    Description / Table of Contents: Summary The sulfid-silver reaction [6] can be readily utilized for the detection of ferritin iron by light microscopy [4] and thus makes possible the demonstration of specific binding of ferritin to the immunologically competent cells of spleen, lymph nodes and peripheral blood. Combining sites were only seen after immunization with apoferritin. Applying ferritin-labeled antigens [1] the method should be adaptable to the detection of specific antigenbinding by immunologically sensitized blood lymphozytes.
    Notes: Zusammenfassung Mit der Sulfid-Silber-Reaktion [6] können lichtmikroskopische Endvergrößerungen von Eisenteilchen über 1:100 000 erreicht werden [4]. Es war daher möglich eine spezifische Bindung von Ferritin an immunologisch kompetenten Zellen der Milz, von Lymphknoten und peripheren Blut mit dieser Methode nachzuweisen. Eine positive Reaktion fand sich nur nach Immunisierung mit Apoferritin. Durch Kopplung von Feritin an definierte Antigene [1] sollte es möglich sein, eine spezifische Antigenbindung an immunologisch sensibilisierten Blutlymphocyten nachzuweisen.

    Description / Table of Contents: Zusammenfassung Dem Nachweis von Antigen-Antikörperpräcipitaten im Agargel sind Grenzen gesetzt, die sich vor allem aus der Konzentration des Antigens in der Untersuchungsprobe ergeben. Geringe Antigenmengen, die bei Anwendung der üblichen Färbemethoden nicht sichtbar werden, lassen sich nur darstellen, wenn die Nachweisempfindlichkeit durch Markierung von Antikörpern gesteigert wird. Als Marker von Antikörpern wurde von uns Ferritin Verwendet, dessen Eisenanteil durch Silberanlagerung (Sulfid-Silber-Reaktion) mit einem Vergrößerungseffekt von mehr als 100 000:1 sichtbar gemacht werden kann. Die Sulfid-Silber-Reaktion ließ sich an luftgetrockneten Agargelpräparaten gut ausführen. Sie eignet sich zur Kontrolle der Ferritinbindung an Antikörper besser als alle früheren Verfahren. Sie führt gleichzeitig zu einer Steigerung der Nachweisempfindlichkeit von Präcipitaten im Agargel. Die bei Routinefärbungen gefundene untere Nachweisgrenze kann um 3–4 Verdünnungsstufen weiter gesenkt werden. Mit Hilfe der Sulfid-Silber-Reaktion lassen sich demnach Antigenmengen erfassen, die dem üblichen Nachweisverfahren entgehen.
    Notes: Summary The demonstration of antigen-antibody precipitations following agar gel diffusion depends on the concentrations of antigen or antibody in the specimen. Small quantities of antigen can only be detected if the sensitivity of the method is increased by marking the antibodies. Ferritin was used as a marker for the antibodies; the single iron molecules are enlarged 100 000 times by the sulphide-silver-reaction and thereby made visible. The sulphide-silver-reaction made it possible to stain precipitation lines in dried agar gel preparations. The method is superior compared to other published data on the analysis of ferritin-antibody conjugates. The sensitivity of the antigen determination is increased for about 3–4 titer steps of dilution. Quantities of antigen can in this way be detected which are otherwise not demonstrable by conventional staining methods.

    Description / Table of Contents: Summary In a patient, aged 16, suffering from severe hereditary haemophilia A, inhibitors against antihaemophilic globulin A occurred as a consequence of repeated applications of antihaemophilic globulin. It has been shown by clotting analysis and immunological investigations that the inhibitors were antibodies of the type of Ig G. This is the explanation for the observation that in this patient the antihaemophilic globulin A not only was ineffective but also provoced shock symptoms. The precondition for the development of antibodies in this patient would be either a lack (the non-existence) of antihaemophilic globulin A, or that the applicated antihaemophilic globulin A became antigenic by the mode of its preparation. There is only an effective therapy after exchange transfusion. In case of species specificity of the inhibitor a therapy with animal antihaemophilic globulin is indicated. It may be possible to suppress the development of antibodies by the application of immunosuppressive drugs.
    Notes: Zusammenfassung Nach wiederholten Gaben von humanem antihämophilen Globulin traten bei einem 16jährigen Patienten mit schwerer familiärer Hämophilie A Hemmkörper gegen antihämophiles Globulin A auf. Die gerinnungsanalytischen und immunologischen Untersuchungen deckten auf, daß die Hemmkörper Antikörpern vom Immunglobulin-G-Typ entsprachen. Dadurch ist erklärt, warum die Zufuhr des antihämophilen Globulin A bei dem Patienten nicht nur unwirksam war, sondern zu Schocksymptomen führte. Die Antikörperentwicklung hat zur Voraussetzung, daß bei dem Patienten entweder primär kein antihämophiles Globulin A vorhanden war, oder das zugeführte Antihämophile Globulin A durch die Zubereitungsart Antigencharakter annahm. Eine wirksame Therapie kann erst nach Austauschtransfusion eingeleitet werden. Bei Artspezifität des Hemmkörpers ist ein Therapieversuch mit tierischem antihämophilen Globulin angezeigt. Es kann versucht werden, durch Gabe immunosuppressiver Mittel die Antikörperbildung zu unterdrücken.

