While the intensity of fibrinous reaction was comparable in the first two groups, the fibrinous reaction after sphincterectomy was more intensive in one eye (grade III). However, response to intensive local antiphlogistic therapy was good. Frequency of fibrinous reaction in the three groups was statistically not significant. It should be pointed out that there was a partial restoration of pupil movements through sphincterectomy: mean pupil diameter was 4 mm, and 2.5 mm under exposure to light. Three patients had no pupillary reaction at all. Conclusions: Partial sphincterectomy facilities intraoperative manipulations of cataract surgery. Postoperative inflammatory reaction is rare and was very responsive to medical treatment. The reconstruction of pupillary movement is part of full visual function and, last but not least a round pupil is aesthetically more desirable.
    Notes: Fragestellung: Da eine vermehrte Pupillen- und Irisirritation während der Operation eine postoperative Inflammation verstärken können, interessierte uns die Frage der postoperativen Reaktionen nach verschiedenen chirurgischen Verfahren wie Pupillenstretching ohne weitere chirurgische Intervention, Iridotomie und Irisnaht oder nach der von uns eingeführten partiellen Sphinkterektomie. Material und Methode: Im Zeitraum von Januar 1995 bis Januar 1996 wurde bei 100 Patienten (103 Augen) mit enger Pupille kataraktoperiert. Bei 59 Patienten (62 Augen) wurde ein Pupillenstretching (Gruppe 1) durchgeführt. Bei 13 Augen wurden eine Iridotomie und Irisnaht (Gruppe 2) sowie bei 28 Augen eine partielle Sphinkterektomie (Gruppe 3) vorgenommen. Die postoperativen Befunde wurden früh postoperativ (bis zum 5. postoperativen Tag) sowie nach ca. 4 Wochen erhoben. Die Fribrinreaktionen wurden in 3 Stufen klassifiziert: zarte Fibrinfädchen, Fibrinnetz und membranöse Fibrinexsudation. Ergebnisse: In Gruppe 1 fand sich in 11 Augen eine Fibrinreaktion (Grad I–II). Diese Komplikation trat nach Iridotomie und Irisnaht in 4 Fällen und nach Sphinkterektomie ebenfalls in 4 Fällen auf. Während die Intensität der Fibrinreaktionen in den ersten 2 Gruppen vergleichbar war, war in 1 Fall nach Sphinkterektomie eine ausgeprägte Fibrinreaktion (Grad III) zu verzeichnen, die sich jedoch unter intensiver lokaler antiphlogistischer Therapie zurückbildete. Die Häufigkeit der Fibrinreaktionen der 3 Gruppen war statistisch nicht signifikant unterschiedlich. Hervorzuheben ist die partielle Wiederherstellung der Pupillenmotorik nach Sphinkterektomie, wobei die durchschnittliche Pupillengröße knapp 4 mm und unter Lichteinwirkung 2,5 mm betrug. Bei 3 Patienten war keine Pupillenreaktion vorhanden. Schlußfolgerung: Eine partielle Sphinkterektomie erleichtert somit die Kataraktchirurgie. Auch die postoperative entzündliche Reaktion ist vertretbar selten und konnte medikamentös gut beherrscht werden. Die Wiederherstellung der Pupillenmotorik dürfte eine Rolle in der visuellen Funktion spielen, und nicht zuletzt stellt sich die runde Pupille auch ästhetisch besser dar.

