    Description / Table of Contents: Abstract Introduction: Postoperative delirium is a common psychic disturbance occurring acutely after various surgical procedures and typically presenting with a fluctuating course. These patients' recovery takes longer. In this study we analyze the incidence of postoperative delirium in patients undergoing vascular surgery and try to identify risk factors for its development. Methods: Patients undergoing elective arterial operations were included. Their medical history, the specific vascular diagnosis and operation performed, the medication and laboratory data were monitored. Additionally the patients were preoperatively interviewed by a psychiatrist. Intraoperatively the drugs, infusions, possible transfusions, blood gases and pressures were monitored, as were the times of surgery and anesthesia. Postoperatively patients were seen daily by the psychiatrist and the surgeon for at least 7 days. Postoperative delirium was diagnosed according to DSM IV criteria, and mild, moderate and severe delirium were distinguished. Results: Fifty-four patients entered the study. Twenty-one (38.9%) developed postoperative delirium (11 mild, 2 moderate, 8 severe). Patients with aortic operations developed delirium more frequently than those with non-aortic procedures(55.5 vs 22.2%, n=27 each). Some preexisting diseases (hearing disturbance) increased the probability of postoperative delirium, while age was not identified as a risk factor. General psychopathological and depressive disturbances increased the likelihood of postoperative delirium. Patients who had a severe intraoperative course developed postoperative delirium more frequently. This was not seen in the absolute time of surgery or anesthesia nor in the intraoperative development of blood pressure or intraarterial gases, which did not differ between patients with and without postoperative delirium. More reliable parameters were an increased intraoperative need for crystalloid volume, intra – or postoperatively decreased hemoglobin values (Hb 〈10 g/dl) and the development of acidosis that had to be treated. Patients with delirium had serious complications more often (8/21=38.1% vs 6/33=18.2%) and needed Intensive Care treatment longer (2.7 vs 2.1 days, only aortic surgery 3.2 vs 2.4 days). Conclusions: Postoperative delirium after vascular surgery is frequent. Patients undergoing aortic surgery, with specific concomittant medical disease, psychopathological disturbances and a severe intraoperative course, are at risk of developing postoperative delirium.

Patienten, deren intraoperativer Verlauf kompliziert war, entwickelten häufiger ein Durchgangssyndrom. Dabei war die Schwere des Verlaufes nicht an der absoluten Operationszeit, dem Blutdruck oder den Blutgasen abzulesen. Aussagekräftiger waren ein intra- oder postoperativ erniedrigter Hämoglobinwert (Hb 〈10 g %), ein erhöhter intraoperativer Infusionsbedarf und eine intraoperative Acidose. Patienten mit Durchgangssyndromen entwicklten häufiger postoperative Komplikationen (8/21=38,1% vs 6/33=18,2%) und mußten länger intensivmedizinisch behandelt werden (2,7 vs 2, 1 Tage, nur aortale Eingriffe 3,2 vs 2,4 Tage). Schlußfolgerungen: Postoperative Durchgangssyndrome sind nach gefäßchirurgischen Operationen häufig. Vor allem Patienten mit aortalen Operationen, mit bestimmten Begleiterkrankungen, psychopathologischen Auffälligkeiten und solche mit einem komplizierten intraoperativen Verlauf sind gefährdet, eine derartige Komplikation zu entwickeln.

Die Osteomyelofibrose (OMF) ist eine myeloproliferative Erkrankung, bei der es durch Fibrosierung und Sklerosierung zu einer Verödung des Knochenmarks kommt. Kompensatorisch findet in Leber und Milz extramedulläre Hämatopoese statt. Die resultierende Splenomegalie kann ihrerseits durch lokale Kompression, Thrombocytopenie und hämolytische Anämie zu bedrohlichen Symptomen führen. Die Splenektomie ist bei diesen Patienten trotz ihrer Risiken die einzig erfolgversprechende Therapie.
    Notes: Summary. Osteomyelofibrosis is a myeloproliferative disorder in which fibrosis and sclerosis finally lead to bone marrow obliteration. Liver and spleen compensate for bone marrow loss with extramedullary hematopoesis. In some patients the resulting splenomegaly causes severe symptoms such as local compression, thrombocytopenia and hemolytic anemia. In such patients, splenectomy is the only promising treatment, although it represents a significant risk.

