Summary 8 persons (4 patients with “primary carbohydrate-induced hypertriglyceridaemia“, 4 controls) were given a carbohydrate-rich breakfast; one and a half hours later a constant intravenous infusion of glucose-U-C14 and palmitic acid-9,10-H3 over a period of 3 h was started and concomitantly a glucose solution (40 g/h) was given orally in small portions at regular intervals. The incorporation rate of plasma glucose carbon into plasma triglyceride (TG) fatty acids (FA) and into plasma-TG-glycerol was calculated from the specific activity of plasma glucose carbon and from the increase of the C14-radioactivity in plasma-TG-FA and in plasma-TG-glycerol. The esterification rate of plasma free fatty acids (FFA) into plasma-TG was estimated from the specific activity of plasma-FFA and from the increase of H3-radioactivity in plasma-TG. In 6 other persons (3 hyperlipaemics, 3 controls), who also received 40 gm glucose/h over a period of 3 h, the same datas were calculated; however, in these persons the labeled glucose was added to the orally administered glucose solution, while the labeled palmitic acid was infused intravenously. The data show that under the given test conditions the major part of the glycerol necessary for the esterification of plasma-TG was derived from plasma glucose. The incorporation rate of plasma glucose carbon into plasma-TG-FA, however, was extremely low in all cases. Therefore, even under massive glucose loading, the incorporation rate of plasma glucose carbon into plasma-TG might play a quantitatively minor role for the synthesis of plasma-TG as compared with the esterification rate of plasma-FFA into plasma-TG. Correlations could be shown between the plasma-TG concentration and the incorporation rate of plasma glucose carbon into plasma-TG-FA and into plasma-TG-glycerol resp. and between the plasma-TG concentration and the esterification rate of plasma-FFA into plasma-TG. The observed correlations, however, show that in the investigated hyperlipaemics under massive glucose loading neither the esterification rate of plasma-FFA into plasma-TG nor the incorporation rate of plasma glucose carbon into plasma-TG (e.g. the synthesis of plasma-TG) were elevated to such an extent that they could be regarded as mainly responsible for the excessively high plasma-TG level. These observations seem to support the hypothesis that in these patients an inadequate efflux of plasma-TG results in the excessive hypertriglyceridaemia.

Zusammenfassung Bei 8 Personen (4 Patienten mit „primärer kohlenhydratinduzierter Hypertriglyceridämie“ und 4 Stoffwechselgesunde), die bereits 1 1/2 Std vor Versuchsbeginn ein genau definiertes kohlenhydratreiches Frühstück zu sich genommen hatten, wurde über einen Zeitraum von 3 Std während einer oralen Zufuhr von 40 g Glucose/Std in Form einer 50% igen Lösung, die in kleinen regelmäßigen Portionen getrunken wurde, gleichzeitig Glucose-U-C14 und Palmitinsäure-9,10-H3 intravenös infundiert; aus der spezifischen Aktivität des Plasmaglucose-Kohlenstoffes bzw. der FFS des Plasmas und dem Anstieg der C14-Radioaktivität in den Plasma-TG-Fettsäuren und im Plasma-TG-Glycerol bzw. dem Anstieg der H3-Radioaktivität in den Plasma-TG wurden die Einbauraten von Plasmaglucose-Kohlenstoff in den Fettsäure-und Glycerolanteil der Plasma-TG bzw. die Veresterungsrate der FFS des Plasmas zu Plasma-TG berechnet. Bei weiteren 6 Personen (3 Hyperlipämiker und 3 Stoffwechselgesunde), denen ebenfalls 40 g Glucose/Std über einen Zeitraum von 3 Std oral verabreicht wurde, wurden dieselben Stoffwechselgrößen bestimmt, wobei jedoch die markierte Glucose der oral verabreichten Glucoselösung zugesetzt wurde, während die markierte Palmitinsäure intravenös infundiert wurde. Aus den mitgeteilten Ergebnissen geht hervor, daß unter den gegebenen Versuchsbedingungen der Großteil des zur Plasma-TG-Bildung benötigten Glycerols aus der Plasmaglucose stammt. Die Einbaurate von Plasmaglucose-Kohlenstoff in Plasma-TG-Fettsäuren war jedoch in allen Fällen außerordentlich niedrig und dürfte demnach selbst unter massiver Zufuhr von Glucose für die Plasma-TG-Bildung gegenüber der Veresterungsrate von FFS des Plasmas zu Plasma-TG quantitativ nur eine untergeordnete Rolle spielen. Es konnten weiters Beziehungen zwischen der Höhe des TG-Spiegels im Plasma und der Einbaurate von Plasmaglucose-Kohlenstoff in Plasma-TG-Fettsäuren bzw. Plasma-TG-Glycerol und zwischen Plasma-TG-Konzentration und Veresterungsrate von FFS des Plasmas zu Plasma-TG nachgewiesen werden. Die beobachteten Korrelationen zeigten jedoch, daß bei den Hyperlipämikern auch unter reichlicher Glucosezufuhr weder die Veresterungsrate von FFS des Plasmas zu Plasma-TG noch die Einbaurate von Plasmaglucose-Kohlenstoff in Plasma-TG, d. h. die Bildung der Plasma-TG, in dem Ausmaß erhöht war, daß dadurch der exzessiv hohe Plasma-TG-Spiegel erklärt werden könnte. Auch diese Untersuchungen scheinen die Hypothese zu stützen, wonach bei diesen Patienten ein ungenügender Abtransport der Plasma-TG zur Hypertriglyceridämie führt.
