    Description / Table of Contents: Summary Micromorphology and distribution of intranuclear spiral bodies, so called “spheroids” in various tissues of the normal human body and several skin diseases are described. In skin lesions of patients affected by Herpes Zoster the simultaneous accurence of spheroids and viral elementary bodies was found. Both corpuscles were located in close proximetry in the nucleus. In hypertrophic nuclei of malignant melanoma “giant spheroids” were detected. It is concluded, that there is no relationship between spheroids and specific disease processes nor do they represent a cause-specific differentiation of the karyoplasm. The question of their significance demands further work. According to the system ofLipschütz the spheroids can be grouped under the inclusion bodies II. order, especially intranuclear inclusion of the B-type (Cowdry).
    Notes: Zusammenfassung Nach einleitender Erörterung der Mikromorphologie und des Vorkommens der intranucleären Spiralkörper, die jetzt übereinstimmend von den meisten Autoren „Sphäridien“ genannt werden, in verschiedenen normalen Geweben des Menschen und bei verschiedenen Hautkrankheiten, wird erstmals das gemeinsame Auftreten von Sphäridien und Viruselementarkörpern in enger räumlicher Beziehung in Zellkernen von Zosterläsionen und vonRiesensphäridien in den hypertrophierten Nuclei beim Melanomalignom beschrieben. Die Sphäridien sind weder erregerspezifische Differenzierungen des Karyoplasmas, noch sind sie bestimmten Krankheitsprozessen direkt zuzuordnen. Ihre funktionelle Deutung muß weiteren Untersuchungen vorbehalten bleiben. Systematisch gesehen, gehören diese Inklusionen zu den Einschlußkörpern II. Ordnung des Lipschützschen Schemas, und zwar zu den intranucleären Einschlüssen vom Typ B (im Sinne vonCowdry). Ein Schema verdeutlicht die hier benutzte Terminologie (s. S. 539).

    Notes: Summary The westernblot analysis of 170 patients with HIV-1-infection demonstrated that 47% of the patients in latent stage, 58% of the patients with lymphadenopathy-syndrome and only 25% of the patients with the full-blown picture of AIDS showed the complete pattern of HIV-specific antibody response. This antibody response is mainly directed against the env-encoded envelope proteins gp160, gp120 and gp41, against the gag-encoded core proteins p55, p24 and p17 as well as against the pol-encoded enzymatic proteins p66, p51 and p31. Antibodies against gp160 and gp120 were present in nearly all patients, whereas the prevalence of the other antibodies decreased with the stage of the disease. Statistical significant differences were found particularly between patients with LAS or AIDS respectively. Antibodies against p17 were detected in 74% of the patients with LAS but only in 25% of the patients with AIDS. The lack of antibodies against p17, p24 or p51 was significantly associated with lower mean CD4/CD8-ratios (p〈0,007) and higher mean serum levels of IgA (p〈0,001) and beta-2-microglobulin (p〈0.001). One third of the patients with LAS and this reduced pattern of antibody response developed AIDS within six months. These results demonstrate that the detection of antibodies against p17, p24 or p51 is of prognostic importance. A serological profile which lacks the antibody response against at least two of those three viral antigens indicates a progression of the disease activity.

    Description / Table of Contents: Summary Electronmicroscopical, histochemical and biochemical investigations were carried out in a case of socalled Reticulosarcomatosis of the skin (Gottron). The electronmicroscopical results show a typical morphological substrate of the tumor tissue. It is composed out of multinuclear cell complexes containing individual mononuclear cells. There is only one single contured membrane of low optical density between the mononuclear individual cells and the multinuclear giant cell formations. It is suggested that the individual cells loose their plasmolemms and murge into the environmental giant cell formations. The almost complete absence of nuclear membranes, the high content of free ribosomes in the cytoplasma, the diminution and the morphological alteration of the mitochondria points towards a high metabolism of these tumor cells mainly directed to the production of cell-own proteins. Histochemically the tumor cells show a high activity of lactate dehydrogenase as a glycolytic enzyme. Histochemical reactions of enzymes of the citric acid cycle and the cytochrome-system were relatively weak. Quantitative biochemical analysis revealed relatively high activities of enzymes of glycolysis and pentose-phosphate-shunt and comparatively low activities of enzymes which are bound to mitochondria like NAD-linked glutamate dehydrogenase or restricted for special metabolic pathways. The results of the experimental investigations show a good correlation between the morphological and biochemical organization of tumor cells in socalled Reticulosarcomatosis (Gottron).
    Notes: Zusammenfassung Es wird über elektronenmikroskopische, histochemische und biochemische Untersuchungen bei sog. Reticulosarkomatose der Haut (Gottron) berichtet. Die elektronenmikroskopischen Befunde zeigen, daß das Tumorgewebe ein charakteristisches feingewebliches Substrat besitzt. Es setzt sich zusammen aus vielkernigen Zellkomplexen und in diesen gelegenen einkernigen Individualzellen. Die einfach ausgebildete und schwach konturierte Membran zwischen einkernigen Einzelzellen und mehrkernigen riesenzellartigen Zellformationen spricht dafür, daß die Einzelzellen unter Verlust ihres Plasmalemms in die sie umgebenden riesenzellartigen Zellformationen aufgehen. Das weitgehende Fehlen von Kernmembranen, die hohe Zahl von freien Ribosomen und die Verminderung sowie morphologische Alteration der Mitochondrien sprechen für einen hohen Stoffwechsel der Tumorzellen, der vorwiegend auf die Produktion zelleigenen Proteins ausgerichtet ist. Histochemisch waren die Tumorzellen charakterisiert durch einen starken Reaktionsausfall auf das glykolytische Enzym Lactatdehydrogenase. Der histochemische Reaktionsausfall auf Enzyme des Citronensäurecyclus und der Atmungskette war vermindert. Die quantitativ-biochemischen Untersuchungsergebnisse zeigten relativ hohe Aktivitäten von Hauptkettenenzymen und des Pentosephosphat-Shunts bei vergleichsweise niedrigen Aktivitäten von Enzymen, die mitochondrial lokalisiert sind (NAD-spezifische Glutamatdehydrogenase) oder speziellen Stoffwechselwegen zuzuordnen sind.

