    Description / Table of Contents: Zusammenfassung 8 Patienten mit Niereninsuffizienz unterschiedlichen Grades (CKreatinin: 0 bis 61,2 ml/min/1,73 m2 Körperoberfläche) und 6 gesunde Probanden erhielten eine einmalige Dosis von 0,75 mg Methylproscillaridin intravenös verabreicht. Die Glykosidplasmakonzentrationen wurden über einen Zeitraum von 58 Std mit einem86Rb-Erythrocyten-Assay gemessen. Die mittleren Eliminationshalbwertzeiten betrugen: 29,4±3,0 Std bei den Niereninsuffizienten und 27,3±4,3 Std bei den Gesunden. Die Eliminationskonstante des Methylproscillaridin zeigte eine schwache (r=0,33), insignifikante (P〉0,1) Korrelation zur CKreat. bei geringfügiger Änderung in einem Bereich von 0 bis 150 ml/min/1,73 m2. Für praktische Belange kann man davon ausgehen, daß die Halbwertzeit des Methylproscillaridin von der Nierenfunktion unabhängig ist.
    Notes: Summary Eight patients with renal insufficiency (creatinine clearance: 0 to 61.2 ml/min/1.73 m2) and six healthy subjects received a single dose of 0.75 mg of methyl proscillaridin intravenously. Glycoside plasma concentrations were measured for a period of 58 hours using a86Rb-erythrocyteassay. Mean half-life times were 29.4±3.0 hours for the patients with renal failure and 27.4±4.3 hours for the healthy subjects. The elimination constants showed some correlation (r=0.33,P〉0.1) creatinine clearance. The changes of the elimination constants were only small in the range from 0 to 150 ml/min/1.73 m2 for the creatinine clearance. For practical use half-life of methyl proscillaridin can be assumed independent of kidney function.

    Description / Table of Contents: Summary Serum digoxin concentration and half-life were radioimmunologically determined in 9 mature newborns after 7 days medication with digoxin. The newborns were in respiratory distress treated with continuous positive airway pressure or were suspected to have serious congenital heart disease. Loading dose was 26 µg/kg body weight intravenously and 35 µg/kg body weight orally, respectively. Maintenance dose corresponded to 1/8th of the digitalization dose twice daily. The serum digoxin level 12 h after the last dose varied between 1.4 and 2.5 ng/ml (mean 2.0 ng/ml, Sx=0.4). The serum half-life of digoxin varied between 21.7 and 42.4 h (mean 30.0 h, Sx=7.7). The mean serum half-life of digoxin of 30 h attained values found in adults without renal disease. This suggests that the serum digoxin levels of newborns which are usually higher if compared with those of adults result from higher digoxin doses per unit body weight and not from diminished digoxin elimination.
    Notes: Zusammenfassung Bei 9 reifen Neugeborenen wurden nach 7tägiger Digoxin-Behandlung Serum-Spiegel und Halbwertszeit von Digoxin radioimmunologisch ermittelt. Die Behandlung erfolgte wegen eines Atemnotsyndroms, das mit kontinuierlich positivem Atemwegsdruck behandelt wurde, oder wegen Verdachts auf schwerwiegenden angeborenen Herzfehler. Die Sättigungsdosis betrug 26 µg/kg bei intravenöser bzw. 35 µg/kg bei oraler Applikation. Die Erhaltungsdosis betrug zweimal täglich 1/8 der Aufsättigungsdosis. Die Digoxin-Spiegel 12 h nach der letzten Dosis variierten zwischen 1.4 und 2.5 ng/ml (Mittelwert 2.0 ng/ml, Sx=0.4), die Halbwertszeit zwischen 21.7 und 42.4 h (Mittelwert 30.0 h, Sx=7.7). Die mittlere Halbwertszeit von 30 h liegt in einer Größenordnung, wie sie bei nierengesunden Erwachsenen gefunden wird. Man kann daher vermuten, daß die im Vergleich zum Erwachsenen gewöhnlich höheren Serum-Digoxin-Spiegel des Neugeborenen Folge einer höheren Digoxindosis pro kg Körpergewicht und nicht einer verminderten Elimination sind.

