    Notes: Summary An immunoradiometric assay for human growth hormone (HGH) has been developed which has a detection limit of 1ng/l and can measure HGH in unextracted urine from normal children and adults. The assay is based on a two-step procedure, using a solid-phase goat-anti-HGH immunosorbent for immunoextraction and [125I]-labeled monoclonal HGH-antibody for detection and quantification. The assay is not affected by urea, NaCl or changes of pH from 5–8. The mean urine HGH concentration in normal children is 6.78±7.6 (SD) pg/ml, in patients with HGH-deficiency 1.3±0.9 pg/ml which increases to 11.7±13.4 pg/ml on the day of growth hormone injection.

    Notes: Abstract Studies on the combined effects of a variety of lipoxygenase inhibitors and cyclo-oxygenase inhibitors revealed strong evidence for the existence of supplementary binding sites on lipoxygenases which modify the reactions of inhibitors with the catalytic site of the enzyme. Independent of their low or non-existent inhibitory reaction at the catalytic site, compounds which interact more effectively with this putative supplementary site are capable of blunting the inhibitory efficacy of potent lipoxygenase inhibitors. Although the degree of interaction with the catalytic site determines the potency of inhibitors, an additional reaction at the supplementary site is also obligatory for inhibitory efficacy. We found that potent lipoxygenase inhibitors possess high affinities for both sites, whereas weak inhibitors and suitable cyclo-oxygenase inhibitors interact predominantly with the supplementary site on the lipoxygenase and possess low or negligible affinities for its catalytic site.

    Notes: Summary In a 19-year-old man who suffered from an athetoid form of cerebral palsy, a status marmoratus was found in both putamina at autopsy, as well as numerous intraneuronal pleomorphic PAS-positive deposits within the outer segment of the pallidum. The histological features of the deposits corresponded to those known as Bielschowsky bodies. Their histochemical properties were consistent with the presence of glucose polymers (polyglucosans). Electron microscopy revealed the presence of ovoid or cylindrical fibrillary aggregates located in perikarya and dendrites, containing spotty electron-dense material in their centre. With the help of tilting stage and stereopairs the fibrils were shown to consist of undulating and irregularly twisted ribbon-like structures measuring about 4×10 nm in cross section. No inclusions were seen in myclinated axons. Golgi-impregnated neurons of the lateral pallidum disclosed bizzare shapes, shortening, and rarefaction of the cell processes as well as focal dilatations of the dendrites, probably corresponding to the inclusions shown by routine histology. Bielschowsky bodies are discussed in relation to Lafora and Lafora-like bodies and are considered to be a unique form of intraneuronal polyglucosan deposits. They are likely to represent an unspecific and topistically limited expression of a disordered metabolism of glycogen, a status marmoratus being one of the possible predisposing factors in their formation. Their functional role is unknown.

    Description / Table of Contents: Summary 1. The excretion of free and conjugated 17-OHCS in 11 male and 10 female volunteers during a fasting period of 9–14 days and also following 7–14 days of refeeding was investigated. In 6 patients at the end of the fasting period 50 g glucose were administered and 17-OHCS again determined. 2. Complete starvation leads generally to a significant decrease of steroid excretion in urine, but certain discrepancies in the behaviour pattern of free and conjugated steroids may be seen. These differences appear also in the course of the increase of steroid excretion during refeeding. 3. The oral glucose load had no effect on the steroid content of the urine. 4. These findings may be explained by a decrease in the functional capacity of the adrenal cortex and changes in the steroid metabolism of the liver.
    Notes: Zusammenfassung 1. Bei 11 männlichen und 10 weiblichen freiwilligen Probanden wurde die Ausscheidung an freien und konjugierten 17-OHCS vor, während und 7–14 Tage nach definiertem Fasten untersucht. Bei 6 Patienten wurde zum Höhepunkt der Fastenperiode eine Glucosebelastung durchgeführt. 2. Nahrungskarenz führt zu einer eindeutigen Abnahme der Steroidausscheidung im Urin. Dabei findet sich ein divergentes Verhalten zwischen den freien und konjugierten 17-OHCS. Auch in der Auffütterungsperiode ist der Wiederanstieg der Steroidausscheidung zwischen diesen beiden Metabolitgruppen deutlich unterschiedlich. 3. Die Verabfolgung von 50 g Glucose per oral bleibt ohne Einfluß auf die Steroidgehalte im Urin. 4. Zur Erklärung der Befunde werden eine Funktionsminderung der NNR und Besonderheiten des Steroidabbaus in der Leber diskutiert.

    Description / Table of Contents: Zusammenfassung Bei 31 adipösen Kindern wurde die Wachstumshormon- und Kortisol-Reaktion auf Insulin-induzierte Hypoglykämie untersucht und mit den Resultaten bei normalgewichtigen Kindern verglichen. Der bei der Adipositas bekannte Befund einer verminderten Wachstumshormonreaktion wurde bei 9 Patienten bestätigt. Bei 13 Testen wurde ein auffälliger, abnormer Verlauf der Plasma-Kortisolkonzentration festgestellt: Der initiale Kortisolwert liegt signifikant über dem Mittelwert der Kontrollgruppe. In der Folge fehlt bei dieser Gruppe adipöser Kinder jede Kortisolreaktion auf die Hypoglykämie. Die Befunde weisen auf eine Dysregulation des Hypothalamus-Hypophysen-Nebennierenrinden Systems bei einer Gruppe adipöser Kinder hin. Es besteht keine Relation zwischen abnormer endokriner Reaktion und Dauer oder Grad der Adipositas. Die mögliche pathophysiologische Bedeutung der abnormen Regulation wird diskutiert.
    Notes: Summary Growth hormone and cortisol response to insulin induced hypoglycemia has been tested in 31 obese children. In addition to an impaired growth hormone response in 9 children, a striking pattern of cortisol was found in 13 tests. The finding indicates a dysregulation of the hypothalamo-pituitary adrenal system in some obese children. There was no relation between abnormal endocrine response and duration or degree of obesity. The observation emphasizes the difficulties of interpreting the endocrine response to insulin-induced hypoglycaemia.
