    Notes: We demonstrate experimentally that a phase-sensitive optical parametric amplifier used to amplify the incoming optical signal on a lossy photodiode improves the noise figure of detection. At high parametric gains this noise figure tends to 0 dB corresponding to a detection quantum efficiency approaching 100%. This result contrasts with the performance of preamplification by a classical laserlike amplifier in which the noise figure tends to 3 dB giving a saturation of the detection quantum efficiency at 50%. © 1995 American Institute of Physics.

    Description / Table of Contents: Résumé L'évaluation de la chirurgie des fléchisseurs est souvent liée à l'utilisation de plusieurs classifications étudiant la mobilité angulaire active et passive des articulations des différents doigts. A partir de 34 patients opérés pour des plaies franches entre 1983 et 1990 au centre d'orthopédie et de traumatologie de la C.R.A.M.A.M. de Strasbourg, nous avons pu étudier 39 doigts, nous intéressant plus particulièrement à la répercussion professionnelle et sur les activités de loisirs des anciennes plaies de fléchisseurs (parfois accompagnées de lésions des pédicules vasculo-nerveux ou de fractures simples) et les comparant aux résultats angulaires classés selon Buck-Gramcko et à l'étude de la force (Pinch et Grasp). Si les résultats observés sont comparables aux différentes séries de la littérature (77 % de bons résultats) l'étude de la gêne fait apparaître pour les doigts longs une gêne dans 50 % des cas non corrélée à des résultats angulaires trouvés bons (diminution de la force). Par contre au niveau du pouce de médiocres résultats angulaires ne sont pas gênants pour les patients dans 30 % des cas (importance de l'articulation trapézo-métacarpienne). Ainsi nous pensons que les résultats de suture des fléchisseurs doivent prendre en compte en plus des résultats angulaires, l'étude de la force et la gêne du patient dans ses activités de tous les jours afin de corriger des résultats à tort optimistes ou pessimistes.
    Notes: Abstract The evaluation of the surgical management of the flexor tendons is often related to the use of several classifications studying the active and passive angular mobility of the different fingers. From 70 patients operated on from 1983 to 1990 at the Traumatology Center of the C.R.A.M.A.M. in Strasbourg (France), we were able to study 34 patients with clean wounds (corresponding to 39 fingers), being most particularly interested in the professional and leisure activities with old wounds of flexor tendons (sometimes accompanied by lesions of neuro-vascular pedicles or simple fractures) and comparing them to the angular results classified according to Buck-Gramcko and in the study of the strenght (pinch and grasp). If the results are compared with the different series in the literature (77 % of good results), the study of the hinderance shows for long fingers a hinderhance in 50 % of the cases non correlated to the angular results found to be good (diminution of the strength). But with the thumb mediocre angular results are not limiting for 30% of patients (importance of the trapeziometacarpal joint). Thus we think that the results of suture of the flexor tendons must take into account, with the angular results, the study of the strength and the limitation of the patient in his everyday activities in order to correct results which are falsely optimistic or pessimistic.

    Description / Table of Contents: Abstract The value of using biodegradable devices and particularly coral in cervical spine surgery is linked to the desire to supress the disadvantages of taking bone graft for arthrodeses (using the Robinson's technique), be it iliac, fibular or tibial. The chemical, microscopical, biomechanical and clinical study of coral since 1970 enabled us to show the value of a material whose structure resembles that of bone, degrading itself completely and being replaced by newly formed bone. Our first results in 37 cases confirm these data by studying the evolution of coral, the good clinical tolerance, its good radiological incorporation with no alteration of the static balance of the cervical spine. A literature review on the subject of the use of biodegradable devices shows the advantages and disadvantages of each explaining the reason for our choice.
    Notes: Résumé L'intérêt d'utiliser des biomatériaux et particulièrement du corail en chirurgie rachidienne cervicale est lié à la volonté de supprimer les inconvénients liés à la prise de greffe osseuse dans les arthrodèses, que cette greffe soit iliaque ou encore péronière ou tibiale. L'étude du corail depuis 1970, chimique, microscopique, biomécanique puis clinique a permis de montrer l'intérêt d'un matériau dont la structure rappelle celle de l'os, se dégradant complètement et étant remplacé par de l'os néoformé. Nos premiers résultats sur 37 cas revus confirment ces données en étudiant l'évolution du corail, sur la bonne tolérance clinique, sa bonne incorporation radiologique sans modification de la statique rachidienne. Une revue de la littérature au sujet des biomatériaux utilisés montre les avantages et les inconvénients de chacun expliquant le pourquoi de notre choix.

    Notes: Abstract.  We describe, in detail, theoretical and experimental aspects related to our recently reported repeated back-action evading measurements performed using two travelling-wave optical parametric amplifiers in series. The state of the observable being measured is almost perfectly preserved after two successive measurements. The final signal, and the intermediate measurement of the two successive setups are quantum correlated with conditional variances that lie in the quantum regime. Moreover, we show that the two independent measurements are quantum correlated up to 30%.
