    Notes: Soft tissue injury of tendons represents a major problem within sports medicine. Although several animal and cell culture studies have addressed this, human experiments have been limited in their ability to follow changes in specific tissue directly in response to interventions. Recently, methods have allowed for in vivo determination of tissue concentrations and release rates of substances involved in metabolism, inflammation and collagen synthesis, together with the measurement of tissue blood flow and oxygenation in the peritendinous region around the Achilles tendon in humans during exercise. It can be demonstrated that this region experiences an increase in blood flow during both static and dynamic exercise, and that exercise causes increased metabolic activity, accumulation of inflammatory mediators (prostaglandins) and increased formation of collagen type I in response to acute exercise. This coincides with a surprisingly marked drop in tissue pressure during contraction. With regards to both circulation, metabolism and collagen formation, peritendinous tissue represents a dynamic, responsive region that adapts markedly to acute muscular activity.

    Notes: Near infrared spectroscopy (NIRS) is becoming a widely used research instrument to measure tissue oxygen (O2) status non-invasively. Continuous-wave spectrometers are the most commonly used devices, which provide semi-quantitative changes in oxygenated and deoxygenated hemoglobin in small blood vessels (arterioles, capillaries and venules). Refinement of NIRS hardware and the algorithms used to deconvolute the light absorption signal have improved the resolution and validity of cytochrome oxidase measurements. NIRS has been applied to measure oxygenation in a variety of tissues including muscle, brain and connective tissue, and more recently it has been used in the clinical setting to assess circulatory and metabolic abnormalities. Quantitative measures of blood flow are also possible using NIRS and a light-absorbing tracer, which can be applied to evaluate circulatory responses to exercise along with the assessment of tissue O2 saturation. The venular O2 saturation can be estimated with NIRS by applying venous occlusion and measuring changes in oxygenated vs. total hemoglobin. These various measurements provide the opportunity to evaluate several important metabolic and circulatory patterns in very localized regions of tissue and may be fruitful in the study of occupational syndromes and a variety of diseases.

    Notes: Abstract We report a prospective series of 101 patients receiving a four-strand semitendinosus-gracilis autograft for anterior cruciate ligament reconstruction using a new bicortical tibial fixation technique with bioabsorbable interference screws. Patients (average age 32 years) were operated on between November 1997 and Mai 1998, and follow-up was at least 12 months postoperatively. The evaluation consisted of history, clinical examination, IKDC score, Cincinnati Knee Score, KT-1000 testing, standardized radiography and magnetic resonance imaging. One hundred patients were available for follow-up. There were two traumatic reruptures 6 and 11 months postoperatively. By IKDC score 87 patients were in groups A and B, 12 in group C, and 2 with a rerupture in group D. Mean Cincinnati Knee Score was 82 (range 46–100), KT-1000 manual maximum at follow-up showed an average difference of 1.7 mm between the sides. Full extension was rapidly achieved in all cases, and flexion averaged 135°. Considering the enlargement caused by the bioabsorbable interference screws, the incidence of tunnel widening was 6.6% on the femoral side and 1.7% on the tibial side. Tunnel widening did not affect the clinical results. Three patients experienced an effusion after more than 6 months postoperatively. There was one infection, which settled without functional impairment after early arthroscopic lavage and intravenous antibiotics. Otherwise no complications occurred. The described technique for anterior cruciate ligament reconstruction combines the advantage of anatomical graft position with sufficient graft fixation. A short intra-articular graft construct with a strong tendon to bone interface was created. Clinically, 87% of patients had a normal or nearly normal IKDC score at follow-up. Magnetic resonance imaging revealed a considerable increase in tunnel area caused by the insertion of the interference screws. On the other hand, the development of a “fibrous interzone” between the graft and the bone tunnel was prevented in most cases. At short-term follow-up the bicortical tibial fixation proved to be efficient even with an aggressive rehabilitation program.

    Notes: Abstract The xenon clearance method was adapted to continuous measurement of interscapular brown adipose tissue (ISBAT) blood flow in anaesthetized rats. The ISBAT-blood partition coefficient for xenon was determined to 3.6 ml·g−1. The blood flow values obtained by Xe clearance were compared with flow values obtained concomitantly by the microsphere technique in 17 cold acclimated rats, at ISBAT blood flows between 0.1 and 6 ml·g−1·min−1. Variations in blood flows were obtained by infusion of noradrenaline at different rates. The blood flow values obtained from the xenon clearance method showed a close correlation to the blood flow values determined with microspheres.Y=0.98.X+0.15 (r=0.96,P〈0.001). The Xe clearance method has the advantages compared to the microsphere technique that it permits continuous monitoring of the blood flow and does not require the sacrifice of the animal.

