Es ist wahrscheinlich nicht so selten, wie die bisherigen Fallzahlen annehmen lassen. Es werden zwei Fälle mit typischen klinischen und pathologisch-anatomischen Befunden dargestellt.
    Notes: Summary The combination of diarrhea with an islet cell tumor (Verner-Morrison Syndrome) is a clinical picture presenting many diagnostic problems. It is probable that larger number of cases will be encountered with more widespread knowledge of this disease. The clinical features and the pathologic-anatomical findings of two characteristic cases are reported and discussed.

    Description / Table of Contents: Zusammenfassung Das Verner-Morrison-Syndrom ist ein eigenständiges Krankheitsbild mit typischen klinischen Befunden, welche bei einem Inselzelltumor des Pankreas auftreten. Es wurde bisher in über 60 Fällen beobachtet. Im Vordergrund stehen Diarrhcen, welche ursächlich nicht abzuklären und therapieresistent sind, zusammen mit einer Hypokaliämie, bei nicht erhöhten Magensäurewerten und fehlendem Ulcus. Daneben finden sich inkonstant weitere Veränderungen (diabetische Stoffwechsellage, Calciumspiegelerhöhungen, Hauterscheinungen). Die Diagnose kann nur aufgrund desklinischen Bildes gestellt werden. Bei Verdacht muß ein Pankreastumor röntgenologisch oder durch Laparotomie gesucht und gegebenenfalls entfernt werden. Pathologisch-anatomisch finden sich ein Inselzelladenom oder Carcinom — seltener Inselzellhyperplasien. Es gelang bisher nicht, die Zellen mit einer der bekannten und cytochemisch oder elektronenmikroskopisch definierten endokrinen Zellarten zu identifizieren. Die Produktion eines Peptidhormones, welches die klinische Symptomatik auslöst, erscheint als sicher. Diskutiert werden Sekretin, gastric inhibitory polypeptide, vasoactive intestinal polypeptide und Hormonkombinationen. Die Befunde auf diesem Gebiet sind noch widersprüchlich. Sowohl zur formalen, als auch zur kausalen Genese der Inselzelladenome gibt es bisher nur unvollständige und unbestätigte Theorien.
    Notes: Summary The Verner-Morrison Syndrome is a clinically defined entity caused by an islet cell tumor of the pancreas. More than 60 cases have been described so long. The syndrome is characterized by diarrhea, hypokalemia and hypochlorhydria. In addition to a diabetic disposition, raised calcium levels and skin alterations may be present. The diagnosis is a clinical one. A pancreatic tumor should be searched for and removed. Morphologically a benign and a maligne islet cell tumor or a diffuse hyperplasia of the islets of Langerhans can be found. Until now identification of the tumor cells has not been possible. There seems no doubt that the tumor cells produce a peptide hormone. Secretin, gastric inhibitory polypeptide, vasoactive intestinal polypeptide and combinations of hormones are discussed. The results are contradictory. Theories concerning the formal and causal pathogenesis are only incomplete and unproved up to now.

    Notes: Semiempirical quantum chemical calculations at the modified-neglect-of-diatomic-overlap self-consistent-field level are performed for differently charged clusters of composition LixC60 with x=0...14. The ground state energies of various isomers are calculated to find the most stable configuration for each cluster stoichiometry. The energies required to remove a Li-atom from these configurations are calculated to determine abundance spectra of distributions of heated LixC60 clusters. These spectra show intensity anomalies at x=6+n (where n=0...+2 is the cluster charge), interpreted to be of electronic origin, and at x=12, interpreted to be of geometric origin. Identical anomalies are observed in experimentally obtained mass spectra of LixC60 clusters.

    Notes: Tissue markers of potential malignancy have been sought for many years. Cell surface markers, particularly blood group and histocompatibility antigens, have shown great promise and several squamous carcinoma antigens have been identified - but not fully studied in potentially malignant lesions. Growth factors and receptors also need further study. Cytoplasmic markers of potential malignancy have been examined and, of these, keratins, filaggrin, and some carcinoma antigens show most promise. Nuclear analyses have promise but are time-consuming and expensive. Image cytometric analyses appear to be sensitive and predictive: oncogene and tumour suppressor analyses remain to be fully evaluated. New investigative techniques at the cellular and molecular level show increasing promise at defining potentially malignant oral epithelial lesions but more prospective studies arc required.

    Notes: The local immune reaction of progressive chronic periodontal disease may be particularly influenced by macrophages and macrophage-derived factors. Among these substances the prostaglandins and lysosomal enzymes may play an important pathogenetic role. Parallel immunohistochemical and radioimmunological studies were done to investigate the relationships of the immune-competent cells and the inflammatory mediators in gingival tissues. The radioimmunological analysis revealed that prostaglandin E increases markedly in the established gingival lesions. Immunohistochemically prostaglandin E was mainly localized within macrophage-like cells. Cytoplasmic lysozyme could be detected in these cells, too. On the other hand, the B-cell response is the prominent feature in established chronic periodontal disease. However, there is apparently a disturbed B-cell reaction as indicated by the irregular IgG-subclass pattern and by the production of mainly monomeric IgA. The possible interactions of macrophages and especially B-cells via prostaglandin E-mediated mechanisms are discussed.

    Description / Table of Contents: Zusammenfassung Bei Ratten im Kaliummangel wurde nach 18stündigem Fasten eine Vergrößerung der A-Zellen in den Langerhansschen Inseln des Pankreas gefunden, während Normaltiere danach eine Degranulierung und Verkleinerung der A-Zellen zeigten. Beziehungen im Hinblick auf den Glucagonstoffwechsel einerseits, den Elektrolytstoffwechsel andererseits werden diskutiert, insbesondere, ob ein durch Kaliummangel hervorgerufener Sekretionsstop für den histologischen Befund eine Rolle spielen könnte.
    Notes: Summary The morphology of the pancreas of rats in severe potassium depletion was studied following 18 hours of fasting. While the exocrine pancreas showed no differences from control animals, the A-cells in the islets of Langerhans of the control animals were decreased in size and degranulated following the fast. In potassium depletion this did not happen; on the contrary the A-cells seemed enlarged. The implications for glucagon secretion and electrolyte metabolism are discussed, with special reference to whether the secretion of glucagon by the A-cells is impaired in potassium depletion.