    Description / Table of Contents: Summary A serum was obtained by sensibilisation of rabbits with pepsin digested human gamma-globulin (Gammavenin®), which contained relatively strong precipitants against the receptors of the h-chain and l-chains (kappa and lambda) of the immunoglobulin IgG. In the immunoelectrophoretic analysis three lines in the IgG-area can thus be demonstrated, which correspond to the h- and l-polypeptid-chains of the complete IgG-molecule. It has been shown, that those antisera are dvantageous for the immunoelectrophoretic analysis of paraproteins.
    Notes: Zusammenfassung Durch Sensibilisierung von Kaninchen mit pepsinverdautem menschlichen Gammaglobulin (Gammavenin®) wurde ein Serum gewonnen, das relativ kräftige Präcipitine gegen die Receptoren der H- und der L- (Lambda- und Kappa-)Polypeptidketten des Immunglobulin IgG enthält. In der immunelektrophoretischen Analyse kommen bei Verwendung dieses Antiserums im IgG-Bereich drei Linien zur Darstellung, die den präzipitierten H- und L-Ketten des komplettenIg G-Moleküls entsprechen. Es wird demonstriert, daß die nach der beschriebenen Methode gewonnenen Antiseren für die immunelektrophoretische Analyse von Paraproteinen erhebliche Vorteile besitzen.

    Description / Table of Contents: Zusammenfassung In 29 Urinfraktionen von sechs Kindern mit hämolytisch-urämischem Syndrom (HUS) wurde in der akuten Phase der Krankheit mittels analytischer Isotachophorese eine spezifische Substanz nachgewiesen. Die Urine von sechs nierenkranken Kontrollpersonen und sieben Gesunden wiesen diese Substanz nicht auf. Es wird angenommen, daß die im Isotachogramm registrierte Substanz in direkter Beziehung zu dem Krankheitsbild des HUS steht.
    Notes: Summary In 29 urinary fractions of six children with haemolytic uraemic syndrome (HUS) a specific substance could be demonstrate during the acute phase of the disease by means of isotachophoresis. Urine tests in six patients with other kidney diseases and with uraemia and in seven healthy persons did not show this substance. From this, we assume that the substance demonstrated in the isotachogram has a direct correlation to the HUS.

    Description / Table of Contents: Zusammenfassung Seit 1966 behandeln wir 22 Patienten mit Colitis ulcerosa. Davon wurden 15 schwere Erkrankungsfälle nach genau definierten Kriterien für die immunosuppressive Therapie ausgewählt. Komplikationen durch diese Behandlung wurden in 2 Fällen beobachtet. In den übrigen 13 Fällen führte die immunosuppressive Behandlung prompt zu klinischer und rectoskopischer Remission. Die mittels Kontrasteinlauf gewonnenen Röntgenbefunde waren in 2 Fällen völlig normalisiert, in 3 weiteren Fällen erheblich gebessert und in den restlichen 10 Fällen noch nicht wieder kontrolliert. Aus Gründen der Strahlenersparnis erfolgt die Röntgenkontrolle grundsätzlich am Ende der immunosuppressiven Behandlung, also frühestens 2 Jahre nach Behandlungsbeginn. Die Nebenwirkungen bei den beiden erwähnten Fällen bestanden in cholestatischem Ikterus (Fall Nr. 7) und heftigem Erbrechen (Fall Nr. 2). Die sonst bei cytostatischer Behandlung festzustellende Wachstumshemmung und auch Alopecie haben wir selbst bei langzeitig durchgeführter Behandlung nicht gesehen. Unsere Erfahrungen mit der immunosuppressiven Therapie der Colitis ulcerosa beim Kinde zusammenfassend möchten wir als durchaus ermutigend betrachten. Ihre Durchführung gehört jedoch in die Hand eines auf diesem Gebiete Erfahrenen.
    Notes: Abstract Since 1966 we treated 22 patients with ulcerative colitis, 15 of which with severe diseases were selected for immunosuppressive therapy according to strictly defined criteria. Difficulties were observed in 2 cases. In 13 cases immunosuppressive therapy promptly resulted in clinical and rectoscopic remissions. Roentgenologic findings of colon were normalized in 2 cases, improved in 3 cases and not yet controlled in the other cases. Side effects were noted in 2 patients, they included cholestatic jaundice and vomiting. Alopecia and decreased linear growth were not observed. In summary our experience with the immunosuppressive therapy of childhood ulcerative colitis so far has been encouraging.

