    Description / Table of Contents: Abstract. Multicentric reticulohistiocytosis (MRH) is a rare disorder of skin and joints, and its aetiology is unknown. We describe a male patient who had first developed osteoblastic lesions of the skeleton in 1976 at the age of 59 years. Three years later, he presented with multiple papular and nodular skin lesions, predominantly on the limbs and upper trunk, and 4 years later he developed painful arthropathy. In 1988 skin examination showed multiple brownish-red papulo-nodules, some of which, at mechanically stressed areas, were ulcerated. The patient complained of pronounced arthrotic disorders with reduced joint mobility. Examination of biopsy specimens from active skin lesions demonstrated extensive numbers of macrophages with partial confluence to giant cells with typical ground-glass cytoplasm. Electron microscopy revealed phagocytosis of collagen-like structures. Examination of bone marrow biopsy specimens revealed diffuse infiltration by histiocytic cells. The diganosis made was therefore MRH. The patient was treated for several years with different therapeutic regimens, including azathioprin, dapsone, prednisolon, chlorambucil and cyclophosphamide, and complete remission of skin lesions and of bone marrow infiltration was observed. However, a myelodysplastic syndrome with refractory anaemia and ring sideroblasts developed, which is generally understood to be a preleukaemic condition. Myelodysplastic syndromes commonly develop between 3 and 10 years after the start of a therapy with antineoplastic agents.
    Notes: Zusammenfassung. Die multizentrische Retikulohistiozytose (MRH) ist eine seltene, ätiologisch ungeklärte Erkrankung der Haut und des Skeletts mit Bevorzugung der Gelenke. Bei dem vorgestellten männlichen Patienten fanden sich 1976 im Alter von 59 Jahren erstmalig osteoblastenaktive Verdichtungen im Skelett. Drei Jahre später entwickelten sich multiple Papeln und Knoten an den Extremitäten, und nach 4 Jahren traten Gelenk- und Knochenschmerzen auf. Bei der Erstvorstellung 1988 fanden sich multiple, livide und z. T. bräunlich pigmentierte Knoten an der Haut, die an mechanisch belasteten Arealen exulzeriert waren. Weiterhin fielen erhebliche arthrotische Veränderungen mit Bewegungseinschränkungen der großen Gelenke auf. Der histologische Befund der Haut zeigte Makrophagen, z. T. zu Riesenzellen konfluierend mit typischem glasigem Zytoplasma. Elektronenmikroskopisch war Kollagenphagozytose nachweisbar. Somit konnte die Diagnose einer MRH gestellt werden. Im Knochenmark fand sich eine diffuse Vermehrung histiozytärer Zellen. Über mehrere Jahre wurde der Patient in unterschiedlichen Therapieprotokollen mit Azathioprin, Dapson, Prednisolon, Chlorambucil und Cyclophosphamid behandelt. Darunter kam es zu einer vollständigen Rückbildung der Tumoren der Haut. Auch Knochenmarksinfiltrate waren nicht mehr nachweisbar, dafür trat jetzt eine refraktäre Anämie mit Ringsideroblasten als Ausdruck eines myelodysplastischen Syndroms (MDS) auf, das als Präleukämie aufgefaßt wird. Myelodysplastische Syndrome entwickeln sich häufig 3 – 10 Jahre nach Chemotherapie.

    Notes: Thiuram mix is tested in the standard series at a test concentration of 1% pet. The single thiurams (DPTD, TMTD, TMTM, TETD), however, are usually tested at 0.25% pet. in Germany. In other countries, the individual components of thiuram mix are tested at 1% pet. The German Contact Dermatitis Research Group (DKG) compared both patch test concentrations in 530 patients in order to find out if (i) a significant number of positive patch tests are missed by testing at the lower concentration, (ii) problems with irritant test reactions occur by increasing the test concentration to 1%, and (iii) the sensitivity of the thiuram mix rises when the breakdown test is done with the higher concentration. Slightly more positive reactions were seen with the higher concentration, but this increase did not reach statistical significance. The reaction index, as a measure for the relation of positive to irritant and/or questionable reactions, remained unchanged for the individual thiurams. The sensitivity of the mix also did not change when the breakdown test was performed with 1% pet. instead of 0.25% pet. Thus, we conclude that both concentrations are of equal diagnostic value in patch testing.