    Description / Table of Contents: Hintergrund: Die Therapie der durch subretinale neovaskuläre Membranen bedingten altersabhängigen Makuladegeneration stellt trotz der Möglichkeiten der Laserkoagulation noch immer ein großes Problem dar. Besonders bei subfoveolär gelegenen Neovaskularisationen kann eine Lasertherapie nur unter Inkaufnahme eines erheblichen sofortigen Visusverlustes durchgeführt werden. Als Alternative wird nun seit einiger Zeit wieder die Strahlentherapie genannt. Die Wirksamkeit und die Komplikationen sollten in der vorliegenden randomisierten Studie untersucht werden. Patienten und Methoden: In die prospektive und randomisierte Studie sind bislang 76 Patienten (51 Frauen, 25 Männer, Durchschnittsalter 77,7 ± 8,6 J.) mit fluoreszenzangiographisch nachweisbaren subretinalen choroidalen Neovaskularisationen und einem Visus zwischen 0,05 und 0,5 aufgenommen worden. Sie wurden randomisiert zu gleichen Teilen einer Strahlentherapiegruppe und einer Kontrollgruppe zugeteilt. Die Bestrahlungen erfolgten innerhalb von 6 Tagen mit 6 × 2 Gy (6 MV Photonen eines Linearbeschleunigers) unter weitestgehender Schonung der Linse. Die Nachuntersuchungen wurden 4 Wochen nach Abschluß der Strahlenbehandlung, nach 3 Monaten, nach 6 Monaten und dann im halbjährigen Abstand vorgenommen. Die durchschnittliche Nachkontrollzeit betrug 15,1 Monate. Ergebnisse: Kontrollgruppe und Strahlentherapiegruppe waren bezüglich des Ausgangsvisus in Ferne und Nähe und des Lebensalters nicht signifikant unterschiedlich. 4 Wochen nach Bestrahlung lag in der Strahlentherapiegruppe ein Visus von 0,13 ± 0,46 (LogMAR) vor. Nach 12 Monaten betrug in der Verumgruppe der Fernvisus 0,11 ± 0,30 und in der Kontrollgruppe 0,09 ± 0,13 (p = 0,838). Bei einem höheren Ausgangsvisus als 0,2 konnten im Trend bessere Resultate nach Bestrahlung erzielt werden. Hervorzuheben ist die Besserung von Metamorphopsien in der Behandlungsgruppe bei 75 % der Patienten. An Komplikationen fanden sich in der Verumgruppe und in der Kontrollgruppe 3 Patienten mit subretinalen Blutungen. Schlußfolgerung: Bei einer derzeitigen Nachkontrollzeit von 15 Monaten lassen sich noch keine eindeutigen Vorteile der Strahlentherapie- gegenüber der Kontrollgruppe erkennen, wenngleich Metamorphopsien nach Bestrahlung signifikant seltener auftraten. Ein höherer Ausgangsvisus als 0,2 läßt tendenziell bessere Resultate nach Bestrahlung erwarten, was jedoch mit höheren Fallzahlen noch untermauert werden müßte.
    Notes: Summary Despite the successful therapy of subretinal neovascular membranes by laserphotocoagulation there are many problems to be overcome. In the case of subfoveolar neovascularization, photocoagulation leads to a sudden decrease in visual acuity.Recently radiotherapy is considered as an alternative. Complications and effectivity were evaluated in this prospective and randomized trial. The initial results are presented. Patients and methods: There are 76 patients (51 women, 25 men, average age 77.7 ± 8.6 years) included in the prospective randomized study. All of them show subfoveolar neovascular membranes in FLA and a decrease in visual acuity between 0.05 and 0.5. They were randomly assigned to either the radiotherapy or the control group. Radiotherapy was done within 6 days by 6 × 2 Gy (6 MV photons). The follow-up was at 4 weeks, after 3 months, after 6 months and then every 6 months after the end of radiotherapy. On average the follow-up is at 15.1 months. Results: Concerning age and visual acuity before therapy, the control group and the radiotherapy group were not significantly different. At 4 weeks after radiotherapy, visual acuity was 0.13 ± 0.46 (LogMAR). After 12 months, visual acuity at a distance was 0.11 ± 0.30 in the therapy group and 0.09 ± 0.13 (P = 0.838) in the control group. Patients with a preoperative visual acuity better than 0.2 improved more after radiotherapy. Metamorphopsy improved in 75 % of the therapy group. The following complications could be observed: In the control group 3 patients suffered subretinal bleeding, in the radiotherapy group 3 patients, respectively. Conclusions: At present, the follow-up is too short to recommend radiotherapy as a standard procedure in the case of subfoveolar neovascularization. The results in patients with a better preoperative visual acuity encourage us to continue this study.
