    Notes: Summary Two patients with non-traumatic acute subdural haematoma were observed, initially without surgical intervention. Eleven days after the onset, each patient developed hemiparesis and an increase in severity of headache. Serial computed tomography scans demonstrated that the initial hyperdense haematomas became hypodense with a definite increase in volume. The term “symptomatic subacute subdural haematoma” was proposed to properly define this pathological process, which necessitated removal of the haematoma in the subacute stage. The mechanism of increase in the volume of the haematoma is discussed.

    Notes: Summary An elderly patient suffering acute subdural haematoma associated with cerebral contusion was treated by percutaneous subdural tapping while preparing for craniotomy. Most of the subdural haematoma, though of high density on computed tomography scanning, proved to be semiliquid. Drainage of the haematoma yielded resolution of the mass effect, and was followed by a rapid improvement of consciousness. This observation suggests the significance of trial subdural tapping for the treatment of acute traumatic subdural haematoma prior to craniotomy.

    Notes: Summary Using immediate ventricular drainage and aggressive postoperative therapy, poor-grade patients suffering ruptured cerebral aneurysms could have a greater opportunity for meaningful recovery. This treatment protocol is facilitated by percutaneous ventricular drainage at the bedside. A simple and less invasive technique reported here is particularly useful for neurosurgeons responsible for the treatment of severe subarachnoid haemorrhage on an emergency basis.

    Notes: Summary  A 57-year-old, previously healthy man who developed acute onset of hemiplegia is presented. Neuro-imaging studies on admission suggested dissection of the middle cerebral artery producing infarction in the frontotemporal region. In contrast to his stable clinical course, serial neuro-imaging studies disclosed rapid growth of malignant glioma, which was confirmed at surgery. Microscopic examination of the surgical specimen demonstrated invasion of glioma cells into the arterial wall associated with intramural haematoma formation of the middle cerebral artery. This case is the first to document dissection of an intracranial artery caused by invasion of tumour cells.

    Notes: Summary Eight reported cases of neonatal epidural haematoma are reviewed. Special attention is directed at the correlation between the haematoma content and computed tomography (CT) features. Seven cases had a high density haematoma on CT, of which four showed liquid, one semi-liquid, one liquid and solid, and one solid haematoma. The low density haematoma in one case was liquid. These results suggest that evacuation of the epidural haematoma via percutaneous epidural tapping is warranted as the first line of treatment irrespective of high density on CT.

    Notes: Summary Four premature infants weighing less than 2,500 grams underwent lumboperidoneal shunt for their communicating hydrocephalus. Using a microscope, mini-laminectomy and exact placement of the proximal end of the shunt tube into the subarachnoid space was achieved. After the lumboperitoneal shunting, hydrocephalus in all four patients was relieved without any complications. The author discusses the availability of lumboperitoneal shunt for hydrocephalus in premature infants.

    Notes: Summary Ten patients with postoperative collection of subcutaneous cerebrospinal fluid (pseudomeningocoele) refractory to nonsurgical treatment underwent lumboperitoneal (LP) shunt. In all patients rapid resolution of the subcutaneous collection was achieved, and no recurrence was observed during the follow-up period. Complications of the LP shunt included transient sciatica in one patient, and acute subdural haematoma after a mild head trauma in another patient who had had a subdural fluid collection. A LP shunt proved to be a less invasive and reliable method of treatment for this condition when it fails to resolve despite aspiration of cerebrospinal fluid combined with mechanical compression of the wound. However, in cases with subdural fluid collection, early withdrawal of the LP shunt may be recommended.

    Notes: Summary Computed tomography features of ruptured arteriovenous malformations in the acute stage were reviewed. Intracerebral or intraventricular haemorrhage was the major finding, and primary subarachnoid haemorrhage was disclosed to be rare. This observation supports the concept that arteriovenous malformations should not be included in the main pathological conditions causing subarachnoid haemorrhage. This information is of value in identifying the source of intracranial haemorrhage in patients with the coexistence of an arteriovenous malformation and an aneurysm, and provides a significant guideline for treatment.