    Notes: Background: We describe a 20-year old woman with retarded healing after a melanoma excision. After short time application of several creams containing antiseptic, antibiotic and anti-inflammatory ingredients she developed a severe contact dermatitis. Patch testing revealed sensitizations against corticosteroids, an antibiotic and quinoline.Conclusion: In our case multiple relevant contact sensitizations appeared within a short period of two months after excision of a malignant tumour.

    Notes: In-vitro and in-vivo data suggest the efficacy of various skin protection measures in the prevention of hand dermatitis (HD). The aims and objectives of this intervention study was to quantify the efficacy of skin protection (SP) measures and UVB hardening in the prevention of HD. SP measures were compared against UVB hardening in a controlled clinical trial of 94 apprentices. The apprentices were assigned to the intervention arms class-wise. Baker apprentices involved in a previous follow-up study served as an additional historic control group representing no intervention. The apprentices were interviewed and examined in a standardised way at the beginning of the vocational training and at four follow-ups. Transepidermal water loss (TEWL) was measured at the back of the hands. Point prevalence of HD in the groups after 6 months was highest in the controls (C)(29.1%) followed by the UVB-group (19.4%) and the SP group (13.3%). UVB hardening and SP measures were associated with a reduction in HD prevalence by 9.7%(95%CI −8.5 to 28.1) and 15.8%(95%CI −2.4 to 33.9), respectively. Unadjusted logistic regression analysis showed atopy (OR = 4.74, 95%CI 1.12–20.11, p = 0.035) and skin care (OR 0.39, 95%CI 0.16–0.86, p = 0.023) to be independent risk factors. Application of SP measures reduced the OR for HD 0.8 (95%CI 0.17–3.70) and 0.33 (95%CI 0.09–1.23) fold compared to the UVB and C group, respectively. The trends in the clinical results were confirmed by statistically significant differences in TEWL values. Apprentices in the SP group consistently had a lower prevalence of HD compared to the apprentices in the UVB and C group. These trends were reinforced by statistically significant differences in TEWL levels. A multi-centre trial is recommended to confirm the efficacy of SP measures in a larger randomised study.

    Notes: Skin protection measures − barrier creams, protective gloves – and skin care are widely recommended for the prevention of occupational hand dermatitis (HD) in skin risk professions, but there is hardly anything known about uptake levels of the measures. The objective of this controlled intervention study was to quantify the uptake and maintenance of skin protection and skin care measures in first-year bakers' apprentices. A total of 94 first-year bakers' apprentices were included in the study in September 2000. The apprentices were assigned to the skin protection and control group class-wise to reduce contamination. The skin protection group comprised 39 apprentices who were trained in skin protection measures at the beginning and after 4 weeks of training. 55 apprentices were assigned to the control group representing no skin protection intervention. Standardized interviews took place at the beginning of the training and at 4 monthly follow-ups (FU). The uptake of skin protection measures differed significantly between the groups (barrier cream p 〈 0.0001, protective gloves p = 0.046, skin care p = 0.025). Barrier cream use in the skin protection group was incorporated in the daily routine very well from the start and reached 100% at the end of the examination period (4th FU). At this time, only 3.2% of the controls used barrier creams. The level of acceptance of protective gloves (4th FU: skin protection group 43.3%; controls 32.3%) was considerably lower than that of barrier creams. The initial level of regular skin care was high in both groups (skin protection group 67.6%, controls 61.7%). After the intervention the acceptance of skin care rose to 88.9% in the skin protection group compared to 68.1% in the controls (4th FU). The present study has shown that skin protection and skin care measures can be introduced successfully in the daily routine of a skin risk occupation and high uptake and maintenance rates can be achieved.