    Description / Table of Contents: Zusammenfassung Die chronische hepatische Porphyrie (CHP) als chronische hepatische Störung des Porphyrinstoffwechsels ist mit einem chronischen Leberschaden regelmäßig assoziiert, kommt in latenten und manifesten Phasen vor und führt bei ihrer klinischen Manifestation zu Hautsymptomen: Porphyria cutanea tarda. Die primäre enzymatische Störung betrifft die Uroporphyrinogen-Decarboxylase (UD; EC 4.1.1.37). Das Enzym wurde bei 107 Patienten mit „sporadischer“ CHP (29 Frauen, 78 Männer) in den Erythrozyten, darunter bei 17 CHP-Patienten (12 latent, 5 manifest) auch im Lebergewebe untersucht. Die hepatische UD-Aktivität war in allen Fällen um 39 bis 57% gegenüber Kontrollen vermindert (p〈0,001). Die Aktivität der UD in den Erythrozyten in 47 Fällen (44%) um 30 bis 55% gegenüber den Kontrollen erniedrigt (p〈0,001). In dieser Gruppe befanden sich 70 Patienten (darunter 30% Frauen) mit klinisch manifester CHP, von denen 43% (12 Frauen, 18 Männer) eine deutlich herabgesetzte UD-Aktivität in den Erythrozyten gegenüber einer Kontrollgruppe (n=96) aufwiesen (p〈0,001). Bei Patienten mit diesem Enzymbefund wird eine hereditäre Anlage angenommen, die erst im Zusammenwirken mit einem Leberschaden, mit Alkohol und/oder östrogenen zu pathobiochemischen und klinisch-dermatologischen Symptomen führt. Haut- und Leberbefunde sind bei der hereditären und nicht-hereditären Form der CHP nicht verschieden. Familienstudien bei der hereditären CHP lassen einen autosomal dominanten Erbgang des Enzymdefekts in den Erythrozyten und latente CHP-Phasen anhand pathologischer Porphyrinurien bei Familienangehörigen erkennen. Nach den vorliegenden Untersuchungen scheinen ca. 40% der „sporadischen“, also nicht-familiär auftretenden Fälle von CHP hereditär vorbedingt zu sein, wobei der UD-Defekt bei der hereditären Form außer in der Leber auch in nicht-hepatischen Zellen vorhanden ist. Eine Störung der hepatischen UD ist generelle Voraussetzung für die Entwicklung einer CHP. Für das Vorkommen einer nicht-hereditären, erworbenen, hepatotoxischen Form spricht, unabhängig von dieser Studie, die chemisch durch Hexachlorbenzol induzierbare CHP bei Mensch und Tier, die pathobiochemisch der nicht-hereditären CHP bei Alkohol-Leber-Syndromen analog ist. Bei 5–10% aller chronischen Leberkranken ist mit einer CHP zu rechnen. Mit Porphyrinbiochemogrammen im Urin werden nicht nur manifeste, sondern auch latente Formen der CHP am sichersten diagnostiziert.
    Notes: Summary Chronic hepatic porphyria (CHP), as a chronic hepatic disturbance of porphyrin metabolism, is regularly associated with chronic liver damage. It appears in latent and manifest stages and in the clinical manifestation leads to cutaneous symptoms: porphyria cutanea tarda. The primary enzymatic disturbance involves uroporphyrinogen decarboxylase (UD; EC 4.1.1.37). The enzyme was studied in 107 patients with “sporadic” CHP (29 women, 78 men) in red cells, and in 17 CHP patients (12 latent, 5 manifest) in liver biopsy tissue, too. The hepatic UD activity was diminished by 39–57% in all cases compared to controls (p〈0.001). The activity of red cell UD in 47 cases (44%) had decreased by 30 to 55% compared to the controls (p〈0.001). In this group, there were 70 patients (of which 30% women) with clinically manifest CHP, of which 43% (12 women, 18 men) presented a marked decrease in UD activity in erythrocytes, compared to the control group (n=96);p〈0.001). In patients with this enzyme level one supposes an inherited predisposition to be present which only becomes apparent with concommitant liver damage, alcohol and/or estrogens, leading to pathobiochemical and clinical-dermatological symptoms. Skin and liver findings are not different in both inherited and non-inherited forms of CHP. Family studies allow to detect, in hereditary CHP, an autosomal dominant genetical defect of the enzyme in erythrocytes, and to detect latent CHP phases in relatives on the base of pathological porphyrinurias. According to the studies made up to now, about 40% of „sporadic“, i.e. not family bound cases of CHP, seem to have inherited predisposition; the UD defect in the hereditary form is found not only in the liver but also in extra-hepatic cells. A disturbance of hepatic UD is a general premise for the development of a CHP. The occurrence of the non-inherited, acquired, hepatotoxic form is proved, apart from this study, by the inducibility of CHP in man and animals chemically by means of hexachlorobenzene. Pathobiochemically it is analogue to non-inherited CHP in alcohol liver syndromes. In 5 to 10% of all chronic liver patients one must reckon with a CHP. Porphyrin biochemograms of urine allow the safest diagnosis not only of manifest, but also of latent forms of CHP.

