    Description / Table of Contents: Zusammenfassung Das Hämangiopericytom ist ein sehr seltenes, von den Capillarpericyten ausgehendes Weichteilsarkom. An der Chirurgischen Universitätsklinik Essen wurden von 1962–1980 zehn Fälle von Hämangiopericytom operiert. Wir haben in 6 Fällen Rezidive, in 5 Fällen Metastasen gesehen; es überlebten nur 3 Patienten in der Nachbeobachtungszeit von 5–10 Jahren. Die weite chirurgische Excision ist unseres Erachtens die einzige erfolgversprechende Therapie. Eine Nachbestrahlung wird trotz nicht eindeutiger Ergebnisse befürwortet.
    Notes: Summary The hemangiopericytoma is a very rarely occurring sarcoma of the soft tissues. At the Surgical Department of the University of Essen from 1962–1980 ten cases of hemangiopericytoma were treated by operation. During a follow-up period of 5–10 years local recurrence was seen in six cases, metastasis in five cases. Only three patients survived to the end of the follow-up period. Wide surgical excision is the preferred treatment, and post-operative irradiation should be done although clinical studies are contradictory.

    Description / Table of Contents: Zusammenfassung 43 von 92 Patienten, die wegen einer Strahlenreaktion des Darmes seit 1962 an unserer Abteilung operiert wurden, boten eine Ileussymptomatik. Relativ häufig war die Fehldiagnose Ileus durch Tumorrezidiv. Nach unseren Erfahrungen kann die Beachtung folgender Gesichtspunkte die Differantialdia-gnose zum Ileus durch Strahlenreaktion erleichtern: 1. die Lokalisation des bestrahlten Tumors; 2. die Höhe der Strahlendosis; 3. die Voroperationen; 4. die Frühreaktion direkt im Anschluß an die Bestrahlung; 5. das Intervall zwischen Bestrahlung und Operation; 6. mit Einschränkung die Röntgendiagnostik; 7. das Vorliegen einer ein- oder doppelseitigen radiogenen Ureterstenose.
    Notes: Summary 43 of the 92 patients operated on in our department since 1962 because of irradiation injury to the intestine exhibited symptoms of intestinal obstructions. Relatively frequently intestinal obstruction after radiation was diagnosed as a recurrent tumor. According to our experience the following points may be helpful in the diagnosis of irradiation injury causing intestinal obstructions: 1. Localisation of the irradiated tumor; 2. Irradiation dose; 3. Preliminary operations; 4. Reaction directly after irradiation; 5. Time between irradiation and operation; 6. Possibly X-ray features; 7. Existence of a unit- or bilateral ureteral stenosis caused by irradiation.

    Description / Table of Contents: Zusammenfassung Zwischen 1962 und 1972 wurden 30 Patienten wegen eines Strahlen-sebadens des Dünn- und Dickdarms operativ behandelt. 20 mal wurde klinisch der Verdacht auf ein Tumorrezidiv geäußert, aber nur 5mal bestätigt. 13 Patienten überleben 5 Jahre, davon 3 10 Jahre. Früh- und Spättodesfälle fielen in die ersten 5 Jahre nach Tumorentdeckung. Vom operationstechnischen her ist die Seit-zu-Seit-Anastomose am Dünndarm sicherer als die End-zu-End-Anastomose.
    Notes: Summary Between 1962 and 1972, 30 patients suffering from radiation injuries of the small bowel and the colon underwent operative treatment. In 20 cases recurrence of tumor was suspected before surgery, but this was only confirmed in 5 cases. 13 patients have survived for 5 years, 3 of these for 10. Early or late death occurred within 5 years after the discovery of neoplasms. Side-to-aide anastomosis of the small bowel seems to be safer than end-to-end anastomosis.

    Description / Table of Contents: Zusammenfassung Es wird über kritische Punkte in der Methodik der CEA-Bestimmung nach Hansen u.a. (CEA-RIA-Test der Firma Hoffmann-La Roche) berichtet. Bei 82 Patienten mit colorectalem Carcinom konnte die CEA-Bestimmung durchgeführt werden. Postoperativ wurden bei 43 Patienten 4 Tage, 8 Tage, 14 Tage, 3 Monate und 6 Monate, vereinzelt auch später, die CEA-Werte beobachtet. Von 61 präoperativ bestimmten Patienten lagen 52,5% über der vorgeschlagenen Normgrenze von 2,5 ng/ml. Bei der statistischen Auswertung mit der Multivarianzanalyse ergab sich eine Wechselwirkung der verschiedenen Stadien auf die CEA-Werte. Postoperativ fallen die CEA-Werte unterschiedlich rasch ab. Ein Wiederanstieg des CEA-Wertes zeigt bereits weit vor der klinischen Manifestierung das Rezidiv an. In einem Fall wurde prä- und postoperativ trotz Lebermetastasen kein erhöhter CEA-Wert gemessen. Erst 11 Monate postoperativ, nach Generalisierung der Erkrankung, konnte ein erhöhter CEA-Wert nachgewiesen werden. Es sollte deshalb auch bei normalen CEA-Werten die Verlaufskontrolle fortgesetzt werden.
    Notes: Summary Some critical points of the Hansen method for CEA determination are presented, and an automatical dialysis unit is described. Forty-three of 82 patients with colorectal carcinoma were studied four, eight, and fourteen days, and three and six months after surgery. In 61 (52.5%) the CEA level was higher than 2.5 ng/ml. Multiple variance analysis showed a significant dependence of CEA levels on the Dukes classification. After surgery, different slopes were seen in the decrease of CEA levels. Several months before its clinical detection, recurrent disease could be recognized by a renewed increase of CEA values. Interestingly, in one patient with liver metastases who initially had normal CEA values, a high CEA level was found 11 months later, when the disease was generalized.

    Notes: Abstract Five metabolites of diclofenac sodium (Voltarol®) have been identified in human plasma. All five metabolites were more than 50 times less potent than diclofenac in inhibiting PGE2 production in zymosan-stimulated mouse macrophages and LTC4 synthesis was not inhibited in these cells. Anti-inflammatory activity (adjuvant arthritis and carragheenan-induced paw oedema in rats) and analgesic activity (phenyl-p-benzoquinone writhing, mouse) of the metabolites were at least 10 times lower when compared to diclofenac. There was a good correlation betweenin vitro PGE2 inhibition andin vivo activities for diclofenac and its metabolites indicating that inhibition of prostaglandin synthesis is a major mechanism responsible for their pharmacological actions.
