    Description / Table of Contents: Summary In 16 hypertensive patients with unilateral, in 3 patients with bilateral renal artery stenoses (RAS) and in 3 patients with unilateral kidney diseases of different etiology we performed renin estimations in peripheral venous plasma (PVRC) according to Brown and split function tests according to Stamey. In 14 patients we estimated, in addition, renin concentrations in renal vein plasma (NVRC). 8 patients with known PVRC and 5 patients with known PVRC and NVRC were operated upon. In our experience, operative cure can be exspected with some probability in a patient with elevated PVRC when inulin- and PAH-clearances of the opposite kidney are normal, and when a second possible cause of elevated plasma renin can be excluded. A normal PVRC does not preclude postoperative lowering of blood pressure. In 12 patients we calculated “renin secretion rates” of separate kidneys. The limit of this method and its practical value are discussed. Summarizing the data of many authors published during the last nine years, we try to find rules how to turn to account different constellations of SNF, PVRC and NVRC when one doubts whether a patient should be operated upon. We, further report on the history of a young female patient who was cured by operation from acute renovascular hypertension and on the orthostatic behaviour of PVRC in 3 patients with NAS before and after operation.
    Notes: Zusammenfassung Bei 16 Hypertonikern mit einseitigen, bei 3 Patienten mit doppelseitigen Nierenarterienstenosen (NAS) und bei 3 Patienten mit einseitig betonten Nierenparenchymerkrankungen wurden die Reninkonzentration im peripheren Venenblut (PVRC) bestimmt und seitengetrennte Nierenfunktionsprüfungen (SNF) durchgeführt. Bei 14 Patienten wurde außerdem die Reninkonzentration im Nierenvenenblut (NVRC) bestimmt. Von 8 Patienten mit NAS, bei denen nur die PVRC, und von 5 Patienten, bei denen PVRC und NVRC bestimmt wurden, werden die postoperativen Blutdruckwerte mitgeteilt. Nach unseren Erfahrungen ist bei Hypertonikern mit NAS eine erhöhte PVRC bei normalen Clearance-Werten der kontralateralen Niere und nach Ausschluß einer anderen Ursache des sekundären Aldosteronismus ein wertvoller Hinweis auf eine gute Prognose des postoperativen Blutdruckverhaltens, während eine normale PVRC Operationserfolge nicht ausschließt. Bei den operierten Patienten mit einer PVRC im Normbereich fanden wir keine signifikanten Seitenunterschiede der NVRC. Bei 12 Patienten wurde versucht, die „Reninsekretionsraten“ der einzelnen Nieren zu berechnen. Die methodischen Grenzen dieses Versuchs werden besprochen. Aus den bisher veröffentlichten Daten über das postoperative Blutdruckverhalten bei Hypertonikern mit NAS und mit verschiedenen Konstellationen von SNF, PVRC und NVRC wird versucht, Richtlinien für die Indikationsstellung zur Operation abzuleiten. Wir berichten ferner über die Krankengeschichte einer Patientin mit akut entstandener renovasculären Hypertonie bei NAS und über das präund postoperative Verhalten der PVRC nach Orthostase bei 3 Hypertonikern mit NAS.

    Notes: Abstract New MR-techniques for quantitative blood flow registration such as phase-mapping (a two-dimensional space-resolved technique with a time-averaged measurement of blood flow) or RACE (real-time acquisition and evaluation of blood flow in one-dimensional space projection) are available for the diagnosis of valvular heart disease. Initial results of grading aortic valve stenosis by these methods are shown in comparison to continuous wave Ultrasound-Doppler. Two groups of 15 patients were examined by RACE or phase-mapping, 12 respectively 8 of whom suffered from an aortic valve stenosis. The shape of blood flow profiles as well as grading of aortic valve stenosis show high concordance when comparing the results of MR and Doppler technique. Good reliability and practicability of the demonstrated MR-method are shown. With respect to the results of RACE and phase-mapping the development of an alternative and competing MR-method for the evaluation of valvular heart disease and shunt diagnostics seems possible.

    Description / Table of Contents: Zusammenfassung Bei einem geistig und statisch retardierten Kind mit Skaphocephalie und Hypospadie wurde eine Translokation zwischen den Chromosomen D2 und G1 festgestellt. Die gleiche Aberration und zusätzlich ein kleines akrozentrisches Extrachromosom fand sich bei der gesunden Mutter des Kindes. Daraus wird geschlossen, daß bei der Mutter eine genetisch balancierte reziproke Translokation D2/G1 vorliegt und deshalb bei dem Kind eine Defizienz beider Chromosomen vorhanden sein muß. Die Symptomatik wird mit ähnlichen Beobachtungen von Defizienzen akrozentrischer Chromosomen verglichen.
    Notes: Summary Translocation between one chromosome D2 and one G1 has been found in a child with psychomotor retardation, scaphocephalus and hypospadias. The child's healthy mother showed the same aberration and an additional small acrocentric chromosome. It is concluded that the mother has a genetically balanced reciprocal translocation D2/G1 causing deficiency of both chromosomes in the child. The symptomatology is compared with similar reports concerning acrocentric deficiencies of chromosomes.