    Notes: Abstract Tissue blood flow was measured by the tracer microsphere technique in the juxta-articular bones of the immature knee, knec joint capsule and skeletal muscles of the hind limbs after unilateral bone cannulation of the distal femoral epiphysis, surgical exploration of the popliteal vessels and unilateral isolated increase of venous outlet resistance in the distal femoral epiphysis during Halothane anaesthesia. Pressure registration was performed in a liquid-filled electromanometric pressure recording system. Significant regional differences in bone blood flow was found within the different bone compartments of both control and test limb. Bone cannulation resulted in insignificant hyperaemia in the distal femoral epiphysis. No changes of distribution pattern of bone blood flow followed the surgical procedure. Ligation of the medial genicular vein caused a three fold increase of venous outlet resistance measured after 5 min, while an insignificant decrease of arterial inlet resistance was calculated. After one hour a significant decrease of both arterial inlet resistance and venous outlet resistance was found. In addition evidence of a veno-arterial reflex mechanism causing a significant unilateral decrease of distal skeletal muscle blood flow was found. The results demonstrate the presence of both venous and arterial regulation, predominantly metabolic, of bone blood flow in the knee.

    Notes: Abstract The aseptic loosening rate in uncemented hip endoprostheses, especially the stem, is one of the most discussed problems among hip surgeons within the last few years. The first generation of uncemented stems had an essentially higher loosening rate compared with cemented stems. Although there are many reasons for this disappointing fact, the type of primary fixation and polyethylene wear are the two main ones. We reviewed 145 sequential uncemented CLS Spotorno stems, which achieved their primary fixation and bony in- or ongrowth predominantly in the metaphyseal region. The mean age of the patients was 54.9 years (range 37–76 years), and average follow-up was 7.8 years. The evaluation included both clinical (Merle d'Aubigné and Harris Hip Score) and radiographic parameters (plain X-rays). The medium-term follow-up showed a survival rate of 96.7% and good functional score results in more than 90%. Special attention was paid to the so-called thigh pain phenomenon which, despite current views, was found to be unexpectedly low (only one case). Although these results are encouraging, a further long-term follow-up of more than 10 years will have to show whether or not comparable results to cemented stems can be realised.

    Notes: Summary The blood flow of the dog urinary bladder measured by radioactive microsphere technique was compared to the clearance of locally injected 99mTechnetium pertechnate (99mTc) in the bladder wall. In semilogarithmic plots the 99mTc washout curves showed a multiexponential course. From the initial slopes (median 5.7 min) the bladder blood flow was calculated to be only 30–62% of the results obtained from the radioactive microsphere technique (blood flow in the muscular layer 21.7–44.8 ml/ 100 g/min). These lower values imply that the rate of removal of the hydrophilic tracer 99mTc at these flow rates is limited by the capillary diffusion capacity. The multiexponential curves reflect recirculation and excretion of 99mTc by the kidneys with accumulation of 99mTc in the bladder. It is concluded, that clearance studies of locally injected 99mTc in the bladder wall are unable to evaluate bladder blood flow.

Current medium term follow up showed a survival rate of 96.7% and a good functional score in more than 90%. Special attention was paid to postoperative thigh pain which occurred in only one patient. These results are encouraging, but a follow up of more than 10 years is required before comparable results to cemented stems can be claimed.

    Notes: Abstract The paper describes a photoacoustic spectrometer for detection of NH3 in power plant emission with a detection limit below 1 ppm. The radiation source is a tunable CO2 waveguide laser, and detection is performed at reduced pressure, where the vibration-rotation lines of NH3 are essentially Doppler broadened. Immunity against interference is ensured by recording a characteristic spectral profile, and problems associated with the high concentration of CO2, and the associated line center absorption are eliminated by utilizing the effect of kinetic cooling on the photoacoustic phase. A computerized spectrometer has been constructed and tested under realistic conditions at a Danish power plant operating a test facility for selective non-catalytic reduction of NO x . Results of this field test are given.
