    Notes: Background:  The atopy patch test (APT) was proposed to evaluate IgE-mediated sensitizations in patients with atopic eczema (AE).Objective:  The prevalence and agreement with clinical history and specific IgE (sIgE) of positive APT reactions was investigated in six European countries using a standardized method.Methods:  A total of 314 patients with AE in remission were tested in 12 study centers on clinically uninvolved, non-abraded back skin with 200 index of reactivity (IR)/g of house dust mite Dermatophagoides pteronyssinus, cat dander, grass, and birch pollen allergen extracts with defined major allergen contents in petrolatum. Extracts of egg white, celery and wheat flour with defined protein content were also patch tested. APT values were evaluated at 24, 48, and 72 h according to the European Task Force on Atopic Dermatitis (ETFAD) guidelines. In addition, skin-prick test (SPT) and sIgE and a detailed history on allergen-induced eczema flares were obtained.Results:  Previous eczema flares, after contact with specific allergens, were reported in 1% (celery) to 34% (D. pteronyssinus) of patients. The frequency of clear-cut positive APT reactions ranged from 39% with D. pteronyssinus to 9% with celery. All ETFAD intensities occured after 48 and 72 h. Positive SPT (16–57%) and elevated sIgE (19–59%) results were more frequent. Clear-cut positive APT with all SPT and sIgE testing negative was seen in 7% of the patients, whereas a positive APT without SPT or sIgE for the respective allergen was seen in 17% of the patients. APT, SPT and sIgE results showed significant agreement with history for grass pollen and egg white (two-sided Pr 〉 |Z| ≤ 0.01). In addition, SPT and sIgE showed significant agreement with history for the other aeroallergens. With regard to clinical history, the APT had a higher specificity (64–91% depending on the allergen) than SPT (50–85%) or sIgE (52–85%). Positive APT were associated with longer duration of eczema flares and showed regional differences. In 10 non-atopic controls, no positive APT reaction was seen.Conclusion:  Aeroallergens and food allergens are able to elicit eczematous skin reactions after epicutaneous application. As no gold standard for aeroallergen provocation in AE exists, the relevance of aeroallergens for AE flares may be evaluated by APT in addition to SPT and sIgE. The data may contribute to the international standardization of the APT.

    Description / Table of Contents: Summary Tyrosinase mRNA produced by melanoma cells can be detected by reverse transcriptase-polymerase chain reaction (rt-pcr) with fine-needle tissue punctures. Fine-needle punctures from six suspected skin and lymph node metastases in three patients with malignant melanoma were analysed via rt-pcr for tyrosinase mRNA. All the suspected metastases were surgically removed and histologically examined separately. In each case the rt-pcr results and the histological diagnosis corresponded. This less invasive and highly sensitive method could prove to be a useful alternative in the diagnosis of melanoma metastasis.
    Notes: Zusammenfassung Melanozytäre Zellen unterscheiden sich in ihrem Metabolismus von anderen Zellen durch die Expression des Enzyms Tyrosinase. Bei 3 Patienten mit malignem Melanom konnte an 6 Feinnadelpunktaten von fraglichen Haut- und Lymphknotenmetastasen die Tyrosinase-mRNA mittels Reverser Transkription/Polymerasekettenreaktion (RT/PCR) nachgewiesen werden. Die Veränderungen wurden im Folgenden operativ entfernt und histologisch aufgearbeitet. Die Ergebnisse der RT/PCR korrespondierten jeweils mit der histologischen Diagnose. Diese wenig invasive und sehr sensitive Methode der Feinnadelpunktion mit anschließender molekularbiologischer Untersuchung könnte für die Diagnostik von Melanommetastasen als Alternative zur diagnostischen Exzision bzw. Aspirationszytologie bedeutsam werden.

    Notes: The nature and extent of bacterially induced allergies are difficult to define. Since peptidoglycan, the main component of the cell wall of almost all bacteria, has been available in a highly purified, chemically and immunologically well-defined form, investigation of the allergological significance of this cell component is feasible. Intracutaneous tests were carried out on 181 test, subjects with five different peptidoglycan (PG) preparations from Staphylococcus aureus, Staphylococcus epidermis and Staphylococcus pyogenes. The results of the investigation were compared with the result of determination of serum PG antibodies and serum IgE concentrations. It was shown that test subjects with dual and late reactions to three three different staphylococcal PGs displayed significantly higher PG antibody titers than test subjects with negative reactions. Such a relationship could not he found with the cutaneous reactions to streptococcal PG. The total serum IgE values were very much higher in test subjects with immediate reactions to staphylococcal PG than in test subjects with a negative reaction. Typical Arthus reaction or late granulomatous reactions were not observed. Humoral antibodies are involved at least in part in the elicitation of dual and late reactions. Thus, there are interesting parallels to allergy to fungal spores and organic dusts.

    Notes: Allergy to kiwi, poppy seeds, and/or sesame seeds often occurs in patients with a simultaneous sensitization to nuts and flour. Previously cross reactions have been verified by RAST inhibition. In this study the nature of this cross-reactivity is further characterized by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE), followed by immunoblotting to nitrocellulose. The degree of cross-reactivity among kiwi, sesame seeds, poppy seeds, hazelnuts, and rye grain was found to be very high in the patients studied. The existence of both cross-reacting and unique components was observed; however, the cross-reacting and unique components could be different for different patients.

    Notes: Zusammenfassung Die Einreibung fetthaltiger Schutzsalben vor jeder einzelnen Applikation von Kaltwellentwicklern und die etwa 3 mal im Laufe des Tages angewandte Auftragung von schweißhemmenden Mitteln erwies sich als die empfehlenswerteste Prophylaxe. Diese Methode kann ggf. noch mit Handbädern in H2O2, oder anderen Fixierlösungen kombiniert werden (Cave: Bromat o. ä. Überempfindlichkeiten?). — Am besten wäre es natürlich, die Friseure würden gar nicht direkt mit den irritierenden Substanzen in Berührung kommen, sondern Handschuhe tragen bzw. Kaltwellentwickler anwenden, die nach dem Aufrollen der Haare auf die Wickel ohne Hautkontakt aufgesprüht werden können (Cascade-Schwarzkopf, Ellerbroek). Für die notwendige fettenthaltende Schutzsalbenform gelten in diesem beruflichen Bereich — zumal wegen des nicht vermeidbaren Umganges mit Wasser und Seifen — in übertragenem Sinne letztlich die Folgerungen aus dem von Gottron stammenden Satz: “Wir haben von der Antike Waschung und Bad übernommen, Salbung und Ölung jedoch vergessen—.”