    Description / Table of Contents: Zusammenfassung In einer Doppelblindstudie wurde bei je 7 Patienten mit Coronarinsuffizienz in randomisierter Reihenfolge entweder 1 mg Methylproscillaridin oder Placebo injiziert. Druck und maximale Druckanstiegsgeschwindigkeit wurden mit dem Katheter-Tip-Manometer vor und im Mittel 62 min nach Injektion im linken Ventrikel gemessen. Zwischen den beiden Messungen wurde eine Coronarangiographie mit Ventrikulographie durchgeführt. MP steigertdp/dt max gegenüber Placebo (p〈0,05). Der linksventrikuläre enddiastolische Druck wurde im Mittel in der Placebogruppe von 12 auf 13,7 mmHg erhöht, in der Glykosidgruppe von 16,1 auf 13,6 mm Hg gesenkt. Aufgrund der Ergebnisse dieser Untersuchung kann die positiv inotrope Wirkung des Herzglykosids am Menschen angenommen werden.
    Notes: Summary In a randomized, controlled, double-blind study 2 groups of 7 patients each with coronary heart disease received either 1 mg methylproscillaridin (MP) a cardiac glycoside of the squill intravenously, or placebo to test the inotropic effect of MP. Pressures anddp/dt max were measured in the left ventricle before and after coronary angiography after an average of 62 min (60 to 70 min). There was a significant (p〈0.05) increase ofdp/dt max in the MP-group (Wilcoxon-Mann-Whitney test). Left ventricular enddiastolic pressure was decreased from 16.1 to 13.6 mm Hg in the MP-group, and increased in the placebo group from 12 to 13.7 mm Hg (mean values). Thus, a positive inotropic effect of the glycoside may be assumed.

    Description / Table of Contents: Zusammenfassung Bei 9 Patienten mit schwerer Niereninsuffizienz wurde über einen Zeitraum von 7 Tagen 1,5 mg Proscillardin (P.) täglich oral verabreicht. Während dieser Periode, sowie 4 Tage nach Absetzen des Glykosids wurden die P.-Plasmaspiegel mit einer modifizierten86Rb-Erythrozyten Methode (untere Nachweisgrenze für P.∼40 pg/ml) gemessen. Die Patienten erreichten nach 2–3 Tagen ein Plateau der Plasmaspiegel mit einer medianen Konzentration von 235 pg/ml. Nach Absetzen des Medikamentes fielen die P.-Plasmaspiegel mit einer medianen Halbwertzeit von 40,6 Std ab. Bei gleicher Versuchsanordnung kumulierten 7 gesunde Versuchspersonen gleichfalls nach 2–3 Tagen zu einem steady state Plateau der Plasmaspiegel (mediane Konzentration=350 pg/ml) und wiesen nach Beendigung der P. Zufuhr eine mediane Plasmahalbwertzeit von 47 Std auf. Die Untersuchungen lassen den Schluß zu, daß P. unabhängig von der Nierenfunktion aus dem menschlichen Organismus eliminiert werden kann. Diese Aussage wird durch 3 Fakten gestützt: 1 P. kumuliert bei Niereninsuffizienten und Nierengesunden im gleichen Zeitraum zu einem steady state Plateau der Plasmaspiegel. 2 Die Höhe dieses Plateaus unterscheidet sich nicht signifikant in den beiden Kollektiven. 3 Nach Beendigung der P. Gabe fallen die Plasmaspiegel bei Niereninsuffizienten ebenso schnell ab wie bei Gesunden.
    Notes: Summary Nine patients with severe renal insufficiency were treated with a daily oral dose of 1.5 mg of proscillaridin (p.) over a period of 7 days. During this period as well as 4 days following the last medication p. plasma levels were determined using a modified86Rb-erythrocyte method (sensitivity for p.∼40 pg/ml). Plasma levels reached a plateau after 2 to 3 days with a median p. concentration of 235 pg/ml in renal insufficiency. After the last application plasma levels declined with a median half life time of 40.6 hours. Using identical experimental conditions seven healthy volunteers accumulated within a period of 2 to 3 days to a steady state plateau of p. plasma levels with a median concentration of 350 pg/ml and showed a half life time of 47 hours. These experiments indicate that p. may be eliminated independently cf renal function. This conclusion is based on three observations: 1 The time necessary to obtain a steady state plateau of p. plasma levels is equal in normals and in patients with renal insufficiency. 2 The level of this plateau is not significantly different in either group. 3 The plasma half life time of p. is equal in both groups

    Description / Table of Contents: Summary The aim of this study was to determine the half-life of β-acetyl digoxin in plasma after cessation of its continuous oral application. 18 healthy male volunteers received 0.4 mg p.o. of β-acetyl digoxin daily for a period of 14 days. Plasma glycoside concentrations were measured up to 108 hours after the last application utilizing a86Rb-erythrocyte assay. Mean plasma digoxin concentration 12 hours after the last dose wasx=1.05 ng/ml,s x=0.301. Mean half-life in plasma was 55.8 hours (s x=21.7). This corresponds to a mean daily elimination rate of about 25.8%. These results are in good agreement with the 20% loss of effect on the heart as reported in the literature.

