    Notes: Abstract A case of histologically proven skeletal metastases from pineoblastoma is presented. The primary tumor was initial1y treated by surgery and radiotherapy. To the best of our knowledge this is the third reported case of skeletal metastases arising from such primary tumor.

    Notes: Recalcification of osteolyses can be determined by the use of dual-energy CT-scanning. Quantitative CT-scans have been performed in a group of 11 female patients with osteolytic metastatic spread of mammary carcinomas under systemic antineoplastic treatment. During their treatment with either chemotherapy or hormones these patients have been investigated every two weeks in a total observation period of three months. Additional to every CT-scan (n = 77) a conventional radiograph was taken. In the course of treatment it was possible to give reliable statements with regard to the response to therapy up to 8 weeks earlier by the use of quantitative CT-scans compared to conventional radiographs.

    Description / Table of Contents: Zusammenfassung Das Auftreten von Hirnmetastasen stellt für Patienten ein eingreifendes und oft lebensbedrohendes Ereignis dar. Eine adäquate Therapie erfordert besonnenes Handeln unter Abwägung aller Behandlungsoptionen. Multiple Hirnmetastasen sollten insbesondere bei schlechtem Allgemeinzustand einer palliativen Ganzhirnbestrahlung zugeführt werden. Bei Patienten mit einem günstigen Prognoseprofil, solitären oder wenigen Hirnmetastasen sind operative Exstirpation und Radiochirurgie gleichwertige Behandlungsmethoden, wobei die stereotaktische Einzeitbestrahlung nicht invasiv ist und auch in inoperablen Regionen durchgeführt werden kann. Der Stellenwert der zusätzlichen Ganzhirnbestrahlung ist noch umstritten. Die Überlebenswahrscheinlichkeit wird durch diese Kombination nicht verbessert, wohl aber kann die zusätzliche Ganzhirnbestrahlung das Auftreten neuer Hirnmetastasen verhindern.
    Notes: Summary Brain metastases are life-threatening events for patients. Adequate therapeutic strategies demand thoughtful planning and careful weighting of all treatment options. Multiple brain metastases should be treated in palliative therapy with whole-brain irradiation. In patients with a favourable prognosis and solitary or few brain metastases, surgical excision and radiosurgery are comparable adequate treatment options. Radiosurgery is an non-invasive procedure that can also be performed in non-operable brain areas. The value of additional whole-brain irradiation is under discussion. It has no influence on survival, but the frequence of new brain metastases may be reduced.
