    Notes: Summary A 28-year-old male with hereditary angioedema died of an extensive stroke. Autopsy revealed cicatricial aortitis with narrowing of the coronary ostia, myocardial infarctions, and a left ventricular mural thrombus. There was neither acute inflammation of the aorta nor systemic vasculitis. A possible association of the aortitis with the hereditary angioedema is discussed.

    Description / Table of Contents: Summary Poland syndrome consists of unilateral absence of the sternal head of the pectoralis major muscle, ipsilateral symbrachydaktylia, and occasionally associated other malformations of the anterior chest wall, mammilla, and mamma. To our knowledge a simultaneous occurrence with dextrocardia was reported in seven patients. We report on an additional patient with this unusual coincidence. A 59-year old man showed mild symbrachydaktylia of the left hand and deformity of the left part of the anterior chest wall, both present since birth. We found an additional dextrocardia with situs solitus, d-loop, and correctly connected great arteries. Besides arterial hypertension there was no remarkable impairment of the patient's condition.
    Notes: Zusammenfassung Das Poland-Syndrom ist gekennzeichnet durch eine einseitige Fehlbildung im Bereich der ventralen Thoraxwand mit Aplasie des sternokostalen Anteils des M. pectoralis major, eine ipsilaterale Symbrachydaktylie und gelegentlich assoziierte weitere Defekte der Thoraxwand, Mamille und Mamma. Nach unserer Kenntnis wurden bislang insgesamt sieben Fälle des gemeinsamen Auftretens einer Dextrokardie mit einem Poland-Syndrom beschrieben. Wir berichten über einen weiteren Patienten mit dieser seltenen kombinierten Fehlbildung. Bei einem 59jährigen Patienten bestanden seit Geburt eine linksseitige Thoraxdeformität sowie eine Symbrachydaktylie der linken Hand. Wir fanden bei Poland-Syndrom eine Dextrokardie bei Situs solitus, d-loop und korrekt verbundenen großen Arterien. Außer einer Hypertonie waren die Herzkreislauffunktionen nicht beeinträchtigt.

    Notes: Abstract From a total group of 782 cases of intrathoracic sarcoidosis, detected in the German Forces (Deutsche Bundeswehr) from 1965–1969 (incidence 28 per 100,000), the 205 youngest cases of about 20 years of age were compared with 1519 unaffected control soldiers of the same age. The regional distribution did not reveal differences between the North and the South of West Germany. Comparatively low rates of industrialized regions (Nordrhein-Westfalen, Saarland) and big cities (Hamburg, Bremen) can only partly explain the observed regional differences. The highest rate was found for Bayern (49.8 per 100,000), the lowest for Saarland (11.2 per 100,000). Communities of more than 50,000 inhabitants show about half the incidence of small communities, suggesting a rural factor in the etiology. Low rates in communities of less than 500 inhabitants and peak rates in communities of 10,000 – 20,000 inhabitants in the birthplace and life-time residence studies point to a hypothetical urban triggering influence in the development of the disease. Farmers show rates twice as high, and cabinetmakers three times as high as the average rate. Farmers seem to be exposed more to a sarcoidosis agent, whereas in the case of cabinetmakers a facilitating action of wood dust most probably has to be considered. Morbidity rates in the various-sized communities and different professions of another group of patients (92 females and 102 males) of the sanatorium Tönsheide, Schleswig-Holstein, diagnosed from 1955 – 1970, confirmed the results obtained by the Bundeswehr study.

    Notes: Zusammenfassung Die prognostische Relevanz von Grading und Lymphangiosis beim invasiven Mammakarzinom wird in der Regel ohne Berücksichtigung des Tumortyps beurteilt. Nach unseren Ergebnissen haben diese Kriterien ihre größte Aussagekraft bei duktalen und dukto-lobulären Karzinomen, jedoch nicht bei lobulären und medullären Karzinomen. Eine dem neuen Tumortyp angepaßte Aussage kann die grundsätzliche Hilfestellung der Morphologie für die Diagnose und Therapie des Mammakarzinom erweitern.

    Notes: Abstract 1. Stimulation within the amygdaloid complex elicited two different behavioural and cardiovascular patterns of response. In the awake animal stimulation of the magnocellular part of the basal amygdala resulted in a defence reaction. Stimulation parameters eliciting only the first stage of this behaviour induced a moderate tachycardia and a shortlasting decrease in arterial pressure followed by an increase. Aortic blood flow measured distal to the renal arteries was increased; hence calculated peripheral vascular resistance was decreased. The increase in blood flow was blocked by both atropine sulphate and methyl-atropine. 2. Stimulation of the central amygdala in the awake cat elicited a behaviour which was different from a typical defence reaction. The response appeared to be more similar to attack or a superimposition of attack on the defence reaction. Stimulation of this area with lower stimulus intensities elicited a cardiovascular pattern consisting of tachycardia, which was sensitive to β-blockade, increased arterial pressure, which remained elevated for the entire period of stimulation and increased calculated peripheral resistance. These latter changes were blocked by application of phenoxybenzamine as well as of phentolamine. Moreover, there was a marked post-stimulation bradycardia, which was abolished by application of atropine sulphate. 3. Intravenous administration of sodium pentobarbitone did not alter the cardiovascular patterns induced by stimulation of the central or basal amygdala if the dose of pentobarbitone did not exceed 25 mg/kg. However, stimulus intensities had to be increased by a factor of 2 to 3 in order to elicit comparable cardiovascular responses. If the dose of pentobarbitone exceeded 25 mg/kg, the well-defined cardiovascular patterns elicited by stimulation of the two different amygdaloid nuclei could not be observed. In contrast, stimulation of sites within the ventromedial hypothalamus, performed in the same animal, showed that the cardiovascular responses could be elicited also after application of 40–45mg/kg of pentobarbitone. Hence it is concluded that anaesthetic agents such as pentobarbitone act largely on neuronal elements between the amygdala and hypothalamus.

    Notes: Abstract. We define the sharply bounded hierarchy, SBH(QL)}, a hierarchy of classes within P , using quasilinear-time computation and quantification over strings of length log n . It generalizes the limited nondeterminism hierarchy introduced by Buss and Goldsmith, while retaining the invariance properties. The new hierarchy has several alternative characterizations. We define both SBH(QL) and its corresponding hierarchy of function classes, and present a variety of problems in these classes, including ≤ ql m -complete problems for each class in SBH(QL). We discuss the structure of the hierarchy, and show that determining its precise relationship to deterministic time classes can imply P≠ PSPACE . We present characterizations of SBH(QL) relations based on alternating Turing machines and on first-order definability, as well as recursion-theoretic characterizations of function classes corresponding to SBH(QL).