    Description / Table of Contents: Summary The effect of psoralen plus long wavelength ultraviolet light (UVA) on3H-thymidine uptake of PHA stimulated human lymphocytes was investigated. PHA induced lymphocyte transformation was inhibited by the combined action of psoralen and UVA irradiation in a dose related manner. Inhibition of DNA-synthesis occurred at concentrations of psoralen that can be expected in the serum of patients treated by systemic photochemotherapy. No effect was noted at these psoralen concentrations in the absence of UVA irradiation. Also did UVA irradiation in the absence of psoralen not inhibit3H-thymidine incorporation into PHA stimulated lymphocytes.
    Notes: Zusammenfassung Es wurde die Wirkung von Psoralen zusammen mit langwelligem ultraviolettem Licht (UVA) auf den3H-Thymidin-Einbau PHA-stimulierter humaner Lymphocyten untersucht. Die PHA-induzierte Lymphocytentransformation wurde durch Psoralen plus UVA gehemmt, abhängig sowohl von der Psoralenkonzentration als auch der Einwirkdauer der UVA-Bestrahlung. Die Hemmung der DNA-Synthese in den stimulierten Lymphocyten erfolgte bei Psoralenkonzentrationen wie sie auch im Serum von Patienten erwartet werden können, die sich einer systemischen Photochemotherapie unterziehen. Ohne UVA-Bestrahlung war bei diesen Psoralenkonzentrationen kein Effekt auf die DNA-Synthese nachweisbar. Ebenso erwies sich alleinige UVA-Bestrahlung ohne Psoralenzusatz als wirkungslos auf das Wachstum PHA-stimulierter Lymphocytenkulturen.

    Notes: Zusammenfassung An einem Patientengut von 45 Patienten mit Psoriasis verschiedenster Ausdehnung und unterschiedlichen Eruptionsphasen wurde die Frage untersucht, ob sich bei diesem Krankheitsbild Störungen im Acetalphosphatidstoffwechsel nachweisen lassen. Bei der Psoriasis zeigt die Plasmalreaktion einen von Fall zu Fall in weiten Grenzen schwankenden Reaktionsausfall in der akanthotischen Epidermis. In der parakeratotischen Hornschicht ist die Plasmalreaktion stets negativ mit Ausnahme eines starken Reaktionsausfalls in den Leukocytenansammlungen Munroscher Abscesse. Die Serumplasmalogenwerte schwanken fast ausnahmslos innerhalb des Normalbereichs, desgleichen die verschiedenen anderen Lipoidfraktionen (Lipoidphosphor, Esterfettsäuren, freies Cholesterin, Estercholesterin). Es besteht keine Beziehung zwischen der Höhe des Serumplasmalogenspiegels und dem Reaktionsausfall der Plasmalreaktion in der psoriatischen Epidermis.
