    Description / Table of Contents: Zusammenfassung Bei Herzschrittmachern mit Epoxidharz-Kapsel eines bestimmten Typs, die längere Zeit im menschlichen Körper implantiert waren, ließen sich mit Hilfe der Immunfluoreszenz sessile menschliche Eiweißkörper im Epoxid nachweisen. Diese Proteine sind mit der beschriebenen Methode aus den Spänen des Epoxidmantels nicht extrahierbar, wie durch Paralleluntersuchungen nach 2 verschiedenen Methoden gezeigt werden konnte. Die Ergebnisse der Untersuchungen nach Lowry sind hierbei nicht verwertbar, da möglicherweise extrahierbare Träger von Aminogruppen miterfaßt werden.
    Notes: Summary On cardiac pacemakers with epoxy resin encapsulation of a certain type which had been implanted in humans for a considerable time, sessile human proteins were detected in the epoxy by means of immunofluorescence microscopy. These proteins are not extractable from shavings of the epoxy casing using the method described, as was shown by two different methods. The results of LOWRY's test on the eluate are invalid, because we assume that extractable amino groups are responsible for the positive results.

    Notes: Abstract Objective To investigate whether determination of right ventricular end-diastolic volume (RVEDV) and right ventricular ejection fraction (RVEF) can be performed with reasonable accuracy and reproducibility using a conventional slow response thermistor pulmonary artery catheter (CPAC) applying an adaptive algorithm. Design To study RVEDV and RVEF simultaneously with pulmonary artery catheters equipped with slow and fast response thermistors (FRPAC) under a broad range of cardiac output. Setting Laboratory of Institute of Experimental Surgery, Technical University. Animals 11 anaesthetised piglets. Interventions Hypovolemia (V−) was induced by withdrawal of blood up to 50 ml/kg, hypervolemia (V+) was produced by retransfusing blood and adding up to 30 ml/kg hydroxyethyl starch. In 5 animals in phases V−and V+ beta-adrenergic stimulation was achieved with dobutamine. Finally pulmonary artery hypertension was induced by infusion of small air bubbles. Measurements and results Cardiac output (CO), RVEDV and RVEF were determined simultaneously with FRPAC and CPAC placed in the same pulmonary artery branch. Measurements were repeated 8 times sequentially in steady state normovolemia. A total of 130 measurements could be analysed. The coefficient of variation was 6.7±4.2 for CO(FRPAC) and 4.6±1.7% for CO(CPAC); for RVEF it was 9.7±6.2% (FRPAC) and 9.9±3.9% (CPAC); for RVEDV it was 11.6±4.8% (FRPAC) and 8.54±3.2 (CPAC). Mean difference (bias) was 0.06±0.39 l/min for CO measured with both methods, 19±35 ml for RVEDV and −3.3±6.5% for RVEF. CO(CPAC) displayed a strong correlation to CO(FRPAC) (R=0.97,p=0.001) as well as RVEF (R for RVEF(CPAC) versus RVEF(FRPAC)=0.90,p=0.001). R for RVEDV(CPAC) versus RVEDV(FRPAC) was 0.67,p=0.001. We conclude that this animal study demonstrates good agreement between RVEF and RVEDV obtained with catheters equipped with a fast response thermistor or with a conventional slow response thermistor allowing accurate monitoring of right ventricular function with a conventional pulmonary artery catheter.

    Description / Table of Contents: Zusammenfassung Zur rechtzeitigen Therapie des Lungenödems ist die Quantifizierung des extravasalen Lungenwassers von großer Bedeutung. Es wurde ein automatischer Meßplatz entwickelt, der auf der Grundlage der Doppelindikatorverdünnungstechnik mit den Indikatoren Farbstoff und Kälte die Lungenvolumina bestimmt. Das Meßsystem berücksichtigt die Einflüsse der Injektionstechnik, des biochemischen Verhaltens der Indikatoren und der Auswertung der Verdünnungskurven auf das Ergebnis durch entsprechende apparative Einrichtungen. In einer experimentellen Anwendung ergab die Korrelation zwischen in vivo und post mortem gravimetrisch bestimmtem Lungenwasser einen Regressionskoeffizient vonr = 0.97.
    Notes: Summary The quantification of the extravascular lung water is one condition for early therapy of lung edema. On the basis of the double indicator dilution technique, a measurement set-up using the markers cold and dye was developed to determine the lung volumes. The system takes into account the effect on the results by the method of injection, the biochemical behaviour of the dye and the calculation of the dilution curves by a corresponding design of the system. In an experimental application, the correlation between in vivo and post mortem gravimetrically determined lung water yielded a regression factor ofr = 0.97.

    Description / Table of Contents: Zusammenfassung Zur extern-mechanischen Stimulierbarkeit des Herzens wurden Druckimpulse mit Impulslängen von 1 μs bis 5 ms und Amplituden von 10–140 bar, die durch Unterwasserfunken und elektromagnetisch erzeugt wurden, transthorakal auf das Herz appliziert. Damit konnte das Herz über längere Zeit hämodynamisch wirksam ventrikulär oder atrial durch Einzel- oder Mehrfachimpulse oder in Serie stimuliert werden; eine andere nicht-invasive Methode einer Vorhofstimulation ist bislang nicht bekannt. Bei AV-Block konnte die Kreislauffunktion aufrecht erhalten werden. Die von der Physiologie her bekannten Phänomene wie Refraktärität, Vulnerabilität, Unterbrechung von Reentry-Mechanismen und Reizschwelle konnten registiert werden. Die Methode hat Bedeutung für die Notfallmedizin.
    Notes: Summary For external mechanical stimulation of the heart, pressure impulses with a duration of 1 μs to 5 ms, generated by a spark discharge in a water bath were administered externally over the precordium of various animals. Effective stimulation could be maintained for prolonged periods of time and ventricular or atrial stimulation could be performed; any other noninvasive form for atrial stimulation is not known so far. We were able to maintain cardiac function after artificial AV-block. Physiological phenomena, such as refractory period, vulnerability, termination of reentry-tachycardias and stimulating threshold were seen. The method of mechanical cardiac stimulation may be of great value for emergency as well as non-invasive diagnostic procedures in man.