    Description / Table of Contents: Zusammenfassung Die enantioselektive Analyse zur Differenzierung zwischen natürlichen und naturidentischen Aromastoffen setzt voraus, daß die Enantiomeren racemisierungsfrei getrennt und quantifiziert werden können. Es wird gezeigt, daß an Heptakis (2,6-di-O-isopentyl-3-O-allyl)-β-cyclodextrin die chirospezifische CGC-Analyse von 2,5-Dimethyl-4-hydroxy-3[2H]-furanon (Furaneol,1) und dessen Methoxyderivat (Mesifuran,2) racemisierungsfrei durchgeführt werden kann. Aus Erdbeeren, Ananas, Trauben und Weinen werden1 bzw.2 als Racemate nachgewiesen. Die chirospezifische Analyse ist deshalb zur herkunftsspezifischen Beurteilung von1 bzw.2 nicht geeignet. Andererseits kann 4,5-Dimethyl-3-hydroxy-2[5H]-furanon (Sotolon,3) mit extremer Auflösung (R Ŝ=5,8) in seine optischen Antipoden getrennt werden. Daraus ergeben sich neue Möglichkeiten zur Qualitätsbeurteilung von Botrytis-Weinen und Sherries.
    Notes: Summary The chirospecific CGC analysis of 2,5-dimethyl-4-hydroxy-3[2H]-furanone and 2,5-dimethyl-4-methoxy-3[2H]-furanone has been carried out without any racemization, using heptakis (2,6-di-O-isopentyl-3-O-allyl)-β-cyclodextrin as the chiral stationary phase. Both compounds were detected as racemates in strawberries, pineapples, grapes and wines therefore, chirospecific analysis cannot be applied to assign their origin. On the other hand, 4,5-dimethyl-3-hydroxy-2[5H]-furanone (sotolone) was separated into its mirror images by extreme chiral resolution (R Ŝ=5.8), suggesting a new method to evaluate the quality of wines and sherries made from grapes affected by botrytis.

    Notes: Summary Resting cells of Rhodococcus sp. ATCC 39484 were immobilized in polyvinyl alcohol (PVA). The PVA-beads showed nitrile-hydrolysing activity with propionitrile as a model substrate. Drying of the beads resulted in a loss of weight of 85 %. Re-swollen beads showed no loss in activity. In a continuously-operated fixed-bed bioreactor, beads were able to convert substrate concentrations up to 100 mM.

    Notes: Summary The effects of human interferon gamma (IFN gamma) encapsulated into liposomes were investigated in vitro. Monocytes were induced to release a cytotoxic factor with either IFN gamma encapsulated into liposomes, free IFN gamma or lipopolysaccharide (LPS). If IFN gamma was applied in the liposomal form, less IFN activity was required to stimulate monocytes. Most of the cytotoxic factor was secreted during the first 4 h of stimulation. The cytotoxic factor in supernatants from PMNLs was completely neutralized by a monospecific polyclonal antiserum to tumor necrosis factor (TNF). Combining subthreshold doses of IFN gamma liposomes or IFN gamma with lipopolysaccharide synergistically enhanced the release of TNF. In fluorescence analysis, altered expression of the class II HLA-DR antigen on LeuM3 positive monocytes was induced with IFN gamma liposomes as well as with IFN gamma. Not only monocytes but also natural killer (NK) cells were stimulated to higher cytotoxicity by IFN gamma liposomes in a dose-dependent manner. In comparison with IFN gamma, the same amount of activity was necessary for adequate stimulation of NK-cells against the K562 target cells. Furthermore, the antiproliferative effects of IFN gamma liposomes and free IFN gamma on several human tumor cell lines was compared. Among several cell lines tested, U937 and A549 turned out to be sensitive to IFN gamma, and both cell lines reacted with 50% growth inhibition at a lower amount of gamma presented by liposomes than in the free form. These data show production of IFN gamma liposomes which possess immunmodulatory and antiproliferative activity in vitro. In several of the test systems studied, liposome-encapsulated IFN gamma was more effective than free IFN gamma.