Die Glykosidplasmaspiegel wurden von 12–108 Std nach der letzten Glykosidgabe mit einem86Rb-Erythrocyten-Assay gemessen. Die mittlere Plasmaglykosidkonzentration betrug 12 Std nach der letzten Einnahme:x=1,05 ng/ml,s x=0,301. Die mittlere Plasmahalbwertzeit betrugx=55,8 Std (s x=21,7).     Description / Table of Contents: Zusammenfassung Unter zusätzlicher Gabe von Chinidin steigt die Digoxinkonzentration im Serum (DKS) deutlich an. Die Bedeutung dieses Anstieges in bezug auf die kardiale Wirksamkeit der Glykoside ist umstritten. Wir untersuchten mit Hilfe des86Rb-Erythrozyten-Assay, ob die Zugabe von Chinidin den Digoxineffekt auf die Na-K-ATPase (den sogenannten Glykosidrezeptor) beeinflußt und damit auch den kardialen Effekt von Digoxin. Die Hemmwirkung von Digoxin (in Konzentrationen von 0–150 ng/ml) auf die86Rb-Aufnahme wurde ohne und nach Zugabe von Chinidin in therapeutischen und höheren Konzentrationen (0–60 µg/ml) gemessen. Der Zusatz von Chinidin führte jedoch zu keiner Änderung des Digoxineffektes auf die Na-K-ATPase-Aktivität. Zusammen mit unseren klinischen Beobachtungen sprechen diese Ergebnisse dafür, daß die erhöhten DKS-Werte unter gleichzeitiger Chinidinmedikation auch kardial wirksam sind.
    Notes: Summary Co-administration of quinidine results in a marked increase in serum digoxin concentration (SDC). The implication of this increase in SDC in regard to an increased digoxin effect on the heart is controversal. The86Rb-erythrocyte-assay represents a perfect model to study whether quinidine interferes with digoxin at the Na-K-ATPase (the socalled glycoside receptor) and thus presumably with the inotropic effect of the glycoside. The inhibitory effect of digoxin (0–150 ng/ml) on the86Rb-uptake was measured in the absence and presence of quinidine in therapeutic or higher concentrations (0–60 µg/ml). Addition of quinidine produced no effect on digoxin-induced inhibition of Na-K-ATPase activity. Together with our clinical observations these results strongly suggest that the increased SDC during concomitant quinidine therapy actually reflects an increased digoxin effect on the heart.

Das Ergebnis läßt auf einen enterohepatischen Kreislauf des Methylproscillaridin schließen. Im Falle einer Glykosidintoxikation erscheint bei diesem Glykosid ein Behandlungsversuch mit Aktivkohle erfolgversprechend.

    Notes: Summary An inter-individual, randomized, double-blind study of digitoxin (Dt) and β-acetyl digoxin (D) was performed in 120 healthy male volunteers. Groups of 10 persons each received orally D 0, 0.1, 0.2, 0.3, 0.4, 0.5 or 0.6 mg and Dt 0.04, 0.08, 0.12, or 0.16 mg daily for 7 days; Loading doses were given for the first three days. Plasma levels were measured with an86Rb-erythrocyte assay 24 h after the last dose. ECG, carotid artery pulse and phonocardiogram were recorded prior to (b) and 24 h after (a) the last dose. QTc, amplitude of T-waves in V2 to V6, electromechanical systole (QS2c) and left ventricular ejection time (LVETc) were measured. The differences between a and b (Δ-values) reflect glycoside-induced changes in heart function. The plasma glycoside concentrations depended on dose and ranged from 0 to 2.4 ng/ml for D and from 0 to 42 ng/ml for Dt. QTc, QS2c, and LVETc were significantly shortened by the glycosides and typical parallel, sigmoid, log dose-response curves were obtained for the Δ-values. Dt was 3.8-times as potent as D in diminishing these parameters. The maximal effect of the two glycosides was almost identical at the highest doses: Δ-QTc=−45 ms, Δ-QS2c=−25 ms, Δ-LVETc=−12.5 ms. The latter two parameters showed a plateau of maximum efficacy. Both glycosides caused significant flattening of T-waves, Dt being 7.2-times as potent as D. Significant relationships between plasma concentration and cardiac effects were observed (p〈0.001)−Δ-QTc (D: r=0.7; Dt: r=0.77), Δ-QS2c (D: r=0.7; Dt: r=0.75), and Δ-LVETc (D: r=0.46; Dt: r=0.43); D correlated less well than Dt with the flattening of T (r=0.46; r=0.76, respectively). The most important conclusions were that: Dt was about 4-times as potent as D in influencing cardiac performance; the effects of D and Dt on systolic time intervals reached a plateau at “therapeutic” doses; Dt induced more pronounced flattening of the T-wave than D; and plasma glycoside levels within the “therapeutic” range corresponded to observed effects on the heart.
