    Notes: Summary About 50% of the circulating platelets can be taken from healthy donors by means of the cell separation technique. In order to investigate the effect of cell separation on thrombopoesis bone marrow samples were taken from 12 voluntary donors up to 5 times. The megacaryocytes were analyzed before and after cell separation by morphometric measures and determinations of platelet count and platelet function (adhesion, aggregation, spreading, retraction) were carried out. Removing of large amounts of platelets form the circulation by stimulation of thrombopoesis resulted in an increase of the peripherical platelet count already 48 h after cell separation. Of the platelet functions tested only aggregation induced by collagen was found to be increased after cell separation as compared to that before the procedure.

    Description / Table of Contents: Zusammenfassung Die Plasmaseparation wurde mit Hilfe von Zellulose-Diacetat-Hohlfasermodulen durchgeführt. Die Porengröße der Membranen betrug 0.2 µm und hatte eine obere Ausschlußgrenze von ungefähr 3 Millionen Dalton. Fünf Liter Filtrat konnten durchschnittlich in 80 min separiert werden. Der Proteingehalt des Filtrates lag bei etwa 60% des Proteingehaltes im Blut. Die Realtionen der Immunglobuline unterschieden sich im Plasma und im Filtrat nur gering. Durch eine Plasmaseparation gelang es, die Konzentration an Immunglobulinen auf etwa 40% der initialen Konzentration zu senken.
    Notes: Summary Plasma separation was carried out using a new cellulose diacetate hollow-fibre module. The pore size of the membranes used was 0.2 µm and the cut-off at approximately 3 million dalton. Five litres of filtrate was separated in 80 min. The protein content of the filtrate was 60% of that in the blood plasma. When plasma and filtrate were compared, there was little difference in the proportions of the immunoglobulins. At the end of treatment, the immunoglobulin content of the plasma had been reduced to 40% of its initial concentration.

    Notes: Abstract This study investigated the effect of extracorporal lipid-lowering therapy by low-density lipoprotein (LDL) apheresis on coronary artery disease in a population characterized by early development and rapid progression of atherosclerosis. We treated 32 patients aged between 15 and 63 years with drug-refractory familial hypercholesterolemia, treated once a week by immuno-specific LDL apheresis for 3 years in a controlled prospective and non-randomized trial; 25 patients (14 females and 11 males) completed the study. Noninvasive data were obtained by physical examination, 12-lead ECG and exercise testing. Invasive cardiological data were obtained by cardiac catheterization according to a standardized protocol in four cardiological centers. Left ventricular ejection fraction was calculated using planimetry. Coronary stenoses were measured quantitatively in 23 defined coronary segments by a panel of four investigators with an electronic digital caliper. In addition, overall coronary atherosclerosis was visually qualified. Final decisions on a classification into one of three groups (regression, no change, progression) of coronary atherosclerosis were based on panel consensus. Six cardiac events occurred throughout the study: percutaneous transluminal coronary angioplasty in one patient, coronary bypass grafting in three and two deaths. Statistical analysis of exercise testing yielded no significant change for maximum power and work capacity during the study period. Hemodynamic data revealed no significant change; mean ejection fraction was calculated as 65.8 ± 15.9% at study entry and 67.0 ± 12.7% at completion. Quantitative measurement of 111 circumscribed coronary stenoses showed a mean stenosis degree of 45 ± 26% at entry cineangio-film and 43 ± 22% at final cineangio-film demonstrating no significant change. Eight localized stenoses showed a regression of more than 10% and 11 stenoses a progression of more than 10%. Panel consensus decision for overall coronary atherosclerosis resulted in regression in no patients, no change in 16, questionable progression in 3, definite progression in 5, and undecided in one. We conclude that specific LDL-apheresis therapy can be used effectively for primary and secondary prevention of coronary artery disease in patients with familial hypercholesterolemia. Its beneficial effect was the prevention of further progression of coronary artery disease in the majority of the study population.

    Notes: Das Ziel extrakorporaler Behandlungsverfahren ist eine Verbesserung der Hämodynamik über eine Beeinflussung rheologischer Parameter.     Description / Table of Contents: Zusammenfassung Die Arbeit berichtet von einem 5 Jahre alten Mädchen mit akuter lymphatischer Leukämie, das nach Transfusion von 2 Granulozytenkonzentraten das klinische Bild einer Graft-versus-Host-Reaktion zeigte. Die Diagnose stützt sich auf den histologischen Befund der Hautbiopsie sowie auf den Ausschluß anderer Erkrankungen ähnlicher klinischer Erscheinung. Wir halten die Bestrahlung von Granulozytenkonzentraten auf Grund dieser Beobachtungen für notwendig.
    Notes: Summary Graft-versus-Host disease following the transfusion of leukocytes from normal donor occured during remission-reinduction therapy in a 5 years old girl with acute lymphocytic leukemia. The clinical picture consisting of rash, diarrhea and somnolence was congruent with the diagnosis. Graft-versus-Host disease was confirmed by histology of skin biopsy and exclusion of other causes. The child survived the disease. We recommend the irradiation of leukocyte concentrates before transfusion.

    Description / Table of Contents: Summary We report three patients with severe Guillain-Barré syndrome (GBS). On clinical examination, no sensory deficit was elicited. Electromyography (EMG) and motor nerve conduction velocity (NCV) studies were indicative of an axonal lesion. In two patients, we found total peripheral conduction block without volitional EMG activity. Sensory NCV and cortical median nerve evoked potentials remained normal. Sural nerve biopsy revealed unequivocal alterations of sensory nerve fibers, some in the form of primary demyelination. Therefore, these cases must bei classified as combined motor-sensory syndromes despite the clinical and electrophysiological findings. Although total denervation of the extremity muscles occurred, causing tetraparalysis, this manifestation appears to be attributable to the secondary axonal variant. In contrast to positive reports in literature, immunomodulatory treatment proved ineffective in all three cases.
    Notes: Zusammenfassung Wir beobachteten 3 Patientinnen mit schwer verlaufendem Guillain-Barré-Syndrom (GBS). Bei klinischer Untersuchung waren keine sensiblen Störungen zu eruieren. EMG und motorische Elektroneurographie sprachen für eine axonale Läsion. Bei 2 Patientinnen bestand ein kompletter Leitungsblock motorischer Fasern, dabei war im EMG keine Willküraktivität nachweisbar. Sensible Nervenleitgeschwindigkeiten und kortikale Medianus-SEP blieben auch im Verlauf unauffällig. Die Biopsie des N. suralis konnte aber jeweils eine eindeutige leichte Beteiligung sensibler Nervenfasern, z. T. in Form von primärer Demyelinisierung, belegen. Somit handelt es sich bei diesen Fällen trotz der klinischen und elektrophysiologischen Befunde um gemischt motorisch-sensible Läsionen. Obwohl es im Verlauf zu einer kompletten Denervierung der Extremitätenmuskulatur mit Tetraparalyse kam, sind sie offenbar eher einer sekundär axonalen Variante zuzurechnen. Im Gegensatz zu positiven Literaturberichten blieb die immunmodulierende Therapie in allen 3 Fällen wirkungslos.