    Description / Table of Contents: Zusammenfassung Die richtige Diagnose bei akuten Bauchschmerzen ist eine Grundvoraussetzung für die sich anschließende adäquate Therapie. In zahlreichen klinischen Studien konnte aufgezeigt werden, daß trotz Verbesserung der Labor- und Apparatemedizin erhebliche Mängel durch unzureichende Anamneseerhebungen und klinische Untersuchungen entstehen. Durch Einführung einer standardisierten und strukturierten Anamnese und klinischen Untersuchung alleine läßt sich die diagnostische Richtigkeit um mindestens 10% verbessern. Ziel dieser Arbeit ist eine Verbesserung von Anamnese, klinischer Untersuchung und Diagnosestellung durch eine möglichst exakte Definition aller relevanten Parameter. Dies erfolgte durch eine nationale klinische Expertengruppe, wobei bereits vorliegende internationale Standardisierungen (z. B. Weltgesellschaft für Gastroenterologie) Berücksichtigung fanden. Diese Standardisierungen wurden zum überwiegenden Teil übernommen, mußten jedoch an einigen Stellen überarbeitet, weiterentwickelt und aktualisiert werden. Um eine weitgehende Verbreitung der Standardisierung zu erreichen, können ein Dokumentationsbogen und ein Dokumentationsprogramm zur Verfügung gestellt werden.
    Notes: Abstract The correct diagnosis in acute abdominal pain is necessary for adequate treatment. In several clinical studies it has been shown that, despite improvements in laboratory and technology medicine, errors occur in a considerable proportion of cases due to insufficient history-taking and clinical examination. By the introduction of a standardised and structured history and clinical examination, the diagnostic accuracy can be improved by at least 10%. The aim of this publication is to improve history-taking, clinical examination and diagnostic decision-making by exact definition of all relevant parameters. This was performed by a national clinical expert group, international standardisations were taken in consideration. The standardisation was based mainly on these existing international standardisations (World Organisation of Gastroenterology); however, revisions and corrections were necessary. In order to introduce the standardisation into clinical routine, a documentation form and a documentation program can be provided.

Fifty-six patients with various hematological disorders were splenectomized between 1990 and 1994. The main indication was noted. Operative success was defined as: return to normal platelet counts without further medication in thrombocytopenia, relief of pain and local compression syndrome in painful splenomegaly, hemoglobin levels 〉10 g/dl without the need for further transfusions in hemolytic anemia, response to chemotherapy after splenectomy for prior resistance because of massive splenic infiltration, and relief of infection in splenic infection. Morbidity and mortality were noted. Five major indications for splenectomy were found: thrombocytopenia (n=36, success 78%), painful splenomegaly (n=8, success 100%), hemolytic anemia (n=5, success 60%), resistance to chemotherapy because of massive splenic infiltration (n=5, success 40%), and splenic infection after chemotherapy (n=2, success 100%). One patient with thrombocytopenia died (mortality 2%). Seven patients had major complications (13%). In hematological diseases, thrombocytopenia, painful splenomegaly and splenic infection are likely to be improved by splenectomy. In hemolytic anemia it can be a helpful approach, while in resistance to chemotherapy because of massive splenic infiltration success is less likely.
    Notes: Zusammenfassung Ziel dieser retrospektiven Studie war, herauszuarbeiten, welche Indikationen bei hämatologischen Erkrankungen zu einer Splenektomie führen und die operativen Ergebnisse in Abhängigkeit von den Indikationen zu analysieren. 56 Patienten mit verschiedenen hämatologischen Erkankungen wurden zwischen 1990 und 1994 splenektomiert. Die Hauptindikation wurde erfaßt. “Erfolg” wurde definiert als: normale Thrombozytenzahlen ohne weitere Medikation bei Thrombozytopenie, Schmerzfreiheit nach Operation wegen schmerzhafter Splenomegalie, Hämoglobin 〉10 g/dl ohne postoperativen Transfusionsbedarf bei Patienten mit hämolytischer Anämie, Ansprechen auf Chemotherapie nach Splenektomie bei präoperativer Chemotherapieresistenz bedingt durch massive Milzinfiltration und Infektionsfreiheit bei Milzinfektion. Letalität und Morbidität wurden analysiert.
