    The pharmacokinetics of ceftriaxone and cefotaxime in cirrhotic patients with ascites (1989)

    Notes: Summary We have compared in two separate studies the kinetics of ceftriaxone and cefotaxime in 8 cirrhotic patients with ascites and 8 control subjects after a single 20 min intravenous infusion of 1 g of each drug. The apparent volumes of distribution (Vz) were found to be significantly higher in cirrhotics than in control subjects (0.87, versus 0.49, l·kg−1, for cefotaxime and 0.23 versus 0.13 for ceftriaxone). The elimination kinetics of ceftriaxone were similar in the two groups. In contrast, the total and non-renal clearances of cefotaxime were reduced in cirrhotic patients. The two drugs rapidly entered the ascitic fluid. Peritoneal concentrations of ceftriaxone were higher than 7 µg·ml−1 from the second hour after the infusion and were 8.9 µg·ml−1 at 24 h. Peritoneal concentrations of cefotaxime were higher than 4 µg·ml−1 from 0.5 to 8 h after the infusion.

    Notes: Summary Fibrin ring granuloma is characterized by a fibrinous ring surrounding a central fat vacuole. It has been found in the liver and bone marrow of patients with Q fever, and occasionally with visceral leishmaniasis, cytomegalovirus, Epstein-Barr virus,Staphylococcus epidermidis infections, Hodgkin's lymphoma, and hypersensitivity to allopurinol. We describe a case of serologically confirmed viral hepatitis A with this lesion in the liver biopsy. A false positive anti-hepatitis A virus IgM result has been excluded. This is, to our knowledge, the second reported case of type A hepatitis with hepatic fibrin ring granulomas. It confirms that hepatitis A should be included in the differential diagnosis of this lesion.

    Description / Table of Contents: Abstract With this device, the degree of skeletal maturity can be calculated in children and adolescents from a bone radiography of the left wrist and hand. This system is useful in child endocrinology, orthodontics, orthopaedic, surgery and orthodromic re-education. The quotation of 22 bones out of the 29 in the hand can be fed into a microcomputer through a typewriter. Several results can be obtained: bone age, chronological age, statural age and quotation subtotals.
    Notes: Sommaire Cet appareil permet d'effecteur le calcul du degré de maturation squellettique chez l'enfant et l'adolescent à partir d'une radiographie osseuse du poignet et de la main gauche. Ce dispositif trouve généralement ses applications en endocrinologie infantile, en orthodontie, en chirurgie orthopédique et en rééducation orthodromique. Un micro-ordinateur est en liaison avec une imprimante qui permet la cotation de 22 os, des 29 de la main, et l'impression des résultats (âge osseux, âge chronologique, âge statural, sous-totaux de coation).

    Evidence for an association between cholelithiasis and diverticular disease of the colon (1981)

    Notes: Abstract A prospective case-control study was carried out to determine if there was a true association between diverticular disease of the colon and cholelithiasis. The prevalence of gallstones was higher in 102 patients with diverticular disease of the colon (45%) than in 102 control subjects, matched for age, sex, body weight, and number of pregnancies (22%) (P〈0.001). The difference was only significant in women. Although these results obtained in hospitalized patients cannot be generalized to the population at large, this study suggests an association between diverticular disease of the colon and gallstone disease. The possibilities of referral bias, detection bias, and chance are considered, and the mechanism of this association is discussed, with particular reference to the role of a fiber-depleted diet.