    Description / Table of Contents: Zusammenfassung Bericht über 141 „sicher” pränatal entstandene Neuroblastome, die bei Feten, Frühgeborenen und NG bis zum 14. Lebenstag diagnostiziert wurden, darunter 3 eigene Beobachtungen. Weitere 126 „wahrscheinlich” pränatal entstandene Neuroblastome (1 eigene Beobachtung) haben mit der ersten Gruppe die Lokalisation des Primärtumors (etwa 50% Nebennierenmark, etwa 50% extraadrenales Sympathicusgewebe) und die Metastasierung (etwa zwei Drittel isolierte Lebermetastasen (Typ Pepper), etwa 10% reine Hautmetastasen (Typ Smith), nur knapp 2% reine Skeletabsiedelungen (Typ Hutchison), etwa 20% „gemischte” Metastasierung in Haut, Leber, Lymphknoten und Knochen) gemeinsam. Bei 199 Neuroblastom-Patienten im Alter von 4–12 Monaten ging der Anteil der isolierten Lebermetastasierung auf ein Drittel zurück, während die reine Absiedelung in das knochensystem auf ein Fünftel anstieg, so daß schon im 2. Lebenshalbjahr die im Kleinkindesalter als typisch anzusehende Affinität der Neuroblastommetastasen zum Skeletsystem erkennbar wird. Die Gründe hierfür sind ebenso unbekannt wie die Gründe für die deutlich bessere Prognose der Neuroblastome beim jungen Säugling. Der Erfolg der verschiedenen therapeutischen Maßnahmen führte bei den Säuglingen im 1. Trimenon zu 56% klinischer Heilungen, 14% der kinder waren noch in der „Risiko”-Periode nach Collins, so daß insgesamt 70% dieser behandelten Neuroblastom-Patienten überlebten.
    Notes: Summary Report on 141 neuroblastomas certainly originating in the prenatal phase, having been diagnosed in fetusses, prematures and newborns up the 14th day of life. 3 own observations are included. Further 126 of probable prenatal origin (1 own observation) are found to be in accordance with the first group in regard to the localisation of the primary tumor (about 50% adrenal medulla, about 50% extraadrenal sympathicustissue) of metastases about 2/3 isolated livermetastases (Type Pepper), about 10% skin-metastases (Type Smith), only about 2% isolated skeletal metastases (Type Hutchison), about 20% “mixed” metastases in skin, liver, lymph-nodes and bone. In 199 patients with neuroblastoma at the age of 4–12 months the portion of isolated liver-metastases receded to 1/3 while metastases to the bone increased to 1/5. Thus the typical affinity of metastases of neuroblastoma to the skeletal-system becomes obvious. The reasons herefore are yet unknown as are the reasons for the better prognosis of neuroblastomas in young infants. The success of various therapeutic measures within the first trimenon led 56% clinical cures. 14% of the children were still within the “risk”-period according to Collins, so that a total of 70% of treated neuroblastoma-patients survived.

    Notes: Summary A case of supratentorial haemangioblastoma is presented. Diagnostic examinations, including computerized axial tomography, were not accurate. Histological examination of the removed tissue disclosed the correct diagnosis. Supratentorial haemangioblastomas are very rare tumours. We describe a case with regard to preoperative neuroradiological investigations, computerized axial tomography, and final histological diagnosis.

    Notes: Background  Biological research suggests that vascular changes may play a major role in rosacea pathogenesis. Chrysanthellum indicum is a plant-based extract containing a unique combination of phenylpropenoic acids, flavonoids and saponosids, and has a well-documented effect on vascular wall permeability and increase of the mechanical resistance of capillaries.Objective  To determine the efficacy and safety of a cream containing 1%C. indicum extract with vitamin P properties in the treatment of rosacea.Methods  This study included 246 patients diagnosed clinically as having moderate rosacea. Patients were randomly allocated to C. indicum extract-based cream (n = 125) and placebo (n = 121) groups. Patients were advised to apply the products on their face twice a day for a 12-week period. The patients were examined at the end of each 4-week period. Severity of erythema (graded by reference to six photographs), surface of erythema and rosacea overall severity scores were recorded at each visit on days 0, 28, 56 and 84. Investigators carried out a final efficacy assessment at the end of week 12. Volunteers’ final overall efficacy assessment was recorded in a self-administered questionnaire. Adverse events were identified through examination, interview and collection of comments in patients’ questionnaires.Results  Treatment with the C. indicum extract-based cream resulted in significant improvement (P 〈 0.05) in severity of erythema, overall rosacea severity compared to baseline and placebo, and investigator and patient overall efficacy assessment scores (P = 0.046 and P = 0.001, respectively) compared with placebo scores. Adverse reactions were mild, and did not differ between the C. indicum extract-based cream and the placebo groups.Conclusion  Chrysanthellum indicum extract-based cream is an effective and well-tolerated topical agent for the treatment of moderate rosacea. The mode of action of the active ingredient suggests that additional efficacy might be expected from combination with other topical treatments.