    Notes: Abstract Whole blood levels of Cd and Pb (μg/g Hb), serum concentrations of Sr and Zn and their urinary excretion were measured in healthy, renal transplant and dialyzed children by atomic absorption spectroscopy. Furthermore, before and after dialysis the concentrations of these elements were determined in blood/serum as well as in the dialysis fluid. The predialytic concentrations of Cd, Pb and Sr in blood/serum were significantly increased compared to controls. Haemodialysis had only minor effects on whole blood Cd and Pb levels but serum Sr and Zn concentrations increased as a result of high metal levels in dialysis fluid. Children after renal transplantation showed no alterations in Pb, Sr and Zn concentrations compared to healthy children but whole blood Cd was increased. According to the renal function the Pb, Cd and Zn urinary excretion decreased in the following order: healthy, renal transplant and dialysis children. Sr urinary excretion was increased in haemodialysis and renal transplant patients. In conclusion, we found no Zn deficiency. Elevated blood levels of Cd and Pb and elevated Sr serum concentrations in paediatric patients undergoing dialysis and increased Cd whole blood levels in renal transplant children could be of clinical interest.

    Notes: Abstract Anaemia is a frequent symptom in children with chronic uraemia. There is only limited information about committed haemopoietic stem-cell function in renal insufficiency. The ability of bone-marrow cells to form erythropoietic and granulopoietic cell colonies in vitro was therefore tested in 13 children with chronic renal insufficiency. Following separation of mononuclear cells from bone-marrow aspirates by Ficoll-Isopaque gradient centrifugation, erythropoietic precursor cells were stimulated in plasma clots by erythropoietin. Granulopoietic precursor cells were stimulated in soft-agar gel using feeder layers of normal human leukocytes. The colonies were identified by staining and counted. The number and proliferative capacity of erythropoietic precursor cells (CFU-E) did not seem to be significantly suppressed. Under in vitro conditions, the responsiveness of CFU-E to erythropoietin seemed to be normal. Addition of autologous serum resulted in different degrees of inhibition of erythroid colony formation. The inhibitory effect of the sera was also obvious when uraemic sera were added to bone-marrow cells from subjects without renal disease. Preliminary data indicate that haemodialysis is effective in reducing this inhibitory activity which may be an important factor in the pathogenesis of anaemia in chronic renal insufficiency. The numbers and proliferative capacity of granulopoietic precursor cells (CFU-C) were normal in all the children tested.

    Notes: Abstract Type 1 allergy against natural rubber latex is an increasing problem in health care workers and children with spina bifida or urogenital malformations. The aim of our study was to evaluate the prevalence of latex IgE antibodies and cross-reacting fruit antibodies in patients with spina bifida compared with atopic and non-atopic controls. Risk factors for sensitization should be determined. Sera of 148 patients with spina bifida and 98 controls (44 with atopy) were screened for IgE antibodies against latex, banana and kiwi by fluorescence enzyme immunoassay (CAP system). Atopies, allergic symptoms after latex contacts and the number of operations were compiled by a questionnaire. Patients with spina bifida developed latex IgE antibodies (≥0.7 kU/l) more frequently (40.5%) than atopic children (11.4%) or healthy controls (1.9%). All 18 symptomatic patients belonged to the spina bifida group and had high values of latex antibodies. The risk for developing latex antibodies increases with the number of operations. There was no difference in the history of atopic diseases and in a screening test of IgE antibodies against inhalative allergens between latex sensitized and not sensitized children with spina bifida. Antibodies against banana were more frequent in the latex sensitized children with spina bifida. (18.3% vs 3.4%, P = 0.002). Conclusion The high prevalence of latex antibodies in children with spina bifida justifies a primary prophylaxis by avoiding latex contacts, especially during anaesthesia and surgery, a correlation between the number of operations and the development of latex antibodies exists.
